Омоложение средней трети лица у мужчин инъекциями филлеров на основе ГК

In the vast majority of cases, aesthetic medicine specialists work with women. However, the shape and proportions of an attractive male face have their own characteristics, and their observance – the key to obtaining successful results of anti-aging treatments.

In this article, Dr. Tahera Bhojani-Lynch describes a case of male midface correction with HA fillers, focusing on clear lines and angles.

Male facial rejuvenation: how to maintain attractive proportions

Any features that make a difference between a male and female face are considered more attractive in the eyes of the opposite sex. In men after puberty, under the influence of growth hormones and testosterone, the expression of skin ligaments on the face increases. In the middle and lower thirds of the face, the distinctive male features are:

•    angular lower jaw;
•    clear lines of chewing and zygomatic skin ligaments;
•    good tone of the subcutaneous muscle of the neck.

In Fig. 1 shows that male features are determined by the width and angle of the chin and jaw, flat cheeks and eyebrows less curved than in women. The shape of the face in men is more square – the distance between the cheeks and the edges of the lower jaw are almost the same; in women, the lower part of the face is much narrower than the middle third. A sharper chin, rounded cheeks and arched eyebrows – also typical features of the female face.

In the middle third of the face in men there is a zone that makes the face more masculine. This is the space formed by the masticatory cutaneous ligament anteriorly and the zygomatic cutaneous ligament superiorly. Filling this "male cavity" can visually rejuvenate the face, but at the same time softens the angles of the male face.

Filling this "hollow" can visually rejuvenate the face, but at the same time softens the angles of the male face.

A decrease in the volume of the deep medial fat pad in a man leads to a loss of support for the infraorbital fat compartment, resulting in "bags" under the eyes. The nasolabial fold becomes more pronounced, and the furrow in the middle third of the face gives the face a tired look.

Effective correction of the male face involves filling the middle third without creating bends. When working, it is necessary to strive to maintain the angularity of the profile and the natural plane of the face. Clear lines must be observed:

•    temporal crest;

•    edges of the eye orbit;

•    zygomatic arch;

•    chewing skin ligament. When working with the middle third of the face, it is important to assess the state of the jaw line and, if necessary, correct it to maintain the natural proportions of the face.

What do male patients want: current issues in aesthetic medicine Fillers for the correction of the middle third of the face in men

To correct the middle third of the face in men, the author uses HA-based fillers. Ideally, the product should be cohesive and provide good projection and be suitable for deep periosteal injection. Equally important is good integration into soft tissues – due to this, safety is ensured during subdermal administration of the drug by cannula.

The procedure is performed either with a needle that is inserted all the way to the bone at the point of maximum volume loss; or with a cannula that is inserted along the zygomatic arch to the deep medial fat compartment.

When working with young patients, the author prefers to use a cannula, and with older patients – needle,
Case Study: Rejuvenation of the middle third of the face with HA fillers

A 54-year-old patient presented to the author's clinic complaining of bags under his eyes (Fig. 3). During the examination, it turned out that the cause of the appearance of bags under the eyes was the loss of volume in the middle third of the face. Therefore, to solve the problem, it is necessary to perform a correction of the middle third of the face with HA-based dermal fillers.

Masculinity in aesthetic medicine: the secrets of masculinization

After that, to recreate the aesthetic appeal of the face, it was necessary to correct the shape of the cheekbones and jawline. The final stage – the introduction of the product under the zygomatic arch at the top point of the masticatory skin ligament. In this way, a clear outline of the typical "male cavity" is provided. and if necessary – support for the nasolacrimal trough. The close location of the angular artery to the target area necessitates the use of cannulas. The patient was injected with 3 ml of cohesive HA filler, which integrates well into tissues.
Five days after the procedure, the patient had hematomas in the area of the right eye.
To enhance the rejuvenating effect (Fig. 3), the patient underwent botulinum toxin injections in the area between the eyebrows and the frontalis muscle. The patient was completely satisfied with the result.
