The hectic pace of life, fatigue and sleep disturbances leave their mark on the modern generation, regardless of a person's age, gender or origin. These factors accelerate the aging of the skin, and also contribute to the appearance of unaesthetic dark circles under the eyes. There are a number of methods to combat this problem, which are characterized by varying degrees of effectiveness. Among them are:

  • various types of peels;

  • other therapeutic and surgical methods.

The occurrence of wrinkles in the infraorbital region is one of the most common reasons for visiting aesthetic medicine specialists. As a rule, problems in this area are accompanied by a loss of bone density, a decrease in the vertical and an increase in the horizontal size of the skull, as well as an increase in the size of the bony orbit. The upper jaw, as it were, retracts, and the fatty body of the cheek falls down, causing the appearance of nasolabial folds. There is a redistribution of subcutaneous fat, loss of tissue volume in the periorbital and maxillofacial zone. The presence of hypertrophy of the infraorbital fat is accompanied by the appearance of fat sacs. Soft tissue changes lead to weakening of the orbital septum and subsequent protrusion of adipose tissue into the infraorbital region.

Loss of volume in the infraorbital region also releases previously unnoticed fat pads in the infraorbital region or "eyelid bags". The ligament in the region of the lacrimal sulcus is compressed even more, passing obliquely from the lateral part of the nose at the level of the inner corner of the eye down to the anterior part of the cheek under the middle part of the eyelid. Downward displacement of adipose tissue in the infraorbital region towards a weakened orbital septum results in a deeper and wider orbit and a biconvex deformity of the lower eyelid. Loss of tissue volume between the orbital muscle of the eye and the nearby skin of the lower eyelid leads to convergence of tissues and darkening of the thin skin of the infraorbital region, which makes the eyes look tired.

Anatomical validity

Anatomy defines the tear trough ligament in different ways. Dr. Wong and co-authors of the study, after dissecting 48 half-faces of cadavers, described the ligament in the region of the lacrimal sulcus as a true osseous-cutaneous ligament, which begins in the middle, at the level of the insertion of the medial central tendon, which is located immediately below the anterior lacrimal crest, approximately to the midline of the pupil. Histological evaluation confirmed that the ligament is similar to the zygomatic ligament.

The use of hyaluronic acid-based fillers is an effective and non-surgical way to correct dark circles in the periorbital region, as well as tear trough deformities.

During the study, 14 patients received a stabilized hyaluronic acid filler called APRILINE®; normal. It is a non-permanent, absorbable, injectable material for the correction of soft tissue defects. APRILINE® Normal is produced by bacterial fermentation and stabilized with a highly concentrated aqueous solution without the addition of other acids or alkaloids. Using this technology, a specific spatial structure of hyaluronic acid molecules is created through the effective binding of DHEBD (BDDE). The concentration of hyaluronic acid on the APRILINE® filler; Normal is 23 mg/ml. The amount of filler used for patients is 0.6 – 0.8 ml.

Filler injection technology

A selection was made of 14 patients who had no contraindications to hyaluronic acid and filler injections. Standard procedures were carried out: all patients were photographed in the necessary lighting on the day of the procedure, as well as 30 and 90 days after the procedure. Written informed consent was obtained from all patients. The procedure for the introduction of fillers was as follows:

  • patients leaned at an angle of 45o, because in this position the deformation of the lacrimal trough is better visible;
  • ice is used as anesthesia, which is applied for a few seconds to the area of ​​skin between the eyelid and cheek;
  • the treated area is cleaned and disinfected with Biseptin spray and a sterile compress;
  • The insertion procedure is performed in a treatment room using disposable equipment;
  • filler based on hyaluronic acid APRILINE® Normal is administered with a 27 gauge needle at doses of 0.6 – 0.8 ml per patient. The needle is inserted at an angle of 90° to the patient's skin, deep into the orbicular muscle of the eye and periosteal tissue. The first injection is administered, as a rule, at a distance of 0.5 cm from the medial canthus. In general, 3-4 filler injections are injected under the orbital ridge. To prevent damage to the neurovascular bundle, it is recommended to inject the filler, avoiding the prominence of the infraorbital nerve. Initially, 0.3 – 0.5 ml of filler. The injection area is gently massaged for optimal distribution of the filler.
