Combination of injection and hardware treatment methods for skin lifting

Cosmetology and aesthetic dermatology occupy an important place in the complex solution of aging problems. Skin aging has an important social component, which is why "rejuvenating" procedures in dermatocosmetology are always popular.

"Rejuvenating" procedures include both methods that allow solving the problems of photoinduced skin aging, and methods aimed at combating age-related changes. There are also methods that allow treating age-related skin pathology, and methods that solve purely aesthetic problems. At the same time, a dermatocosmetologist is not limited to the skin. Special methods allow you to restore the lost volume of the hypodermis (volumetric fillers), eliminate persistent spasm of mimic muscles (botulinum toxin preparations), i.e. solve the problems of aging in a complex.

Gravity descent of soft tissues (ptosis) as one of the elements of skin aging is traditionally a problem of aesthetic surgery. Despite this, the use of dermatocosmetic procedures can be interesting, especially in the early stages of ptosis.

Pathophysiology of age-related ptosis

The dermis undergoes the greatest changes in the senile character, namely its collagen and elastic fibers.

It has been established that due to the decrease in the level of estrogen, in women in the first 5 years of menopause, the quantitative and qualitative composition of collagen changes significantly. At the same time, atrophy occurs in the hypodermis, which is associated with vascular sclerosis. Clinically, this process is manifested by an increase in relief:

  • deep nasolacrimal grooves are formed, the cheeks become "hollow", "bags" appear under the eyes.
  • The subcutaneous tissue on the face and neck is reduced.
  • The contour of the face along the arch of the lower jaw becomes blurred, so-called "bulldog cheeks" are formed (photo 1).

Types of senile changes in the face and neck are important for deciding on the choice of the method of therapy, drawing up, if necessary, a plan for surgical intervention and predicting postoperative results. For this purpose, early stages of aging are distinguished (premature aging and an early form of natural aging): "tired face", "wrinkled face", "deformed face", as well as the fourth combined type and the fifth – muscular. The late stage of aging refers to the so-called "senile, or exhausted face".

The use of combinations of various methods of aesthetic dermatology for the purpose of skin lifting can significantly increase the effectiveness of the treatment of early stages of soft tissue prolapse. In this case, hardware and injection methods of treatment are most often combined.

Microcurrents + mesotherapy

In the initial stages of ptosis treatment, stimulating procedures are of particular importance. These procedures include microcurrent therapy.

This is the only method that works at the level of the cell membrane. The action of microcurrents leads to the normalization of the potential of the cell membrane, the opening of ion channels, the activation of metabolism, and the enhancement of ATP synthesis.

When changing the architectonics of the face, oval, the appearance of deep folds and wrinkles, as well as the formation of ptosis of the upper eyelid, hernia-like protrusions of the lower eyelid, microcurrents restore the original state and length of the facial muscles, contribute to modeling the oval of the face. This effect causes a mild tonic effect on the mimic muscles of the face, while creating conditions for the synthesis of new collagen and elastic fibers.

Microcurrent therapy is a course treatment, however, in order to optimize the results of skin lifting, it is better to combine it with other methods of influence. At the same time, the combination of microcurrent therapy with mesotherapy gives the best results.

This combination is recommended to be carried out in an alternating mode.

Alternating mode involves alternating procedures of different operating principles, but at the same time – solving the same problem. The interval between procedures should be at least 2 days.

Mesotherapy involves classical mesolifting with biorevitalization. The course of mesotherapy is 4-5 procedures with an interval of 1 time per week, including 2 biorevitalization procedures with an interval of 1 time in 2 weeks (see diagram 1).

Mesotherapy + RF therapy

It is very effective to use radio wave lifting instead of microcurrents (see diagram 2).

Radio wave technology is intended for non-surgical treatment of wrinkles of I-II degree, as well as lifting of flabby and sagging skin in various parts of the body. So, at a frequency of 4.0 MHz, the resistance of subcutaneous fat is more than 12 times greater than the resistance of the skin. Thus, undamped energy of moderate power penetrates into the subcutaneous fat, causing the formation and accumulation of heat in it up to 63-65 °C, while the surface of the skin does not heat up more than 44 °C. At a temperature of 63-65°C, collagen denaturation occurs, leading to a controlled inflammatory response, which causes contraction and the formation of new collagen.

In the dermis and subcutaneous fat, collagen reconstruction occurs, causing a superficial skin tightening.

During the course of treatment, radio wave skin lifting is carried out in zones: 1st procedure - upper third of the face, 2nd procedure – middle third of the face, 3rd procedure – lower third of the face, 4th procedure – neck.

It is known that sagging of the face oval and ptosis of the cheeks is in most cases the main problem of the general sagging of the skin of the face. Often such ptosis is combined with local hyperlipodystophy – the formation of local fat deposits in the chin, cheeks, ridges between the nasolacrimal sulcus and the nasolabial fold.

In such cases, it is possible to improve the results of a course of treatment by combining mesolifting with local mesolipolysis using lipolytic preparations.

Mesolipolysis is carried out immediately before mesolifting into separate zones (chin, face oval, facial ridges), after which mesolifting is performed over the entire surface, including the above areas. This combination allows you to simultaneously act on excess body fat and stimulate skin tightening. Mesolifting is carried out with an interval of 1 time in 2 weeks. As part of the course treatment, you can limit yourself to 3 mesolifting procedures.

Supporting Procedures

Course treatment for skin lifting allows to delay surgical lifting, however, given that we are talking about therapeutic methods of exposure, treatment courses must be repeated after 6 months. In the period between courses of treatment, it is reasonable to use supporting mesolifting procedures (1 time per month), supporting hardware cosmetology. Vacuum massage and/or ultrasound therapy and phonophoresis can be performed in intercourse treatment.

Vacuum massage – mechanical impact on the skin by air with a pressure below atmospheric (vacuum discharge up to 0.3 atm) in a limited area.

Adequate use of vacuum massage stimulates microcirculation processes, activates lymphatic drainage, improves trophism, and improves skin tone.

Ultrasound therapy and phonophoresis are performed using an ultrasonic wave (continuous or pulsed exposure to high frequency ultrasound of 3 MHz and a power of up to 3 W/cm). Oscillatory movements of ultrasound are transmitted to a depth of 12 cm, a micromassage effect occurs in combination with local heating. Ultrasound or phonophoresis with external lifting agents multiply the effect of skin tightening.

Thus, the combination of microcurrent therapy or radio wave facial skin lifting with mesotherapy is an effective complex that allows you to achieve the effect of skin tightening in the early stages of gravitational descent of soft tissues.
