The effect after correction of the lower eyelid with dermal fillers, as a rule, lasts for 1-3 years. HA also provides good volumization, contouring and lifting of the lateral part of the eyebrow.

Filling brow wrinkles with HA fillers

The correction of glabellar wrinkles is associated with risks of vascular complications, therefore, in most cases, botulinum therapy is the gold standard for working in the glabellar area.

For the administration of HA fillers, the author always uses a 22 gauge cannula and inserts it from a point located on the top of the forehead. After that, the instrument is inserted several times under the folds, using the subcision technique, to create space for the injection of the filler – otherwise, lateral displacement of the preparation is possible and, as a result, deepening of wrinkles.

The author always compresses the supratrochlear and supraorbital arteries on the treated side of the face in order to reduce the risk of intravascular filler injection. It is important to inject the drug slowly and immediately stop the injection in case of signs of vascular damage, then massage the area and inject the appropriate dose of hyaluronidase.

"Wrinkles of anger": correction with botulinum toxin

Superficial injection of fillers such as Restylane Silk or Belotero Balance, using an intradermal injection technique directly into the frown lines, provides a satisfactory effect without the risk of vascular damage.

HK fillers in the mid-face and crow's feet

As in the case of brow wrinkles, botulinum toxin or hardware techniques are more suitable for the correction of crow's feet.

To improve the condition of the area around the eyes and smooth out the wrinkles under the lower eyelid, it is often enough to inject the filler into the nasolacrimal and palpebromalar grooves.

For crow's feet, the technique of superficial injection of very small amounts of preparations such as Restylane Silk or Belotero Balance can be used to smooth them out. To replenish volumes lost with age in this area, a small amount of filler can be injected slightly lateral to the outer corner of the eye.
Highly elastic fillers (eg Juvederm Voluma, Radiesse, Restylane Lyft) are great for creating attractive curves in the middle third of the face. They provide good tissue support and can often reduce the appearance of the infraorbital sulci without direct correction.

Improvement of the contour of the cheekbones and lifting of the nasolabial folds, as well as a visual reduction in the length of the lower eyelid – effects that can also be achieved with the aforementioned drugs.

In most cases, for lifting and revolumizing the middle part of the face, the author uses the technique of deep deposition of the filler on the periosteum. The introduction of HA should be started from the upper point of the zygomatic arch, located slightly below and lateral to the outer corner of the eye. Correction is carried out with a 22 gauge cannula. If the doctor works with a needle, it is inserted perpendicular to the skin to the level of the periosteum. Without removing the needle, the author injects the drug until the desired effect is achieved.

The amount of the injected drug depends on the patient and is 1-2 ml of filler (for both sides of the face).

Eyebrow contouring and lifting with HA filler injections

For brow contouring and lifting, the author injects an average of 0.2-0.4 ml of hyaluronic filler using a 22 gauge cannula. The choice of the instrument is determined by the location of the superficial temporal artery/vein in close proximity to the fat pads of the eyebrows.

It is important to inject the filler deeply to avoid contouring of the filler, which can occur if injected too superficially.

The drug is injected into the lateral part of the eyebrow, which is approximately 30-40% of its total length.
To improve the lifting effect, several units of botulinum toxin can be injected into the lateral part of the orbicular muscle of the eye.

To emphasize the peak of the arch of the brow, you can inject more filler into this area and gradually reduce the amount as you move towards its outer edge.

HA fillers can be used to optimize the results of lower eyelid surgery, as well as to correct post-traumatic deformities.

How to prolong the anti-aging effect of hyaluronic acid fillers

To reduce the severity of dark circles under the eyes, as well as maintain the results of correction with HA fillers, the author prescribes to patients:

•    HA Intensifier (SkinCeuticals, USA) – topical serum based on HA;

•    AOX Eye serum (SkinCeuticals, USA) – topical serum based on vitamin C, phloretin and ferulic acid;

•    retinol;

•    medical moisturizers;

•    Products with physical sunscreen.

The author often combines HA filler injections with botulinum therapy in the area around the eyes, since neuromodulators reduce the rate of mechanical destruction of HA in the dynamic areas of the face.
