What particularly relevant trends in the development of modern dermatology can you identify
In fact, dermatology is not just developing now, but is developing very quickly and very actively. There is a lot of new in dermatology now, because new methods of diagnostics and treatment have appeared, which we are lucky to know about, and we can use them, because we often travel abroad, to all European congresses and significant scientific events, where we always draw the latest information . The fact is that dermatology, a priori, was doomed to develop, because this is what everyone sees, and the patient himself in the first place.
Internal problems are not always noticeable, and sometimes a person does not pay attention to them, but what is on the skin, in addition to the fact that the person himself sees it, is also seen by everyone around.
Therefore, naturally, there is a great demand for knowledge in the field of dermatology, the deepest research is being carried out in the pathogenesis of skin diseases, and new methods of treatment are being invented. We are approaching good compliance, when the patient does not need to undergo long courses of treatment, but, on the contrary, we try to make it convenient for him to use the course of treatment, so that there are fewer side effects, so that he feels comfortable being on this treatment, and, of course, it was a very good result.
What diseases do patients most often come to you with
They treat all problems, I cannot single out any one nosology, because I practice in general dermatology, pediatrics, and dermato-oncology. I can say that now there is a lot of acne, a lot of acne rosacea, of course, there is always a stable lot of psoriasis and atopic dermatitis, as well as other allergic dermatosis.
Considering the level of ecology, what we eat and drink - the level of sensitization of the body by allergens is quite high, so allergic dermatoses are quite important. Mycotic diseases also do not leave the arena, and the so-called bacterial infections of the skin - they are also quite relevant.
Are there any new trends in the treatment of autoimmune dermatoses
Yes, of course, there are new trends, or rather, new directions, because immunobiological therapy, which is number one in the world, continues to be researched very actively. In fact, there are already a lot of studies, but they are not yet completed. Also, new directions, new schemes are being invented, various randomized trials are ongoing, and I am very pleased that Ukraine is also taking part in this. Our doctors take part in clinical research at a very high level, and our work is also very well quoted.
If we talk about the new in dermatology, could you tell us about the latest developments in the diagnosis of dermatological diseases?
Diagnosis really became much easier for us, because the first thing we have – This is dermatoscopy. We have taken over dermato-oncology, and we are dealing with skin neoplasms, therefore, of course, a dermatoscope greatly facilitates the diagnosis of neoplasms, and every self-respecting dermatologist must be proficient in dermatoscopy.
We also have the opportunity to work with quality reagents and laboratories. Having a certain set of markers, we can work with both allergic dermatoses and autoimmune diseases. Now it has become much easier, because, let's be honest, a lot has already been invented in diagnostics, we use a lot, thanks to the developments and research of our colleagues from the European Union and the USA, and the companies that bring all these new products to Ukraine, all that remains is to adapt them , register in our country, and we can safely use them.
What is important for both doctors and patients to know about diagnosing nevi
A simple word is very important - prevention. There is prevention on the part of the patient, who himself must clearly understand that he is responsible for his health. People are born with nevi, or they appear in the process of life. But any person who has a certain number of nevi, whether he was born with them or they appeared during his life, should go to a dermatologist who has a good dermatoscope and is experienced with it.
Then the specialist will evaluate the dermoscopic picture, and say to the patient, for example: “You are fine, but this is“ good ” we need to control, so it is enough for us to meet with you once a year so that I look at all your nevi, and you understand that everything is fine with you. But we agree with you that you do not abuse solar radiation.
Once again, I want to address both colleagues and patients, and remind them that the sun should not be abused, because, in addition to photoaging of the skin, this can lead to a large cascade of problems!
But carcinogenesis is, of course, associated with solar radiation, so you need to use sunscreen, not stay in the sun for too long, rest actively, intensively, and in motion, but under sunscreen. And it is very important to know that a visit to a dermatologist and dermatoscopy are especially necessary for those patients who have a large number of neoplasms on the skin, because it will be difficult to independently notice the appearance of a new nevus, which may turn out to be malignant, and for patients who have a history of malignant diseases among close relatives. skin neoplasms.
And for us, doctors, it is certainly important to understand that dermatoscopy diagnostics, you need to constantly learn, regularly update your knowledge, since there are now all the conditions for this.
