Плазменное иссечение – инновационный подход к коррекции десневой улыбки

The shape, position and size of the teeth, as well as the gums and lips – factors that determine the attractiveness of a smile. In patients with a gingival smile, there is a violation of the proportions between the visible part of the teeth and gums, which causes the presence of this aesthetic defect.

Traditional approaches to correcting a gingival smile include surgery, laser treatment, and botulinum toxin injections.

The above methods are associated with certain risks (the appearance of asymmetries after operations and botulinum toxin therapy, overheating of the pulp when using laser devices). Plasma excision, according to Dr. Massimo T. Rota (Massimo T. Rota), due to its effectiveness and safety, can serve as a worthy alternative to affordable methods of correcting a gingival smile.

Plasma excision: the essence and advantages of the technique

In aesthetic medicine, plasma excision is used to perform, for example, non-surgical blepharoplasty, smooth wrinkles and eliminate other aesthetic skin imperfections.

In the context of gingival smile correction, plasma can be compared to a lightning micro-discharge that acts directly on the epidermis and connective tissue of the oral cavity.

Plasma energy, which acts precisely on target tissues, is created by ionizing gases in the air. The process is not accompanied by the formation of eddy currents that could damage the surrounding tissues.

Plasma excision with the Plexr Plus system (GMV srl, Italy) has a number of advantages for both the doctor and the patient:

•    non-invasiveness;

•    no risk of asymmetry after treatment;

•    no risk of iatrogenic damage to teeth;

•    lack of bleeding and stitches;

•    no risk of scarring

•    efficiency;

•    short recovery period (less than a week);

•    great results.

A retrospective study: plasma excision as a method of gingival smile correction (H2)
A retrospective review of data obtained as a result of gingival smile correction in patients between September 2016 and June 2017

Inclusion Criteria: Adult patients who smile with 2 mm or more of gingiva visible.

Exclusion Criteria:

•    taking anticoagulants or drugs that increase gingival hypertrophy (eg, nifedipine, cyclosporine);

•    therapy of oral diseases;

•    epilepsy;

•    pregnancy and lactation.

Plexr Plus Plasma Excision is an outpatient procedure. For maximum patient comfort, the target area is pre-anesthetized with an anesthetic spray or by superficial anesthetic injections.

Success of minimally invasive eye contour correction: effects of plasma excision

During the initial consultation:

•    patients were given all the necessary information about the procedure;

•    Written consent has been obtained;

•    contraindications were excluded;

•    an oral assessment was performed;

•    photos were taken before the procedure.

A second consultation is scheduled in a week, and a follow-up visit – 2 months after the procedure.

Plexr gingivectomy performance was assessed using the following criteria:

•    measurable reduction in gingival tissue;

•    changes in the ratio of the visible part of the teeth / gums (the main criterion);

•    photo before and after the procedure;

•    Patient satisfaction.
Full recovery and healing of tissues after plasma excision of the gingival smile takes 4-7 days.

After correction, patients are prescribed alcohol-free mouth rinses and are advised to maintain proper oral hygiene.

How does the gingival smile correction procedure with plasma excision work

Before the procedure
After clinical observation of the patient in a static and dynamic state (during a smile), X-ray examination and probing, which are carried out to determine the boundaries of exposure, proceed to anesthesia of the target area (spray- anesthetics or superficial infiltration anesthesia).

During the procedure
Using a red handpiece (high intensity), plasma is generated in a constant mode, with which the doctor evenly affects the target area. Thus, the doctor forms a new border of the gum contour, sublimating the gingival tissue. After that, you can use the white maniple (low intensity) to align the borders.

After the procedure
After aligning the edges and polishing the teeth, the patient is prescribed a mouthwash with chlorhexidine (without alcohol), which is used twice a day for a week. It is recommended to give up smoking until the tissues are completely healed.

In case of gingival hypertrophy caused by drugs or pathologies, the procedure can be repeated.

The author recommends Plexr Plus Gingival Smile Correction to his patients as it is effective and safe, while providing permanent results that can be enhanced by other techniques (e.g. filler injections or botulinum toxin) if needed.
