Aesthetic gynecology – This is a fairly new, but very progressively developing direction in modern medicine. Patients with the most delicate problems come to an appointment with a gynecologist engaged in aesthetic gynecology, which is why there are very important features of working with them.
In an exclusive interview for estet-portal.com, she spoke about the most important features of receiving patients in aesthetic gynecology obstetrician-gynecologist, physiotherapist of the Department of Aesthetic Gynecology and Rehabilitation of the National Medical Research Center for Obstetrics, Gynecology and Perinatology. IN AND. Kulakova, co-founder of the "Association of Specialists in Aesthetic Gynecology" Elena Gorbunova.
What usually stops patients from visiting an aesthetic gynecologist?
Immediately before going to an aesthetic gynecologist, patients are sometimes stopped by embarrassment. Still, this is an intimate, hidden sphere, and women do not always consider it necessary to deal with the elimination of aesthetic flaws in this area. They are embarrassed to discuss these issues, and sometimes even embarrassed to undress during a gynecological examination.
When a problem is identified that needs to be eliminated with the help of aesthetic gynecology, and the question arises of what can be done to eliminate it, both doctors and patients, unfortunately, are stopped by the lack of information.
For doctors, this is precisely the lack of knowledge, their own practical skills, algorithms, guidelines and clear recommendations regarding which method and under what condition it is necessary to use, what complications can be, how to prevent them, what kind of comprehensive examination and preparation of the patient for this effect, and which method should be combined with which.
Actually, for the sake of this, doctors come to us for training in order to master this information. But aesthetic gynecology has not yet been singled out as a separate specialty: we have gynecology, we have obstetrics, and oncogynecology, but there is no aesthetic gynecology as such.
Accordingly, this spread of information and lack of knowledge makes doctors look for them in other areas of activity, and, at the same time, look for those people who already have the necessary skills, who have their own experience, and who are ready to share knowledge.
What stops patients? This is also a lack of information, first of all, about high-quality specialists. We know that often a good doctor is transferred simply from hand to hand, by word of mouth, because the Internet – this is one advertisement, but what comes out in reality, not on an advertising module, shows that there are complications, and problems, and promises often do not match what we get in reality.
Expectation and reality – These are the two pictures that come to mind. And one more important point – this is a lack of reliable information, both for doctors and patients.
What must an aesthetic gynecologist discuss with each of his patients at an appointment?
A patient who visits an aesthetic gynecologist for the purpose of intimate correction should be aware that we do not deal only with a narrowly focused intimate area, but we approach each patient as a whole organism. And taking into account the fact that any patient turns to intimate aesthetics and aesthetics of the face when he sees changes associated with age, and, accordingly, he wants to stop aging, and live long, beautiful, and look good, naturally, we must understand that any of our, even minimal, impact should have a good base.
It starts with proper nutrition and adequate nutraceuticals, we need to support micronutrients in all our procedures that we perform, as well as correct hormonal imbalances. Many of our manipulations are aimed at lifting, stimulating fibroblasts for the synthesis of collagen and elastin, but for this stimulation to be effective, we must have a nutritional base for fibroblasts. The second is lifestyle.
If a person leads a sedentary lifestyle and hopes for a "magic pill" and our magical methods - the effect of the procedures will not be 100%, but only 30-40%. And that is why we always repeat to our patients that we participate in our treatment together.
And one more thing that we must definitely take into account - whenever we select a treatment, we prescribe some kind of set of measures, and these measures overlap each other, they are consistent and mutually reinforce each other, that is, we provide their synergy, and therefore we achieve a 100% effect.
When vaginal plastic surgery – it is not only an aesthetic correction
Tell us about the use of laser technology in aesthetic gynecology
It should be noted that in aesthetic gynecology, we do not use a low-intensity laser, and not supravenous and intravenous blood irradiation with a laser, which is used in traditional physiotherapy for gynecology. Our laser – it is either an erbium laser which allows for photothermal tissue reconstruction.
This is a non-invasive technique, and it is due to tissue heating that fibroblasts are stimulated for the synthesis of collagen and elastin, as well as a lifting effect on the mucous membrane of the vagina and vulva, and on the skin of the vulva. At the same time, I note that lifting & nbsp; The vaginal mucosa directly depends on how moist it is, because the erbium laser works with such a chromophore as water.
Erbium is much more absorbed by water than a CO2 laser, respectively, for young women, an erbium laser – this is laser number 1.
Syndrome of "stretched" of the vagina, prolapse of the vaginal walls after childbirth of the 1st and 2nd degree, stress and mixed urinary incontinence, urgent urination, overactive bladder – these are the positions that are completely covered by the erbium laser. It has a photothermal effect and works by heating.
The second laser that we actively use in aesthetic gynecology – it is a fractional CO2 laser that provides fractional photothermolysis. In this case, we also do tissue heating, but due to the fact that this is a CO2 laser, and it allows you to penetrate tissues, that is, to make a thin ablation and, accordingly, to deliver heat more deeply - it has wider indications for clinical use.
Main causes and treatment of vaginal wall prolapse
This is not only the "stretched" syndrome; vaginal prolapse and prolapse of 1-2 degrees, prolapse of the vaginal walls and urinary incontinence, but also sexual dysfunction, correction of problems associated with aging of the intimate sphere externally, that is, laxity of the labia, hyperpigmentation, lipodystrophy, lichen sclerosus, as well as genitourinary syndrome in menopause - a condition in which, on the contrary, the mucosa is dry, and we observe vulvovaginal atrophy, thinning and dryness of the mucosa, which brings enormous suffering to women, especially those women who, for one reason or another, refuse hormone therapy.
Fractional CO2 laser – this is the way out for these women, and this is the way out for the doctor. The effect of treatment with fractional CO2 laser for 3 sessions is enough for two years.
