The editors of the popular science portal "Aesthetic Medicine" went to visit the Clinic of Vascular Pathology, where we had the opportunity to talk with its head, practicing vascular surgeon Sergey Vilenovich Lirnik.
Among his patients, one can meet both business ladies who combine care procedures with a cup of coffee in the office of the head physician, and elderly people exhausted by unsuccessful long-term treatment of trophic ulcers. Sergey Vilenovich strictly and professionally monitors the health of each of his patients. The beginning of treatment is preceded by a consultation with a doctor. Careful history taking, the appointment of tests and hardware diagnostics help to identify the causes of the disease and correctly draw up a treatment regimen.
In the Clinic of Vascular Pathology Sergey Lirnik uses unique therapeutic patented techniques. The world-renowned surgeon combines therapeutic and surgical methods for the treatment of both stage 3-4 cellulite and varicose veins, lymphostasis.
What is the secret of your approach?
The principle of my work is based on the classical approach to medicine. The treatment mechanism is based on scientific developments that have been successfully applied to the rehabilitation of pilots in the aerospace and military fields.
How do classical principles of medicine differ from modern ones?
Medicine used to be perceived as a service to society. There was an institute of zemstvo doctors, when the doctor knew not only about the pathology of the patient, but also had the opportunity to track hereditary predisposition to certain diseases.
Today medical schools train "family" doctors. How do you assess the level of training of young personnel?
Total commercialization of education dealt a big blow to the medical staff. The problem is not only that most students go to study medicine not out of their hearts, but out of financial considerations, but also in the fact that professionals and medical specialists who have valuable knowledge avoid sharing their experience with colleagues and students.

What caused this informational isolation?
The pursuit of profit. In the medical community, this is a powerful tool to slow down the progress of technology, which affects thousands of patients every day. This problem could be solved by the state medical insurance program, which would be based not on the principles of increasing the capital of financial structures, but on social humanity.
You practice not only in Ukraine, but also in Israel and Germany. Surely you know the facts of a successful insurance program in other countries...
Israel can serve as an example. The cost of compulsory insurance for Israeli citizens is more than moderate compared to the level of the average monthly family income. And at the same time, commercial health insurance is developed there, the difference in payments for which, in comparison with state insurance, fluctuates between 30-50 US dollars.
How do colleagues from other countries treat our specialists? Many of our compatriots prefer to be treated in Israel and Germany. Is there really a high level of diagnosis and treatment there?
Today, Ukrainian medicine still has scientific potential, on which it is possible to build a developed institute of social medicine. The developments of our compatriots are highly valued in the world community.
Do you communicate with colleagues through special forums or web resources?
Vascular surgeons from all over the world often turn to me for advice. Many Western doctors, when faced with the problems of treating rare diseases in this area, first of all remember my works and publications on this topic.
Effective Vascular Treatment: Dr. Lirnik Vascular Pathology Clinic
What was the recognition of you as an absolute authority for Western experts in this field?
Firstly, 30 years of practical and scientific experience in vascular surgery, based on the developments of Soviet scientists. But last but not least, the openness to a collegial dialogue is important, which allows you to build friendly relations with professors of many Western clinics. Unfortunately, in our realities it is still difficult to establish such relationships. To be honest, Ukrainian specialists prefer to simply "borrow" photos of treatment results to advertise their own practice, without going into the scientific and theoretical side of treatment methods. They are not so much trying to cure the patient as to "advance" themselves.
What is the difference between the old school approaches of vascular surgery and the modern one?
The problems of vascular surgery are only gaining momentum today. First of all, this is facilitated by the lack of clinical thinking in most doctors. Surgeon – he is a well-armed therapist who pursues an ailment and looks for its causes, like a detective of a criminal. Analytics in work with patients – basis for effective treatment and prevention of diseases. But the vast majority of doctors prefer not to delve further than their core area.
How do patients know when to see a vascular surgeon?
Take, for example, varicose veins. A huge number of women suffering from this disease could be cured in the early stages of the onset of the disease, if gynecologists began to pay attention to its first signs. But neither women's doctors nor the patients themselves are thinking about this yet.
Varicose veins are now on the first place among vascular diseases?
Yes, and we can expect a rapid increase in the number of patients with varicose veins in the very near future. If until recently the causes of the disease were hereditary factors, working conditions and severe cardiovascular pathologies, today a problem has been added to them, which for the most part is far-fetched - cellulite.

This disease is treated by cosmetologists, but the cause of its occurrence lies in the vascular pathology?
Let's start with the fact that cellulite is an advertising term, but in the very near future it threatens to cause varicose veins in many patients. More precisely, not cellulite itself, but some ways of dealing with it, which are imposed on us by the beauty industry.
What do you mean by "advertising term"?
The term "cellulite", as a cosmetic defect, was first coined in 1973 in New York. Since then, multibillion-dollar sales of miraculous "anti-cellulite" creams have started. Meanwhile, "cellulite" is a medical concept that refers to acute inflammation of the subcutaneous tissue. And it has nothing to do with the infamous "orange peel effect".
The beauty industry lives by its own laws, separate from the standards of medicine.
For many years, millions of women have been consumed by the chimera of "cellulite treatment". And how many services and methods of dealing with it have been invented! And on this cosmetology front there is a whole army of specialists who are far from the issues of medicine and varicose veins.
What is your attitude towards cellulite treatment in aesthetic medicine clinics? After all, most often they fight it as a cosmetic defect.
