Aesthetic procedures aimed at lip beautification are very popular among patients of all age categories. When working with such a delicate area, the practitioner must be well versed in its anatomy and be able to choose the right preparation and injection technique.
Surgeon, member of the German Society of Surgeons, the Society of Mesotherapy, the European Academy of Aesthetic Surgery and the International Society of Aesthetic Medicine, author of scientific articles on volumetric facial modeling, Dr. Wolfgang Redka talks about his approach to injectable lip correction using innovative Cytosial fillers -Freedom (Wolfgang Redka-Swoboda, Germany).
What features of embryology are important to consider when injecting lip correction
B. R-S.: When working with lips, it is necessary to remember that the upper lip is divided not into two, but into three parts – two lateral and one central, which are connected in the region of the filtrum. The lower lip consists of two parts – right and left, which are connected in the center. Such features of embryology must be taken into account when correcting lips with fillers.
For example, when volumizing the lips, do not inject the drug into the central part of the lower lip – it's unnatural. The presence of volumes is typical for the left and right parts of the lower lip, and there is a small depression in the center. It is this feature that my colleague from the UK used as the basis of his technique called the Heartly Lips.
When volumizing the lips, do not inject the drug into the central part of the lower lip – it's unnatural. The presence of volumes is typical for the left and right parts of the lower lip, and there is a small depression in the center.
The same principles apply to the upper lip: you should not inject the filler only into its lateral parts, neglecting the central – in some cases, the results of such a correction will be unnatural. Therefore, during lectures, we pay attention to both anatomy and embryology.

B. R-S.: Many techniques can be used to correct the lips. When attending congresses and master classes, you will hear hundreds of speakers, each of whom talks about his own features of the injection correction of this zone. The most important principle – the doctor must know what he is doing. My multiple puncture technique, performed with a very small 4mm needle, – one of many ways to get good results.

Which lip injection techniques do you prefer to use
B. RS: At the moment I prefer to use cannula techniques. The fact is that lip correction is often of interest to young girls. As a rule (but there are, of course, exceptions), such patients seek volumization, but in the vast majority of cases they have a beautiful lip contour. Therefore, I inject a good volumizing filler with a cannula using a safe and painless technique.
When working with young patients, I no longer resort to correcting the lip contour directly. Instead, I add volumes, thereby providing an attractive projection of the contours.
Provided that the drug is injected correctly at the level of the labial muscles, a beautiful lip curve can be obtained, including at the border between the red border and the skin, and its projection can be improved. Therefore, when working with young patients, I no longer resort to correcting the lip contour directly. Instead, I add volumes, thereby providing an attractive projection of the contours without touching them. The beauty of the lip contour is easy to break with injections, and our main goal – get natural results.

How are principles of embryology applicable to lip injections
When we are talking about the lips we have to respect that upper lip is not divided into 2 parts, because due to embryology it is divided into 3 parts: 2 lateral and 1 central part, and the fusion line is the philtrum. In the lower lip we have only 2 parts, right and left, and the fusion line is in the center. So this knowledge we should respect when we are injecting lips. For example, when we add volume to the lips we should not inject in the center of the lower lip. Normally, this is not natural.
Naturally, you have volume on the left side, on the right side and in the center there is a small dimple which my colleague from the UK is using in his technique called the Heartly Lips. The same principles apply to the upper lip: you cannon inject only the lateral parts and don’t respect the central part. In some cases it won’t look natural. That’s why we teach anatomy and embryology in the lectures.
Where can one use the principles of injecting the contour of the lips: one and multiple injections
You can use all techniques in all lips. If you go to the congresses, workshops and have 100 different speakers, everybody is injecting the lips a little bit different. The principle is you have to know what you are doing. And this multipuncture technique with a very small 4 mm needle is one way to do the job to get a good result.
What technique do you prefer
I personally prefer at the moment injecting the lips by cannula. For I found out that normally lip injection is a question for younger ladies. Normally, they want more volume. But normally, young patients have nice contour of the lip, so now I only inject volume with a cannula in a very safe way so the patient don’t feel pain with a nice filler giving a little bit of volume. And when the placement of the volume is on the right level in the muscle of the lip you get a nice eversion of the lip and vermilion border, so the contour of the lip, will be projected better.
So at the moment when I have a young patient I don’t inject any more the contour. I only give volume and by the projection also the contour will be projected nicely without touching it. The problem is when you have a beautiful contour and you start to inject you can destroy a beautiful lip. And we want a natural result.
