Cosmetic novelties in the field of anti-age therapy of facial skin cannot but please cosmetologists, as they make it possible to effectively slow down the manifestations of aging and select a variety of individual face care programs for patients. OnCosmetics once again invited doctors to a training seminar where they could learn about the age-related features of choosing a drug for deep facial volumization.
The speaker of the seminar was Andrey Viktorovich Danilyuk, an aesthetic surgeon, a specialist in regenerative medicine, a certified trainer of OnCosmetics for invasive techniques in aesthetic medicine.
The first signs of aging and their correction by giving volume to the face
According to Andrey Danilyuk, the speaker of the seminar, the choice of preparation and technique for facial rejuvenation should be based on the patient's own perception of his appearance flaws. The concept of age is very relative – it largely depends on the attitude of the patient to himself. The doctor works only with those changes in appearance that, in the opinion of the patient, make him old. It is important how the patient sees himself in the mirror and with what requests he comes to the doctor. For example, the formation of the nasolabial fold often depends on a decrease in the volume of the face in the middle third, although the patient does not see this. Main – to give the patient self-confidence, and not to achieve an unknown ideal of beauty and youth. At the same time, the beautician must imagine the final picture of what he wants to achieve, he must be a little sculptor,
There is one of the theories of aging – theory of damage accumulation. According to this theory, anabolic-catabolic cellular transformation occurs with age. After about 25 years, tissue repair processes begin to slow down and cellular damage accumulates. The processes of cell destruction are accelerated. The volume of tissues decreases by about 1% annually. Under the influence of age-related changes in the process of "bone remodeling" bone resorption begins to predominate.The soft tissues of the middle zone of the face undergo the most pronounced changes.
With age:
- deepening of the nasolacrimal sulcus increases,
- painting (cheekbones) bags are progressing,
- sagging of the nasolabial fold increases,
- the line of the lower jaw is deformed.
The right choice of the optimal preparation for giving volume to the face
The most effective in correcting age-related changes associated with the loss of volume will be preparations that model the foundation, "foundation"; all tissues of the face. These are solid supraperiosteal implants.
The speaker reminded the guests of the seminar that, before starting work with a patient, one should remember to be careful when passing through the so-called zones of special attention: the exit points of the neurovascular bundles.
To create facial volume, we have 2 groups of preparations: based on high density hyaluronic acid – 26 mg/ml and based on calcium hydroxyapatite.
Approach to the choice of drug: in young patients, drugs based on reticulated HA are preferred, since they are denser, they have better volume and shape modeling. For the purpose of anti-aging, it is more advisable to use a drug based on HAC, since they give a prolonged effect, provide tissue lifting due to neocollagenogenesis, they exert minimal pressure on the surrounding tissues and do not aggravate dystrophic changes in them.
Dr. Andrey Danilyuk shared with the audience his own method of introducing drugs into the correction zone: during supraperiosteal injection of the drug with a needle, he recommended first pierce the skin with the cut of the needle down perpendicularly, and then turn the needle about 30 degrees with the cut down to the periosteum. It is important to put the cut of the needle on the periosteum. With this technique, you will never get ischemic complications.
In the correction of age-related changes, according to the speaker, preference should be given to the drug Delicehidroxy. Calcium hydroxyapatite is a biocompatible material. It provides:
- immediate volume effect,
- has the same mineral composition as bone, the same porous structure (supports fibrovascular ingrowth),
- The filler stimulates the synthesis of its own collagen and is a scaffold for further collagenogenesis,
- is not subject to resorption, which contributes to stable long-term results,
- does not require prior allergy testing.
The technique of drug administration is peculiar: Andrey Danilyuk recommends using a cannula. The bolus method of administration will give volume to the face, but will not provide lifting. The fan-shaped introduction of cannulas with the laying out of the drug will linearly give both volume and lifting in the corrected zones.
At the end of the seminar, the speaker reminded the doctors of some anatomical nuances of the facial structure that allow competently performing the volumization procedure without complications.
