The effect of low-level laser therapy on tissues and its beneficial clinical effects is called photobiomodulation.

NLT is widely used in dentistry, neurology, pain management, physical therapy and other areas of medicine. The effect of NLT is not fully understood by the medical community, however it is known that in addition to activation of cellular metabolism through stimulation of the respiratory chain (primary mode of action), there is also cell signaling (secondary mode of action).

Mechanism of action of low-level laser therapy

NLT has an effect on adipose tissue, which is associated with the absorption of infrared photons by chromophores in mitochondria. This is followed by a chain of events:

  • potential of the mitochondrial membrane increases;
  • oxygen consumption increases;
  • there is a short-term increase in reactive oxygen species and the release of nitric oxide (NO).

The mechanism of action of NLT on adipose tissue remains somewhat controversial. The use of NLT to remove fat has no visible effects on the surrounding tissues and does not increase the temperature of the tissue, unlike a procedure called laser liposuction.

Wavelengths between 630 and 640 nm have been proven to be optimal for biomodulation.

Scientists explain the effect of NLT on adipocytes by the formation of transient micropores in the adipocyte membrane, through which fat seeps from inside the cell into the interstitial space. This phenomenon can be explained by the fact that NLT stimulates an increase in the level of ROS, resulting in lipid peroxidation of cell membranes, due to damage to which pores are formed.

Another proposed mechanism is that elevated levels of cAMP may activate protein kinase which may stimulate cytoplasmic lipase, which converts triglycerides into fatty acids and glycerol, which can pass through xxxx>pores in cell membranese.

Based on the results of a vast number of randomized, double-blind studies, the US FDA has approved NLT as a non-invasive aesthetic treatment to reduce hip, waist, and upper abdominal circumference for patients with BMI (mass index body) between 30-40 kg/m2.
