At the congress held in Kyiv within the framework of the International Association of Aesthetic Plastic Surgeons (ISAPS) on May 19 and 20, a report was presented by Professor Daniel Cassuto, Plastic Surgeon, Professor of Plastic, Reconstructive and Aesthetic Surgery, Department of Plastic Surgery, University of Modena Hospital (Italy) , international expert Merz-Aesthetics. Professor Cassuto shared with estet-portal.com his opinion about the event, the most important tasks of plastic surgeons and the most effective facial rejuvenation techniques.
How do you feel about the idea of hosting events such as today's congress, where real operations from top plastic surgeons will be demonstrated?
- I think these kinds of events are the best way to get the latest information. Of course, familiarization with the scientific literature is mandatory, however, as a rule, it takes a long time to evaluate one's own technique, the results of its application and their publication. By the time we can see the results of the application of new techniques, it will be several years. And at such congresses, we have the opportunity to observe the current work of the most gifted and talented colleagues from all over the world. This allows us to stay up to date and improve our daily clinical practice.
What do you think are the strengths of our plastic surgeons and what should they learn more about?
- I have met many very, very good surgeons – beautiful, smart people with skillful hands. I travel a lot, communicate with many colleagues, and I believe that Ukrainian plastic surgeons are not much different from their foreign colleagues. I guess all plastic surgeons have a few common flaws. First – as a rule, we do not improve the quality of fabrics, but only change their position and quantity. It is these limitations that we must learn to overcome. We have learned how to use adipose tissue to improve our performance, now we have to learn how to use other substances. Second – we pay too much attention to techniques, but first of all it is necessary to improve our indications, i.e. understand when it is really necessary to carry out the procedure and, more importantly, – when it doesn't need to be done.
What methods of combating facial skin aging do you think are the most effective today?
- As I said earlier, it must be remembered that our face ages at the levels of all tissues – bone, fat, connective and, finally, skin. If we work only with the skin all the time, we will not be able to provide natural results. If, however, we begin to see the aging face in three dimensions and take care not only of the quantity, but also of the quality of fabrics – we will get a combined medical method of skin rejuvenation. And this is much more important than just trying to eliminate small imperfections in one area or another.
What role do minimally invasive techniques play in the practice of a plastic surgeon?
- This is the future that has already arrived. Very often in traditional operations we need to open large areas in order to solve rather limited problems. Today, we have minimally invasive surgical techniques that do not result in significant tissue damage to the patient. For example, with an optical fiber only a few hundred microns in diameter, we can penetrate even deeply located areas in order to change their shape. In this way, we can achieve excellent results. Of course, this requires an ideal knowledge of anatomy, since it is impossible to see everything during the operation. This is the first. And second, as I said earlier, we have to combine tissue enhancement and projection, restoring a certain amount of volume that the face inevitably loses with age.
