What plastic surgeries are especially relevant today among your patients
PD As the National Secretary of the International Association of Aesthetic Plastic Surgeons ISAPS, I have the opportunity to observe various global trends and obtain statistical data. I can say that we do not observe any differences between Ukrainian trends in plastic surgery and European ones.
The most relevant plastic surgeries worldwide are breast augmentation surgeries. It is these operations that are given leadership in Ukraine. A very popular plastic surgery is also rhinoplasty.
Which type of plastic surgery is most interesting for you personally
P.D. Nose and chest correction operations. When a girl comes to me with youthful breasts, and after a few hours she rises already with feminine forms – this is amazing. By the way, these patients solve a lot of social problems. Sometimes girls come to me after a period of plastic surgery with children, they even feed them in the clinic, which is proved by the fact that the implant is not a contraindication to breastfeeding.
Which of your plastic surgeries do you consider the most difficult
P.D. The most difficult – these are plastic surgeries on the nose, especially repeated ones, when you have to correct the shape of the nose after injuries. Nose Reconstruction – these are always lengthy and complex operations, but they are successfully performed at the "ANA-COSMO" clinic.
When a person comes in with a nose problem – it is a mass of physical and psychological problems. But when I restore the shape of the nose and I give a person the opportunity to look normally at himself in the mirror – That's fantastic. Yes, this is a huge piece of work and sometimes additional corrections are required. But when after 5 days we remove the plaster for the first time and the patient, seeing himself in the mirror, begins to cry with joy – it can't help but delight.
No other plastic surgery gives such visible amazing results. Because the nose – it's a part of the face that you can't hide. And it is after rhinoplasty that patients are most grateful to the doctor for the work done.
Tell us about your method of harvesting autologous fat for rejuvenation and creating volume with your own fat cells
P.D. In lipofilling, it is important to make the procedure so that the fat takes root, because the transplantation of non-viable fat cells leads to the disappearance of the effect just a few months after the operation. Based on our experience, we have developed our own approach to the selection, preparation and purification of transplanted fat.
The process of patenting our fat sampling method is currently underway, in which an assistant is involved in the process. Usually, the doctor has to work with both the syringe and the cannula at the same time while taking the autograft.
We have invented a special adapter, due to which the doctor's hands are free and he works only with the cannula, while the assistant creates a vacuum and gains fat. This is our know-how.
In addition, we purify fat cells at the minimum centrifuge speed: from 300 to 500 rpm, due to which the membrane of transplanted cells is not damaged and they remain viable.
How do you enrich autograft fat
PD We perform double enrichment of fat: we enrich it with plasma and stromal-vascular fraction. Our clinic uses a special ultrasound machine that breaks down fat and turns it into an emulsion. This emulsion is cleaned with water, centrifuged, and thus we get a variant of the stromal-vascular fraction, which has the maximum concentration of stem cells. It is with this fraction that we re-enrich the fatty substrate.
The method of double enrichment of fat autograft guarantees its maximum engraftment, which allows providing the patient with a lifelong effect.
What is the survival rate of transplanted adipose tissue
P.D. To date, the survival rate of transplanted adipose tissue in our clinic is 80-90%. This is a very high percentage, before it was only 50%, but due to the use of new methods, we were able to significantly increase it.
Tell us about your author's technique correction of full faces
P.D. I have developed a three-level approach to the rejuvenation of the full face, since it has more pronounced subcutaneous fat, these patients also have more Bish's lump, and with age not only the skin goes down & nbsp; face, but also muscles. With the help of ultrasound, I remove the subcutaneous fat, after which I remove Bish's lumps.
If age-related changes are observed – SMAS facelift is performed. This approach gives a gorgeous result after such plastic surgery.
