In early September, as part of the grandiose event for plastic surgeons Visiting Professor Program II, Ukraine was visited by one of the most prominent specialists in the field of plastic and aesthetic surgery – Dr. Oscar Ramirez. The guest from the USA not only shared his many years of experience with colleagues, but also demonstrated his own techniques of endoscopic facelift and nose correction. Especially for the readers of estet-portal.com, Dr. Ramirez spoke about his unique surgical techniques and features of correcting facial imperfections.

Dr. Ramirez, at the moment, many patients prefer endoscopic surgery to other types of surgery. Do you think they can replace traditional surgical techniques?

Actually, these techniques are more exclusive because they are for a specific age group: under 50s. In such patients, we can perform a lifting of sagging tissues without removing excess tissue. Although, if we are talking about patients who are already over 50 and above, when we see a rather strong sagging of the oval of the face and neck, then a combination of different surgical techniques is necessary, which may be needed. But even if we are talking about patients over 50-60, even 70-80 years old, we still do some part of the intervention endoscopically. For example, the middle third of the face is very well rejuvenated with the help of endoscopic techniques.

Tell our readers more about rhinoplasty in patients with thick nasal skin. What is the complexity of the surgical technique for such an operation?

Yes, this is indeed one of the problems of plastic surgery. Even though we can rebuild the bone structure very well, then this thick skin is not lined very well. And also in patients who have thick nose skin, as a rule, cartilage is not very good, so we additionally take a cartilage graft from the nasal septum, or from the rib. And this is one of those surgical techniques that creates the illusion that the patient has a thinner nose. Here it is very important that there is good, adequate support.

If you imagine that the – this is the thick skin of the nose, if we provide good, solid support from below, then it will give the impression that this skin is thinner.

In addition, these patients need to be handled with care, because they have a fairly large layer of adipose tissue under their thick skin. Another strategy used by – This is the performance before surgery of certain cosmetic procedures that thin the skin of the nose by influencing the sebaceous glands. This prepared skin is easier to redistribute and spread over the nose.

What surgical technique do you use to correct a hunchbacked nose?

This is another tricky part of rhinoplasty because some of the structures that support the nose need to be removed. And when I remove the hump of the nose, I work in the longitudinal direction, and the standard procedure with the rasp is done in the horizontal direction. And after we remove the hump, we need to provide support for the lateral parts of the nose with the help of cartilage, so that later the valves in the middle part of the nose do not collapse, which happens after standard rhinoplasty.

What does this trip to Ukraine and participation in the Visiting Professor Program II mean to you?

This satisfies my passion, my hobby – this is training.

All that I have learned over the years of my work, all this knowledge in the field of plastic and aesthetic surgery, I pass on to younger generations, and I really like it.

I am a member of the International Society of Aesthetic Plastic Surgery, and very often attend various events as a guest professor, so it was a great honor for me that I was invited here to speak to members of the Ukrainian Society of Plastic and Aesthetic Surgery. And if we talk about the personal aspect, then this, of course, is an opportunity to meet new people and visit a new country. This is my first time in Ukraine and I am very pleasantly surprised by what a beautiful country and people are here.
