It is no secret that many of our compatriots are convinced of the advantages of medical care provided abroad and do not trust domestic medicine too much. However, many problems, including those in ophthalmology, are now successfully solved by our specialists.
For example, the head of the ophthalmology department at the Medical Club clinic, Andrey Nikolaevich Ruban, doctor of the highest category, candidate of medical sciences, shared with Estet-portal.com readers his observations about the peculiarities of the work of Western clinics and what advantages they can have our patients, not leaving your country.
— You had the opportunity to observe how clinics in Europe work. How is their approach to the patient fundamentally different?
— There are several key differences, in my opinion. First, in all civilized countries there is insurance medicine based on evidence-based, the most effective methods of diagnosis and treatment eye diseases. Based on these methods, standard patient management protocols have been developed, which are covered by insurance. This makes it possible to make specialized ophthalmic care more accessible and of high quality for the majority of the population and to develop a single "language of communication"; in the medical environment. But there are also disadvantages. The standard protocol implies a "standard patient", and there are no such cases. This is a certain drawback of the "Western" approach, since standardization largely kills the individual approach. Powerful pressure insurance companies leads to the fact that the doctor should use only those methods that are recognized and paid for by these & nbsp; insurance companies and discourages any desire to do things differently. As a result, most of non-standard approaches in ophthalmology, is currently being developed mainly by specialists from non-leading, in the usual sense, ophthalmological schools.
Secondly, our, let's say, "Slavic approach" more based, in my opinion, on certain psychological relationships with the patient. Doctor has always been considered a special person who solved problems related to the health of the patient, not only due to his education, medical literacy, level of intelligence, but also due to communication skills, the ability to communicate with the patient and compassion.
In our division, we have tried to combine both of these approaches to get the best results.
— How is the work of your surgical department at the Medical Club clinic organized?
— Our division, like the entire Medical Club clinic, works on the principle of an outpatient surgical center (ACC). This is a model that exists in many American and European clinics means that the vast majority of surgeries are performed on an outpatient basis. The patient comes to the appointed one day, he gets an operation and leaves.
To a large extent, this has become possible due to the use of the most advanced minimally invasive sutureless technologies, in which operations on the vitreous body and retina are performed through incisions of 0.4-0.5 mm. This approach can significantly reduce the surgical trauma and the patient's rehabilitation time.
Of course, in some cases there may be situations where it is necessary to leave the patient overnight for observation. For this, the clinic has comfortable single rooms.
— Does this mean that the patient does not need a special rehabilitation period with strict adherence to some rules?
— Yes, our patients do not need it. Even after complex, combined operations lasting 1-2 hours, there are no special restrictions, patients are quite mobile and the main type of postoperative treatment – this is the instillation of eye drops.
— Does it happen that patients come to you from other clinics where they could not be treated?
— Of course, we are treated by patients who could not be helped in others & nbsp; clinics or just wanting a second opinion. I had to do re-operations on patients previously operated in & nbsp; Italy, Austria, Germany, Portugal, France, USA, Russia.
Someone we helped, someone, unfortunately, could not help. But today we can say that the level of technical equipment of the clinic, the professionalism of the staff allow us to solve problems at the highest level. I've never been ashamed of what we do, not in front of a single patient or colleague from another country, and I absolutely responsibly affirm that we work at the most modern level. Of course, there are still problems that we cannot solve, but believe me, they are in all clinics in the world.
