Редька-Свобода: дорогой продукт в руках непрофессионала превращается в оружие

Plastic surgery — a kind of sport, the same races in which victory depends on several things at once: the skills of the driver-surgeon, the quality of the fuel, which in our case becomes a filler, as well as the patient’s expectations from the “finish”; — end result.

For the procedure to bring both professional satisfaction to the doctor and the patient, it is impossible to do without a combination of the professionalism of a plastic surgeon, only the best products for administration, as well as adequate plans for the result on the part of the patient.

How to maintain a balance of all three components, a plastic surgeon, associate professor of the Paracelsus Medical University, told estet-portal.com in Salzburg, head of the anatomical course Expert Day Anatomy, member of the German Society of Surgeons, the Society of Mesotherapy, the European Academy of Aesthetic Surgery and the International Society of Aesthetic Medicine, author of scientific articles on volumetric facial modeling Wolfgang Redka-Svoboda (Germany).

How important is it to choose the right facial product?

B. R.-S.: You know, the doctor is simply obliged to find and use only the highest quality products. The other side of success — in the practical knowledge of the surgeon. It turns out that plastic surgery — a sphere that is under the influence of several factors at the same time: its specialists face many tasks on which the result directly depends.

First of all, the doctor is looking for his patient, who will not contradict the obvious, thereby taking up precious time, after — defines a truly problematic area, then selects the most suitable filler for its correction, and only then does the interaction begin.

Remember: the best, most expensive product in the hands of a non-professional turns into the most dangerous weapon!

The conclusion I would make for myself — yes, facial correction products — a significant thing, but the skills of your doctor — even more important!

Which manufacturer's material do you personally prefer?

B. R.-S.: I would not want to go into all the heavy advertising and marketing, especially since no one ordered them from me, but I can still give a good guide to the market for those who need it. Choose big, famous and well-known brands.

If a brand has managed to survive in the face of fierce competition for 10, 15, or even 20 years, it really is worth something.

A wave of criticism falls even on big, famous brands, — the beauty industry also has its own intrigues, — but in most cases it is temporary. I repeat: if in the conditions of a wealth of choice, hundreds of thousands of other brands and fierce competition, someone could not only survive, but also prove his economic viability for 20 years, he is worth something and a priori cannot be bad, dangerous and further down the list.

My personal "instruction for use" not only hyaluronic acid, but also any other — choose the best, don't be fooled by the attractive price: it never justifies itself in the end.

Low price — low production cost, low quality materials, — and hence the low quality of the product itself at the output.

I'll tell you a "secret": there is no cheap production without violating norms and standards, and such savings will certainly not benefit the skin.

What is the fundamental difference between Teosyal cosmetics and other market offers?

B. R-S: I know that Teosyal has several product lines. The first, basic, classic — These are hyaluronic acid fillers. What can I say about them? These are good, high-quality products that are more or less similar to other market offers in the same price category. But recently, just a few years ago, Teosyal began to release a fundamentally new product — RHA (Resilient Hyaluronic Acid — hyaluronic acid that easily adapts to any skin type).

The main feature of Teosyal RHA, presented by "Academy of Scientific Beauty", — the fact that by choosing this remedy, we are dealing with natural hyaluronic acid, a substance of 100% natural origin, packaged for storage and further use. What is the advantage of natural hyaluronic acid?

That the doctor takes fewer steps to make it work on the patient's face. Teosyal RHA — the most elastic substance, with which you do not need to lay a million other chemical "bridges" to see the result. Natural hyaluronic acid — the product is flexible, which easily adapts to the natural mobility of the skin. Imagine bamboo stalks, slingshot rubber band — natural hyaluronate, just like them, returns to its original position without loss, wrinkles and changes.

Are there any products that can compete with Teosyal today?

B. R.-S.: From the point of view of advertising, I would answer a categorical and irrefutable "no". Each sandpiper praises his own swamp, and if I pursued purely marketing goals, I would say something like: “We — the only and unique, we — one company in the whole world that offers something really worthwhile.

No, I won't tell you that. In return, I will share the truth about Teosyal RHA — the product you just talked about. To make it a little clearer and more visual, let's look at an example. Let's say you have a specific task: to get from Tbilisi to Kyiv. How will you do it? You can drive a 1973 Volkswagen or a brand new Tesla S Model. Yes, outwardly everything converges: this is a car and that car, each has 4 wheels, the same trajectory, but inside everything is completely different — different technology, different quality, completely different level of comfort.

