Forms of acne: features of manifestations of different subtypes of the disease

Acne is a chronic disease with a relapsing course. Acne is usually found on the face, upper back and chest.

Acne is classified into subtypes.

Different subtypes of the disease can occur simultaneously in the same patient.

There are subtypes of acne:

1.    Acne comedonicus: morphological elements include both open and closed comedones, while only minor inflammatory phenomena are observed;

2.    Acne vulgaris, also known as papulo-pustular acne: characterized by the presence of red papules and pustules, as well as comedones;

3.    Cystic acne: characterized by deep skin lesions, accompanied by the formation of comedones, papules and pustules;

There are also more severe forms of acne, which are discussed further in the article.

Rare but dangerous forms of acne: conglobata, fulminans, inversa

The following forms of acne are rare in the practice of doctors, but are very dangerous in terms of possible complications.

These include:

1.    Acne conglobata – is a variant of the complicated course of the form of cystic acne, characterized by purulent and ulcerative skin lesions. In addition to the face, upper back, and chest, acne conglobata also affects the skin of the buttocks and flexor surfaces of the extremities. This form of acne is characterized by the presence of bifurcated sebaceous glands, in which one duct opens through two openings;

2.    Acne fulminans (fulminant form of acne) is a rare disease observed almost exclusively in young teenage boys. This form of acne is characterized by severe inflammation and ulcerative lesions of the skin, accompanied by fever, joint and muscle pain. In some cases, there are bone deformities similar to changes in osteomyelitis;

3.    Acne inversa - is a chronic disease that is difficult to treat. This form of acne is characterized by ulcerative inflammation of the apocrine sweat glands. Acne inversa affects the flexor areas of patients, in particular the axillary and groin areas.

The above types of acne are difficult to treat and usually require the appointment of systemic complex therapy.

Acne treatment – a long process that requires a competent approach

Acne – a disease requiring long-term and thorough skin care. The doctor should warn the patient that acne therapy – a long process and the first results may appear only after a few weeks.

Some acne-prone skin care products can cause dryness and redness of the skin, especially in the early stages of treatment.

The doctor should approach the treatment of acne responsibly, taking into account the quality and effectiveness of the prescribed cosmetic products.

Topical and systemic agents are used in the treatment of acne. At the same time, the first-line therapy is precisely local treatment, while systemic therapy is indicated only in severe cases of widespread acne (according to EBM Guidelines «Acne», 2018).
