When carrying out any aesthetic procedure, both the doctor and the patient equally rely on its effective result. Of course, its main components are several factors at once, among which the main one can safely be called the quality of the selected drug.
This is especially true for anti-aging procedures, which are in such high demand at the present time. In an exclusive interview for estet-portal.com, Candidate of Medical Sciences, dermatovenereologist, cosmetologist, international trainer in injection and apparatus methods Dyachenko Yulia Yurievna spoke about which drugs should be preferred, and what important features attention must be paid to get the best result of anti-aging treatments.
What should specialists be based on when choosing a drug for anti-aging procedures
We choose the drug for anti-aging procedures based on at least two points. This is very important, because if we are talking about the Restylane line of fillers (Restylane), the preparations have a different consistency. The entire line – it is a stabilized hyaluronic acid and various consistencies: less dense and more dense.
In order to choose the right drug, we need to understand the volume of the area in which we work.
The capacity of the dermis is much less than that of the subcutaneous fat, deep fat pads, mid-medial section, zygomatic zone. There are a lot of nuances, but, in fact, everything is simple. The more superficial we are, the less dense preparations we work with.
A very important question of the second – the question of the density of integumentary tissues. Because the back of the nose will be very different from the area of the lips, and in order to work in the area of the back of the nose, to form a very clear contour with a minimum amount of preparation, we need a preparation with high lifting ability, high resistance to tissue deformation.
When we talk about anti-aging procedures, we must take into account three parameters: aging of the skull bones, aging of the muscles and aging of the subcutaneous fat. Tell us how muscles age?
Muscles age in very different ways, and as Dr. LeLuarne's research has shown, what we always thought of as atrophy, in fact, all age-related changes are accompanied by a deterioration in metabolism in tissues and their thinning. Dr. LeLuarne showed very interesting work when X-ray diffraction analysis was used to examine muscles in young patients and in aged patients, and they found aging marker tufts.
The aging marker bundle is not located in every muscle, most of them, of course, in the mimic muscles. And the zone in which we see a very clear reflection of this work – this is the periorbital region. Beam-marker of aging – it is a rigid tissue that is not capable of contracting and sliding along the deep subcutaneous fat layer.
That is why the diffusion of this very subcutaneous fat layer occurs, and we see wrinkles in the thin periorbital region not only due to the work of the muscle and thinning of the skin, but also due to the replacement of superficial subcutaneous fat with deep packets. A very important conclusion of this work is that it is necessary to inject drugs for contouring deeply to compensate for the deficient package, and use the minimum dosages of botulinum toxin so that the muscle can at least be relaxed at rest.
Which deep injecting drugs do you recommend
Since we choose drugs for deep administration, depending on muscle tone and the amount of integumentary tissue, in fact, approaches for men and women will differ. And if women have a tendency to use less dense preparations for anti-aging procedures, then for men, Restylane Perlane (Restylane Perlane) – it's a bestseller. It can play both the role of a volumetric and the role of an intradermal filler, due to the physiological characteristics of the skin.
Skull aging is also a very important aspect in anti-aging therapy. How can we correct this shortcoming?
You mentioned the work of Dr. Mendelssohn. She is many years old and very interesting. When we see the radiographic features of an aged face and a young face, we see a very clear localization of bone resorption. These are the superior-medial angle of the orbit, the inferior-lateral angle of the orbit, the piriform zone, into which we will add both the medial-medial section and the area of the nasolabial fold, this is the angle of the lower jaw, the projection zone of the mandibular ligament and the apex of the zygomatic bone.
Please note that in all these areas we work deep on the bone, because where there is a zone of bone resorption, it is correct to inject the drug deep on the bone. And even without knowing about this work, we did exactly the same.
Which drug do you recommend to choose for osseous injection?
On the bone, we need dense preparations that, with a minimum amount, will give us a lifting effect. These are Restylane SubQ (Restylane SabKyu), Restylane Perlane (Restylane Perlane), Restylane Defyne (Restylane Define). We know that the Restylane line is expanding, new preparations are appearing, and now you and I will have a choice between preparations that have a very clear lifting ability, and preparations that are more able to integrate into the surrounding tissues.
How can we, with anti-aging treatments, affect the aging of subcutaneous fat compartments?
We are based on the degree of ptosis. Superficial subcutaneous fat is more prone to ptosis than deep subcutaneous fat. Therefore, areas in which we want to obtain a lasting effect without tissue displacement, of course, are subject to deep injection. First of all, this is the middle third of the face, the angles of the lower jaw, the chin area. These are the areas in which we need the injected drug to remain in one place for a long time.
Are there any special features of anti-age procedures for men?
Men look tired, and in order to eliminate the signs of fatigue with anti-aging treatments, we need to, in the same way, remember the middle third, and eliminate the upper third of the nasolabial fold, which gives a shadow. We are now talking a lot about this play of light and shadow, because, literally with one puncture, with one injection of the drug, we can change the visual perception of the face without changing any volumes.
Hypertonicity musculus depressor anguli oris works in both men and women, so this is the tired face that we all start to age from, men have a much longer duration, and we work with them in the same way.
What new drugs are Galderma currently bringing to market, and what are their fundamental differences?
Galderma has added a number of sisters and brothers to our familiar Restylane.
The new Restylane preparations have a very interesting feature – they have a great integrative capacity.
If we speak our cosmetic language with you, these preparations are softer. And if, for example, when working in the middle third deeply, osseously, we want to make up for the deficit in the volume of the lateral part, we need a dense preparation, and we are well aware that Restylane Perlane (Restylane Perlane) in this regard will be indispensable.
But if we have a patient with soft tissues, a small amount of subcutaneous fat and a sufficiently reduced muscle tone, and we want to work on the palpebromalar groove, we will take a drug called Restylane Refyne (Restylane Refine). This is a new drug that has a greater integrative capacity, and it will be more gently distributed in the injection zone.