Don't compare the incomparable! Conventional hyaluronic acid and innovative remedy Teosyal RHA  — two big differences. Yes, in the hands of an inept racer Tesla, like the Teosyal RHA, can cause trouble, just like a rare Volkswagen in the hands of Schumacher — benefit.

What advice would you give to surgeons who do not use Teosyal fillers?

B. R.-S.: First of all — do not overdo it with the quantity. No need to inject too much filler into the top layer of the skin. A modern technique for increasing the volume of any area of the face — this is primarily a slow, gradual introduction of the filler in two stages: first, the doctor injects into the deep fat layer, and then — into the upper dermis.

Only in this way the result will be natural in the long run, because a properly performed procedure will provide support to the skin from below, thereby preventing sagging and reducing the achieved effect to zero. To summarize, if the doctor uses a classic, non-elastic filler, you do not need to inject too much of it, especially — into the upper skin layer.

Do plastic surgeons know about the topic you spoke about?

B. R.-S.: Yes, absolutely! I am sure that any practicing medical esthetician knows about the topic of my report at least on a whim, intuitively. When we inject a filler, its projection is always imprinted somewhere in the deep layers of the skin. If there is a projection, additional deformation of the dermis appears. Plastic surgeons are aware of this phenomenon, but some of them do not know about all the intricacies of the mechanisms of its occurrence — and then I appear with a report in which there are answers to all their "why?", "how?" and "from where?". That's all! In addition, I explain why the method of introducing a filler into two skin layers at once — deep and upper — turns out to be the most effective.

What secrets do you need for practitioners to discover while making a presentation?

B. R.-S.: Basically, I talked about the secrets of projection and distribution of fillers in the skin — in fact, exactly in tune with the topic of my report. I shared my knowledge about back projection. Let's go back to visually accessible examples, shall we? Consider a backpack with oranges. When its volume is filled to the top with fruits, there is a projection — simple, not surprising to anyone, bulge.

Now let's start getting oranges out of it — slowly, one by one, what happens at this moment with the backpack? That's right, it's starting to sag! Approximately the same with age, the skin loses volume — and becomes flabby, hollows and wrinkles appear. Conditional point A, which used to be above the left temporal zone, falls lower and lower to the line of the eyebrows.

When the doctor injects a quality filler, he returns to our skin "backpack" a few ripe oranges — and the projection returns to its original, younger position.

By injecting the filler into the cheek area, we also lift the skin of the lower zones upward, smoothing out the problematic nasolabial fold. The main secret of youth — restore the skin to its former volume, that's all! If the doctor does not know how to correctly inject the same hyaluronic acid or overdoses it, the result may be the opposite of what is expected.

Recall that  Teosyal RHA — a unique elastic filler based on hyaluronic acid, presented in Ukraine by «Academy of Scientific Beauty».

Wolfgang Redka-Svoboda: even the best product in an untrained hand can turn into a nightmare and is worth literally nothing

Plastic surgery is a kind of a sport &mdash ; and could be easily compared with racing, where the question of winning depend on several factors: skills and experience of driver-surgeon, the quality of fuel you are currently using, which in aesthetic medicine becomes a filler, and your patient's expectations on the finish — the final result. To make the doctor professionally satisfied, and the bring the personal self-enjoyment to his patient, you need to combine the three most essential things: the surgeon's professionality, high quality products you inject and reasonable expectations of your client. How to reach the harmony on these basic needs, specially for estet-portal.com told plastic surgeon, Associate professor of the Medical Paracelsus University in Salzburg, Head of the anatomucal course Expert Day Anatomy, a member of the German Society of Surgeons, the Society of Mesotherapy, the European Academy of Aethetics Surgery and the International Society of Aethetic Medicine, the author of scientific articles on volume face modeling Wolfgang Redka-Svododa (Germany).

In your opinion, how important is it to choose the right drug for a face correction?

As everything, you know, when we are treating patients, and it doesn't matter if it's aethetic or real medicine, — we always need to choose high quality products! But, on the other hand, we also have to train the doctors. It is very important to use a good product, but it is also important that your doctor knows how to use it.

