Клинический случай: коррекция зоны вокруг глаз

The article presents two clinical cases that describe volumetric correction of the eye area.

In the first case, we are talking about treatment of a complication that occurred after the introduction of a hyaluronic filler into this area. The second talks about camouflage of hernia sacs under eyes in order to harmonize and improve appearance.

The main mechanisms of aging of the soft tissues of the face —

gravitational ptosis

and
volumetric changes

 — have been studied quite well by now.My default image

Their severity and significance vary from one area of the face to another, which is due to the embryogenetic, functional and anatomical features of the structure of soft tissues.

The medial area of the face

seems to be one of the most difficult to correct both due to its anatomical structure and in terms of a combination of gravitational and volumetric changes in soft tissues.  Clinically, this is expressed in the appearance of depression zones, as well as progressive

gravitational changes

in the form:

nasolabial folds;

flattening of paint elevations;

hernial protrusions of intraorbital fat in the projection of the lower eyelids. All these

changes
    give the face an aged, tired, haggard look.
  • Lacrimal
  • and
  • palpebromalar furrows
  • can be not only a manifestation of age-related changes, but also exist as an anatomical feature at a young age.

    Even in the absence of hernial protrusions of the lower eyelid and a tendency to edema, correction of the area of the lacrimal and palpebromalar grooves presents certain difficulties.

To minimize the risks of developing possible

complications

, some rules must be observed:

The hyaluronic acid preparation must be injected deep, supraperiosteally;

The volume of the administered drug should not exceed 1 ml on both sides in one session;
Correction in the projection area of the angular artery and vein using a needle or cannula is unacceptable due to the possibility of embolization or compression of the vessel

in.

hernial protrusion

of the lower eyelid significantly complicates the work of a cosmetologist and opens up opportunities for the work of a plastic surgeon. However, quite often, patients, for whatever reason, are not ready to perform the indicated surgical intervention.

In this case, one of the methods of correction can be

camouflage of the hernial protrusion

by filling the lacrimal and palpebromalar grooves. This delicate area places high demands not only on the injection technique, but also on the filler used.

Basic requirements for the filler used to correct the area around the eyes:

low hydrophilicity,

good fluidity;
sufficient elasticity.

All these properties are possessed by

  • The experience of using this filler showed
high degree of satisfaction

of patients with the results of correction, reliability, absence of side effects associated with the physicochemical characteristics of hyaluronic acid preparations, — the drug did not migrate, did not palpate, did not increase swellingb.

complication

after correction of the lacrimal and palpebromalar sulci, performed for 6 months. before that filler based on hyaluronic acid.

The complication was manifested by drug contouring, the Tyndall effect, and tissue swelling in the projection of the drug administration (

Fig. 3A

). During the examination, a superficial — between the skin and the circular muscle of the eye — the location of the drug, causing deformation of the skin contour and a typical discoloration of the skin of the compromised area. During the first session intralesional injection of

longidase 3000 I.U.

diluted with 4 ml of NaCl in a volume of 0.3-0.4 ml, a complete reduction of the incorrectly located filler was achieved ( Fig. 3B).

2 weeks

after the follow-up examination, the lacrimal and palpebromalar sulci were corrected with YVOIRE classic, presented by «Academy of Scientific Beauty".

The drug was administered with a 25G × 40 mm supraperiostally at a dose of 0.5 ml on each side (Fig. 3C). After the end of the injection, the drug was distributed with massage movementsand.

Clinical case 2: camouflage of lacrimal and palpebromalar sulci

Patient L., aged 36, complained of
tired appearance

. During the examination, hernial protrusions of the lower eyelid were diagnosed, accompanied by the presence of lacrimal and palpebromalar furrows of type I & ndash; II according to F. Nahai (Fig. 4AMy default image).

Sent for a consultation with a plastic surgeon, but refused the proposed surgery.

In order to harmonize and improve the appearance,

injection procedure

camouflage of the lacrimal and palpebromalar grooves with YVOIRE classic at a dose of 0.4 ml on each side was performed. A patient-satisfied esthetic result has been achieved (

Fig. 4B

).

The lacrimal and palpebromalar sulci are among the most sought-after and difficult correction areas.

Adequate aesthetic result

can only be achieved if:

My default imagecompetent diagnostics;

use of high-quality hyaluronic acid preparations, which include
YVOIRE line preparations
;

compliance with the injection protocol, which ensures the prevention of complications and adverse events.

    With the advent of innovative
  • hybrid fillers
  • that meet the requirements of these delicate areas, the possibilities of aesthetic medicine are expanding, allowing you to achieve good results and avoid possible complications.