It is unlikely that any of us have not heard of anti-cellulite massage. Manual, hardware, can. Painful to the point of loss of consciousness and not so much. As masseurs say: "Swelling and bruising - this is just a sign of a successful anti-cellulite therapy session." Is that right?
Isn't that right?
So, however, along with them, the thinnest fragile lymphatic capillary system is broken. The circulatory system is disturbed and after a while the "orange peel" appears with a vengeance. The vicious circle closes, but the worst thing is that in a few years the ranks of patients with pathological disorders of blood flow in the lower extremities will be replenished with such stubborn victims of the fight against "cellulite".
So what is cellulite from the point of view of a vascular surgeon, and are there methods to combat it?
Exist. Let's take a comprehensive look at the phenomenon that in the modern world is called cellulite. Irregularities of the skin occur due to loss of elasticity and stretching of collagen fibers. Weakness of the collagen structure and friability of tissues contribute to the appearance of fatty tubercles on the surface of the skin, which, with normal collagen density, would be evenly distributed in the layers of the epidermis.
So cellulite is not a problem of excess fat?
The problem of cellulite, as a cosmetic defect, in most cases is not associated with the problem of excess weight. A slight weight gain of up to 5% with a weakened collagen structure provides a deterioration in the appearance of the skin and the very insidious reverse effect after sessions of anti-cellulite procedures. In medicine, this phenomenon is called lipoid lipodystrophy.
So this is a real tissue disease?
This is a syndrome bordering on a healthy state, which is expressed in a violation of the microcirculation of the circulatory and lymphatic systems. Only the restoration of healthy metabolic processes in the skin can contribute to the strengthening of the collagen structure and the disappearance of cellulite.
Therefore, no traumatic massage procedures are effective for these purposes in the long term.
And anti-cellulite creams? Do they help?
Human skin is intended to be a barrier substance, the task of which is to protect internal organs from external factors, and not a single miracle cream can penetrate deep into the epidermis. They can only have a temporary effect due to irritation of the capillary system. That is why in France the term "anti-cellulite cream" is prohibited by law.
How do you think cellulite should be diagnosed and treated?
Treatment and prevention of lipoid lipodystrophy should begin with a thorough examination of the patient, determining the state of his circulatory system, hormonal levels, analyzing lifestyle and nutrition. We offer proven hardware compression therapy procedures aimed at improving blood microcirculation. The mechanism of compression massage is based on scientific developments that have been successfully used for the rehabilitation of pilots in the aerospace and military fields. In addition to hardware procedures, in order to get rid of cellulite, it is necessary to adhere to a culture of balanced nutrition.
I'm talking about a balanced diet, as most modern diets do not take into account the mechanism of metabolic processes in the body. Abuse of a protein diet contributes to the rapid combustion of subcutaneous adipose tissue, but at the same time provokes a weakening of collagen fibers and sagging of the skin.
Fast-based diets can lead to irreversible changes in liver cells, and pose a real threat to the lives of patients.
But some look at girls with model appearance and mistakenly take thinness as a sign of beauty, and therefore health. Anorexia is common enough to be seen as a psychological problem.
Anorexia can also be caused by a mental disorder, but then, as a result of prolonged fasting, the cellular structure of the liver is disturbed, as a result of which the body is not able to function and digest food in a healthy way. Medicine is powerless to help at this stage by methods of psychological rehabilitation.
What kind of physical activity is it right to give to your body so that the vessels are in good shape? Many argue about the dangers of running and the benefits of walking. Step training has been in fashion for a long time, and now Pilates. How to properly distribute the load?
The most important factor in the fight against not only the "orange peel", but also the entire group of diseases of the lower extremities associated with circulatory disorders, is the conscious control of a healthy lifestyle. God created man to move, not to stand or sit. The entire anatomy and constitution of the human body is "designed" to move in an upright position. The highest venous blood pressure is observed in the legs, so there is the most vulnerable area of blood flow. Unfortunately for women, their collagen fibers are much weaker than men's, which affects the aesthetics, but both women and men are equally affected by severe disorders of the circulatory system.
Causes of disorders of the circulatory system of the lower extremities:
- excessive load on the legs,
- vibration operation,
- prolonged immobility of the lower extremities.
What makes your treatment principle unique?
Modern medical practice tends to separate approaches in the treatment of diseases of the venous, arterial and lymphatic systems. This is the main reason for the ineffective treatment of vascular diseases. Violation of tissue trophism entails a malfunction of both arterial inflow and venous outflow, and lymphostasis of the lower extremities. My method of fighting diseases - starting from "cosmetic cellulite" and ending with such severe ischemic pathologies as trophic ulcers - is based on the principles of a comprehensive restoration of arterial blood inflow and venous blood outflow. The therapeutic effect is achieved by stimulating blood flow, which, with the right therapy, can be increased up to 1000%.
In this way, the venous system is unloaded, lymph flow improves, swelling and pain in the legs disappear. Cosmetic efficiency of this technique is 92%.
You spoke about the application of developments for space technologies. What is their essence?
Cosmonauts and pilots experience extremely high loads on their vessels during flights and after returning to earth. An analysis of the state of blood vessels and the antioxidant defense system at the early and late stages of the recovery period, and the determination of the role of hypergravitational influence made it possible to understand the mechanisms of human adaptation to a complex of factors that accompany the transition from weightlessness to terrestrial conditions. The research and discoveries made by doctors with the support of physicists outlined the basic principles for correcting adverse changes in the body of astronauts. It is these principles that form the basis of my author's method of compression therapy.
