On April 16, 2016, the conference "Three-profile planning for the correction of the face and neck of a difficult patient" was held in Kyiv, the main organizer of which was the "Aesthete Center of Dr. Borodko". The main topic of the conference was the need for joint work of specialists of three different profiles on a difficult patient.
Olexandr Borodko himself, a plastic surgeon, a surgeon of the highest category and a member of the All-Ukrainian Association of Plastic, Reconstructive and Aesthetic Surgeons, made several reports on the features of planning the correction of the face and neck of a complex patient and options for aesthetic correction. Especially for the readers of estet-portal.com, Dr. Alexander Borodko agreed to answer a few questions.
Alexander Vasilyevich, what do you mean by saying a difficult patient? What features of changes in the face and neck are we talking about?
When I talk about a “complex” patient, I mean something that we dermatologists and plastic surgeons have not yet considered in our office consultations: dynamic profile changes. We never mention during the consultation about the peculiarities of the posture of the head and neck in relation to the shoulder girdle. With age, the position of the head changes: if a small child holds his head straight and somewhat back, and walks with his tummy sticking out, then with age a person bends his head forward more and more. It is from this that such problems arise as a sloping chin, senile prognathic bite, and so on. Everything is much more complicated than we think.
Please tell our readers what is three-pronged planning?
It is about the joint work of several specialists. This is a plastic surgeon, an orthodontist, and a cosmetologist-rehabilitologist, that is, a person who works with a non-fascial system, with the restoration of the dynamics of the ratio of details of the face, neck, and shoulder girdle. Based on the reports presented at the conference, it can be seen that even without the use of any fillers and botulinum toxin, it is possible to achieve an improvement in appearance, not to mention functional improvements, normalization of the functioning of facial systems.
What maximum results can a doctor achieve when working with a difficult patient, and what effect can the patient himself expect?
The main result is a more complete service to the patient, who should feel that he has been further served, improved. Running from one specialist to another, the patient feels some kind of underdevelopment over himself, and today's conference is aimed at finalizing him. This is not the end, but only the beginning.
What modern methods in aesthetic medicine can you single out as those that have proven to be the most effective in working with difficult patients?
This is volumetric facial modeling, botulinum toxin therapy and PRP therapy in combination with outpatient lipografting, that is, autotransplantation of fat enriched with PRP plasma. But this, I believe, will be the topic of our next conference, which is planned for September.
Do you plan to introduce any novelties of aesthetic medicine in the work of your center in the near future?
It is the combination of PRP-therapy, that is, PRP-plasma enriching auto-fat, that is planned to be introduced in the near future.
What are the services of the "Estet Center of Dr. Alexander Borodko" are especially popular today?
Since we do not have operating rooms, on the basis of our center, in addition to applied aesthetics, PRP therapy is gaining momentum, which in itself cannot be monotherapy. Everything is good in combination, in a complex.
