Даниэль Кассуто: «Гиалуроновая кислота – это не просто филлер»

At the congress held in Kyiv within the framework of the International Association of Aesthetic Plastic Surgeons (ISAPS) on May 19 and 20, a report was presented by Professor Daniel Cassuto, Plastic Surgeon, Professor of Plastic, Reconstructive and Aesthetic Surgery, Department of Plastic Surgery, University of Modena Hospital (Italy) , international expert Merz-Aesthetics.

Professor Kassuto in an interview for estet-portal.com spoke about the features of male facial rejuvenation, the latest data on hyaluronic acid and the important points in compiling an anti-aging program for a patient.

 

How to properly design an anti-aging program for a patient

Now you are talking about the most important of our tasks – to teach our patients to lead a correct way of life. I mean prevention, healthy aging. If a person ages in a healthy, natural way, nothing is impossible for a doctor. If we start improving tissue quality, position, and quantity before age-related problems begin, we can keep patients attractive even as they age.

If we start working with patients who are living the wrong lifestyle when it's too late to prevent problems from occurring, patients will expect miracles from us that we can't do.

Therefore, task number one – to teach the patient to lead the right way of life, task number two – explain to the patient that we are not trying to sell them an operation or other procedures, we are not going to unconditionally comply with all his requirements.

Our task as doctors – listen to the main complaints, understand them and understand the needs of the patient. We should find out what is bothering the patient and find the best way to solve the problem, and not let him tell us what to do.

Why should you pay special attention to the condition of the patient's neck when choosing anti-aging treatments

Neck – this is an area whose rejuvenation methods have until now been limited. Unlike the soft tissues of the face, which allow us to adjust the volume with high precision, we did not have good ways to restore the volume of the neck. As we age, the neck loses volume like any other part of our body.

The new generation of hyaluronic acid products, which are not cross-linked BDDE (butadiol diglycidyl ether), provides much better tissue integration than any previously available drug. New drugs allow us to restore volume to the neck in a natural way, instead of removing what we think is excess skin.

Moreover, hyaluronic acid also improves skin quality in the long term. As such, it is the ideal (medical, not commercial) way to keep neck tissue young and healthy.

Is it possible to achieve a significant effect of facial rejuvenation with the help of hyaluronic fillers

Just a couple of years ago, we thought that hyaluronic acid was intended solely for filling space, restoring volume. I think it's time to stop calling hyaluronic acid a filler and find a more appropriate name for such products, since scientific publications contain information that hyaluronic acid – excellent stimulator for fibroblasts and (attention!) adipocytes.

Recently published research results indicate that – an active molecule, not just a void-filling gel that we inject to restore volume in a certain area. Therefore, we need to change the order of priorities.

We are now injecting HA into the face first to improve the quality of the tissue, and reshaping the face helps to reverse the rest of the signs of aging.

What are the peculiarities in carrying out anti-aging procedures for a male face

Actually, the aging process for men is no different from the aging process for women, it just starts much later. Women begin to age and lose facial volume from about 25 years old, men – about 40. True, men age, although later, but faster than women. This is provided that they lead the right way of life.

Of course, there are some interesting features.

For example, women often cover the sides of their faces with hair, so we can perform treatments that allow temporary side effects. In men, the temporal and lateral regions of the face are always open, so we must be very careful.

Also, when working with the lower third of the face, it is important to remember that a more pronounced mandibular line makes the face more masculine. Therefore, this feature of rejuvenation is often used when working with men, as is the increase in volume on the sides.

For women, we tend to stick to an oblong, oval face shape, so we hide volume under the jaw to avoid protrusion. Of course, there are a whole lot of other nuances that you can talk about for a very long time. One thing is certain – there are a lot of differences between the rejuvenation of a female and male face.
