Resorption of bone tissue, reduction of tissue elasticity and redistribution of volumes – age-related changes that occur in all areas of the face. In the lower third, they lead to deformation of the lower jawline, the appearance of a double chin and/or jowls. Accordingly, patients, especially women, turn to a cosmetologist with a request to eliminate the above disadvantages and tighten their faces. You can get the desired effect with dermal fillers based on hyaluronic acid.
This article by estet-portal.com presents a case from the practice of Dr. Paul Baines, in particular, lifting of the lower third of the face with HA fillers.

Consultation before lifting the lower third of the face with HA fillers
A 41-year-old patient came to Dr. Paul Baines' clinic for a full facelift without focusing on specific problem areas. To achieve this goal, it was necessary to work with all three lifting vectors: in the area of the cheekbones, chin and lower jaw.
The patient agreed to the proposed procedure because, according to Dr. Baines, the chosen approach allows for the maximum lifting effect and at the same time preserves the naturalness of the face.

Choosing a hyaluronic acid-based filler for lifting the lower third of the face
In the middle third of the face, Dr. Baines used the Restylane Volume filler for correction, which, according to the author, based on his own experience, is quite soft, provides good contouring and expressiveness of this area.
In order to lift the lower third of the face – along the line of the lower jaw and in the chin area – the doctor worked with Restylane Lyft with a higher G’, which allows to create a good projection. The doctor injected the same filler at a point behind the angle of the upper jaw to provide precise lifting without the need for lateral volumization.

HK filler injection technique for lifting the lower third of the face
In the chin area, the author injected approximately 1 ml of Restylane Lyft hyaluronic acid filler at six points: three on each side of the midline.
The chosen approach allows you to get the maximum lifting effect and at the same time preserve the naturalness of the face.
In order to increase the vertical length and anterior projection of the chin, the drug was injected supraperiostally under the mental ligament. Since the needle inevitably becomes blunt with this insertion technique, the author prefers to use a new needle after each injection to reduce the risk of bruising. Since even a small amount of Restylane Lyft filler provides a good correction result, the loss of a meager amount of the drug when changing the needle is not significant.
A personalized approach to the correction of the middle and lower third of the face
The exact location of the posterior injection site will vary by patient. In this case, the author injected one 0.5 ml bolus of Lyft directly onto the periosteum at a point behind the angle of the maxilla. Having opened the patient's jaws, the doctor determined the contour of the back of the angle of the upper jaw by palpation. Placing the needle exactly at the angle, the doctor fixed the edge of the upper jaw with his thumb. After performing the aspiration test, the author injected 0.5 ml of filler. Immediately after insertion, there is an immediate retraction of the soft tissues and the effect of lifting the jaw line.
Then the doctor performed similar manipulations on the second half of the facesa.
Results of lifting the lower third of the face with fillers based on hyaluronic acid
There were no side effects and complications after lifting the lower third with HA fillers, the patient, completely satisfied with the result of the correction, returned to work the next day.
Before and after lifting of the lower third of the face with HA filler
After 4 weeks, a second visit to the doctor was scheduled; at this point, the product is fully integrated into the tissues.
Correction results are retained for approximately 9-12 months.
Correction results are retained for approximately 9-12 months. After 6 months, it is advisable to evaluate the results obtained and determine the need for a corrective procedure.
Bacterial biofilms after fillers: a guide for cosmetologists
The author prefers to wait until the filler is fully integrated before administering additional doses of hyaluronic acid filler. This also minimizes the risk of biofilm formation, which increases with larger drug boluses.
