In practice for professionals: the lower third of the face and its layer-by-layer restoration

Layer-by-layer correction of age-related changes in the lower third of the face, as a rule, is especially needed by patients at the age of "45 years plus". It is during this time period that the most pronounced deformations of the appearance occur, which force a woman to seek help from a cosmetologist in order to slow down the aging process. The lower third of the face on one side – most clearly demonstrates the influence of age on appearance, and on the other hand – has a rather complex structure, requiring a balanced approach to the correction of each component.

How and by what means to achieve a good effect of such a correction, Tatyana Epifantseva, dermatologist, cosmetologist of the medical center, shared a practical example with estet-portal.com readers "ADASSA" (Dnipro), head of the training center for medical cosmetology and minimally invasive surgery «EMET Medicine».

The cosmetological approach to patients of the age of “45 years plus” is always very interesting and at the same time difficult, especially to the “cosmetologically naive”, that is, those who first applied for help to a cosmetology clinic and want to get a significant result with minimal costs - both financial and psychological. The process of age-related changes in the lower third of the face by this age has usually already formed into a complex picture and is figuratively similar to a “layer cake”, where each layer has its own characteristics of both structure and involution, which means that it needs a specific tool and approach that rejuvenates it .

Below is a schematic representation of the layer-by-layer complex restoration of the attractiveness of the lower third of the face with the correction of both internal and external factors of aging from Emet^ТМ (Fig. 1).

It will take at least 1 month and 4 visits to the cosmetology clinic for the patient to complete such a program.

Clinical case

A 56-year-old female patient who underwent part of the Emet^TM program for layer-by-layer restoration of the lower third of the face. The aim of the complex of procedures was the individual natural improvement of the contours and oval of the face.

Photo 1 schematically shows the injection sites and the procedures performed in layers at each visit to the clinic by the patient.

1. Intradermal micropapular and linear injections MesoWharton P 199 (MesoWharton) (manufactured by ABG Lab, USA)

Purpose: To restore skin turgor and elasticity and reduce its surface area. The drug acts as a mobilization factor for the skin's own tissue-specific stem cells, activating their proliferation and differentiation into young functioning cells of the epidermis and dermis.

Technique: along Langer tension lines 1.5 ml – face, submandibular region, neck.

Number: 4 treatments at 7 day intervals.

2. Injections MesoSculpt C 71 (mesoscalpt) (ABG Lab, USA)

Aim: To safely reduce the volume of superficial facial fat pads, both by reducing the size of the adipocytes themselves and by reducing the volume of interstitial fluid

Amount: 1ml, 2  treatments at intervals of 14 days.

3. Ona botulinum toxin injections

Purpose: to reduce the activity of depressor muscles in the lower third of the face.

Quantity: 15 ED  in total (once).

4. Injections of the tightening and regenerating mesocomplex md:complex Firming-Regen (manufactured by md:ceuticals, England)

Purpose: the preparation contains DMAE 3%, chondroitin sulfate and elastin, helps to stimulate the activity of the levator muscles of the lower third of the face, serves as a replacement therapy for loss of elasticity and skin turgor.

Technique: inject 1.2 ml in total into 12 projection points of the levator muscles of the lower third of the face.

Number: 4 treatments spaced 7 days apart.

Procedures carried out according to an individual treatment plan, consisting only of injections of various drugs into three layers of the lower third of the face – skin, subcutaneous fat and musculoaponeurotic layer – made it possible to achieve a pronounced rejuvenating effect quite quickly and naturally, and most importantly, without pronounced material and psychological costs on the part of the patient.