So, it is a multifactorial thing. We have to choose the right patient, we have to find the right indication, we have to choose the right filler, but even the best product in an untrained hand, or with a doctor who doesn't know what he should do with this product , can turn into a nightmare and is worth literally nothing.

Are there your own guidelines or criteria for working with drugs — for example, hyaluronic acids? Which one do you personally prefer? What kind of brands is it a pleasure for you to work with?

Ok, I don't want to make now a marketing promotion. So, I think that big brands in general are all good products. All these brands, which are 10, 15 and even 20 years on the market can't be actually bad. Sometimes we have these waves, economic downturns, when somebody, who is paid by big brand's rivals, is telling you that the brand is bad, causes a lot of post-surgical complications and so on.

Personally I think that is over 10, 15 or 20 years on the market, it cannot be the worst product ever.

So, I would choose a good product, I would never choose a very cheap product: any cheap product means its cheap production, and cheap production is only possible, if the producer breaks the rules. Basically, you need to have a good product, but I don't want to make my speech a promotion for one or the other company.

What do you think is principally different in Teosyal drugs in comparison to other offers on the market?

Teosyal brand ranges its products, making them different — from essential to the best ones. Teosyal has its so called 'classic range'. These are classic hyaluronic acids, fillers with more or less the same production procedure, that other products have on the current market. But since a few years we have a new range, which is called RHA — Resilient Hyaluronic Acid. The principle in this RHA is that we are now working with the preserved natural hyaluronic acid. Natural hyaluronic acid has long chemical chains, so we need less bridges to connect and reticulate them. If you have less bridges, if you have too long pieces and you connect them with one bridge, this product is much more elastic. And vice versa — if you have very short pieces and you have to connect them, and have one bridge between every pair of short pieces, you get a very non-flexible, too much rigid product. This is the main difference: we preserve natural hyaluronic acid, so it is adapted to the movement of your patient's face. Our product is resilient, and resilience means, that you work with material, which can be deformed by a force from outside, but when you release the force from outside, it comes back to its origins — turns back to the form it naturally has. It's like a bamboo, it's like a rubber band – it is resilient. So, now you can see RHA's differences to all the other similar products at the current moment on the market.

Are there any products today, which can become decent counterparts for Teosyal?

It is a question, where from marketing side I should say something like that: "None! There's nobody, who can start a business struggle with us, we are the only ones who have this innovative resilient technique!», — but to tell the truth, I would not be honest, saying these words. Yes, RHA is different, it's like a Tesla in comparison to Volkswagen 1973. You can go from Tbilisi to Kyiv both ways, they are two cars, but their technologies are completely different and, therefore, are non even comparable! You can have problems with Tesla as well as with any models of Volkswagen. It also depends on the driver.

What kind of recomendations would you like to give to surgeons, who are not into using Teosyal fillers yet and tend to prefer other simpler products?

Not to overdose volumizing products into the superficial layer. The modern way of volumizing is to do it in double-layer technique. First of all you inject the deep fat compartments to give projection. Then you inject the superficial layers to give extension to the skin, so you give support under the skin. And, if the product is too rigid, or of the product comes from a classic range, you should not overdose the filler in the superficial layer. To make a logical conclusion: working with a product from a classic range, you should always try to put the main support in the deep layers and a little bit of it — superficially.

Your speech was on the secrets of projection and dispensation of fillers while hyaluronic acid face correction. In your opinion, are other surgeons aware of the topic you were presenting?

I think, they know about this question instinctively. If you put a filler on the face, you obviously give it the projection somewhere in the deep part. And when you give projection, you also give extension on the skin. So, I think all the doctors know about it, but some of them don't know why it works like this. And here comes my part: I want to tell them why fillers injected in two different layers are working.

Which secrets did you reveal to the doctors during your announcement?

The secrets of projection, what is changing if you give the face back projection. The same happens when you have a backpack full of oranges. This backpack has a projection, but if you take out the oranges, one by one, step by step, — you lose the volume, what happens with the backpack? It is dropping down! The projection that first was up, goes down. And with a filler we give back some oranges, so we can give back the projection. In a nutshell, this is the only secret I revealed of projection.

If you give back projection in a midface, you have a stretching and extension of the skin in the nasolabial folds, or the sagging in the jawline. It all gets better with filler. We give back the volume — that's the secret!
