Combination of various anti-aging techniques can give a much more significant effect in preventing facial aging than the use of single techniques. We are talking about the use of fillers and sebum in combination with hardware techniques.

For the readers of estet-portal.com, we offer one of the publications of the Belgian plastic surgeon Alex Verpaele, Director of the Coupure Center for Plastic Surgery (Ghent), in which he describes the results of SNIF facial skin rejuvenation technology.

Minimizing the appearance of fine lines is an extremely important part of the treatment, which may include resurfacing, intradermal filler injections, or a combination of the two. Intradermal fat injections can serve as an inexpensive and safe substitute for classic intradermal fillers.

We conducted a study to evaluate the suitability, validity and safety of the sharp needle intradermal fat grafting (SNIF) technique.

The results were evaluated by clinical examination and photographs of the patients. The improvement was achieved for more than a year, which exceeds the expected duration of absorbable dermal fillers (3-6 months). No significant complications were noted.

Results of fat injections during SNIF procedure

Of the 250 patients who participated in our study, 222 (89%) were women and 28 (11%) were men. The mean age of the patient was 53 years (cf. 35-71 years). A total of 282 procedures were performed for 250 patients (mean 1.1 procedures per patient). In this study, the areas most frequently used for injection were the perioral and glabella.

32 patients underwent a second intradermal fat grafting, on average 8 months after the first procedure. Refilling was carried out in case of a suboptimal result, at the request of the patient or on the recommendation of the therapist. In 182 SNIF patients, the procedure was performed in combination with a cranial lift (MACS). In 68 cases, SNIF was performed as a separate procedure. Among individual cases, 35 patients underwent fat grafting as a "maintenance procedure" after a previous (MACS) lift. For these patients, the SNIF procedure was performed on average 4.25 years after the MACS facelift.

31% (n=78) of 250 patients underwent Erbium:YAG laser resurfacing in combination with the SNIF procedure. The mean follow-up time was 14 months. 96 patients were examined for at least 1 year with stable results during this period; in this subgroup, refilling was not clinically necessary and was not required by one of these patients. The results obtained from the use of classic dermal fillers lasted 3-6 months.

Based on the information from this study, it can be argued that the results of our SNIF technique are long-term. No indurations, cysts, infections, or other serious complications such as necrosis or fat embolism due to intravascular injection were observed during the study. Bruising occurred in 38% of patients (n = 95), redness and swelling in 9% of patients (n = 23), all problems were resolved within 4 days after the procedure.

There were no skin irregularities among the patients. Weight gain by patients did not cause hypertrophy of fat grafts. However, despite the fact that 1 patient significantly lost weight, the result of the correction remained stable.

Clinical results are shown in fig. 4-8.

Pay attention to the stable and long-term correction of wrinkles through the use of a combination of procedures. Such results cannot be achieved by single treatments.

Thus, the SNIF technique is a safe, effective and inexpensive alternative to procedures that use synthetic fillers, but its results are longer lasting. Contrary to popular belief, intradermal grafting can be performed safely using a sharp needle, as it is done in the superficial plane of the skin while the needle is withdrawn. Fat harvesting and injection techniques must be carefully followed in order to achieve optimal results and prevent complications.
