Specialists of aesthetic medicine today cannot imagine their practice without thread techniques. The threads are actively used in the work of both cosmetologists and plastic surgeons, as they allow to achieve a pronounced effect of skin lifting, and at the same time they are a safe and minimally invasive method. With the help of threads today it is possible to solve the problems of many parts of the face and body, including the problems of the lower third of the face. What thread techniques can be used to correct the lower third – read on estet-portal.com in an interview with a plastic surgeon, a member of the German Society of Plastic Surgeons, General Director of the Total Charm aesthetic medicine clinic (Moscow) Eric Azizyan.

What methods of correction of the lower third of the face with the help of threads are used today?

We can divide the correction of the lower third of the face with threads into several methods. This is a method that allows you to pass absorbable threads and move soft tissues, a method with fixation of threads on the periosteum, which gives us the opportunity to additionally tighten the lower third of the face every 3-4 years, or rather the submandibular zone, that is, directly the second chin. In addition, there are also combined methods, such as liposuction plus soft tissue stitching. This method is used because liposuction alone is often not enough, because in the future, if we do not strengthen the skin and do not fix it in any way, sagging will begin.

Tell us more about the correction of the lower third of the face with periosteal fixation threads?

There are surgical and cosmetic techniques for correcting the lower third of the face with threads fixed on the periosteum. We can use Aptos Needle 2G threads, which can be absorbable or non-absorbable. Aptos threads in Ukraine are represented by Top cosmetics. We can perform reverse threading, that is, first we lead the thread in one direction, and then we return, bringing it to the injection point. In this way, we can make a small loop, and tie a knot that will hold the thread in a fixed state. There are also Aptos Needle 2/0 threads without notches, which, with the help of small incisions on the periosteum, are fixed in a U-shaped way, that is, a U-shaped seam is first applied, and then the thread itself is fixed to it.

The advantage of this method is that after the node is moved, the patient does not go through rehabilitation, as we do not perform the procedure again.
Accordingly, we have neither hematomas, nor edema, nor the need for rehabilitation, and the procedure itself takes 10 minutes of the patient's time. It is also very economically advantageous, because there is no repeat procedure, and neither the material nor the doctor's work costs much.

What is the difference between surgical and cosmetic methods of correction of the lower third of the face with threads fixed on the periosteum?

The difference lies in the fact that the surgical method uses non-absorbable threads, the setting of which requires small incisions, and cosmetologists cannot do this. The advantage of the cosmetic method is that no incisions are required. Lifting is carried out using absorbable threads, which also have the effect of biostimulation due to the release of polylactic acid. Non-absorbable threads made of polypropylene do not have such an effect, but polylactic acid and caprolactone, which are the basis of absorbable threads, can improve skin condition. The disadvantage of cosmetic methods is that in the future we cannot additionally tighten the installed threads, but their advantage is that they are also working with the skin. The choice of method depends on what kind of threads the patient wants, and what methods the doctor can work with.

What are the main difficulties in correcting the lower third of the face with thread techniques?

If we work in the right layers, and we mainly work in the subcutaneous adipose tissue, there are practically no difficulties and dangerous points. It is very important to choose the patient correctly, and that the area into which we pass the thread is not too ptotic, that is, that there is not too much excess skin, which does not allow us to get good results. With regards to the submandibular area, if we are talking about the improvement and creation under 90°; the lower third of the face and especially the submandibular zone, it is very important to take into account the volume of subcutaneous fat.

If the patient has dense fat in the submandibular area – it will not allow us to move the soft tissues, and therefore we must remove this fat before placing the threads.

Cosmetologists have lipolytics for these purposes, and surgeons have the opportunity to do liposuction. But, in any case, to get a good face shape, you need to combine methods, remove excess adipose tissue, and only then install threads. But if the patient has a very large excess of skin – it all depends on the decision of the patient. If he agrees to a slight correction – we will achieve this effect, but if the patient wants to get the maximum movement of tissues, but he has too much skin – it is better to cut it, and then with the help of threads to maintain the result that we have achieved.

What other procedures can be combined with thread lifting of the lower third of the face?

In the lower third of the face, a combination of thread techniques with botulinum toxin is mainly used, which allows you to slightly relax the platysma bands and carry out thread lifting. We carry out such a procedure in our clinic, and we get a longer and more pronounced result than if we simply performed a thread lifting procedure. In addition, thread techniques are combined with mesotherapy. Hyaluronic acid in combination with Aptos threads, presented in Ukraine by

Top cosmetics

, allows us not only to move soft tissues, but also to obtain a positive result of skin biorevitalization.
