The market for fillers is filled with a wide variety of drugs, and this stimulates their manufacturers to create more and more competitive products. Doctors estheticians can not but rejoice. However, their clients are often apprehensive about the use of fillers for contouring and refuse the procedure even if there are objective indications. Among the reasons for refusal, they cite distrust of drugs. Often, patients who are objectively indicated for contour plastic surgery refuse it and say that they are not ready for such a procedure. There are many reasons for this refusal. Among them there are also psychological – fear of seeing changed facial proportions after the procedure and not accepting a new appearance. There are also reasons associated with distrust of the quality of the fillers themselves. At the same time, many of the patients do not refuse in principle injection procedures. I have seen many times when they undergo redermalization and other injections of drugs that improve the quality of the skin, but avoid fillers.

Depending on the concentration of hyaluronic acid, five types of redermalizers are known. Then there was a short-term experience of using fillers not of our own production.

The ability of a dermal filler to regulate fluid accumulation in tissues
Institute Hyalual's Alexa line of fillers consists of three products of different densities: Alexa Smooth (15mg/ml), Alexa Medium (17.5mg/ml) and Alexa Volume (20mg/ml). Alexa Smooth perfectly corrects small dynamic wrinkles and improves skin quality. Alexa Medium is suitable for lip augmentation, medium depth wrinkle correction. Alexa Volume – an effective volumizer for the correction of deep creases and replenishment of tissue volume in case of its deficiency.
Alexa fillers feature a physiological pH (7.2 – 7, 4) as well as a unique PbS buffer system.
Due to the buffer system, post-traumatic edema after the procedure is very moderate. When working with these drugs, you do not need to make allowances for the fact that hyaluronic acid will draw water. Alexa fillers accumulate liquid in a ratio of 1:1 in relation to the injected gel. This allows you to get a clean volumizing effect without the risk of fluid accumulation in the injection zone.
The filler does not contain lidocaine, so it is ideal for patients with a burdened allergic history. Typically, injection pain occurs for two reasons:
• the volume of the injected drug causes an unpleasant feeling of fullness;
• The pH level of the product is very different from the intradermal acid-base balance.
The pH level of Alexa fillers is almost the same as intradermal pH, so the injection of the drug for the patient is as comfortable as possible and does not cause pain.
It is worth noting the minimum amount of residual BDDE in the product (less than 1 ppm) – this is even less than the FDA recommended limit.
Practical Experience with Alexa Hyalual Dermal Fillers
On the basis of the scientific and practical center in Kyiv, preclinical trials of Alexa fillers were carried out. Study design: 115 people.
• Group A: 15 women from 35 to 50 years old. Medical device – Alexa Smooth (15mg/ml).
• Group B: 60 women from 30 to 45 years old. Medical device – Alexa Medium (17.5 mg/ml).
• Group B: 40 women from 25 to 45 years old. Medical device – Alexa Volume (20 mg/ml).
All correction decisions were made at the discretion of the investigator, who was guided by the appropriate instructions for the use of the drug (provided in the instructions).
Before the injection and after it, photofixation was carried out in dynamics on the 14th, 30th, 60th and 90th day. In group B, an additional examination was carried out on a Dub skin scanner during the first 7 days after the procedure.
During the study, most patients noted the following subjective sensations:
• comfort of drug administration,
• absence of pronounced post-traumatic edema,
• satisfaction with the procedure as a whole in dynamics.
Filler injection technique:
The Alexa Medium and Alexa Volume preparations are administered equally comfortably both with a needle and using a cannula. The Alexa Smooth filler is very plastic, so it is injected very superficially. It shows excellent ability to fill dynamic and static creases in the finelift technique. After the injection of Alexa Smooth, the bottom of the epidermal/dermal defects was significantly raised in dynamics.
