–  What reasons most often make the patient decide to correct the shape of the nose with injections?

–  Often, patients do not even realize that they can apply to us for an injectable rhinoplasty modeling procedure, because they believe that only plastic surgeons can help solve their problems. But since surgical rhinoplasty is anesthesia, it is a serious intervention, it is a long period of rehabilitation, and, moreover, it is quite a costly event, many of these factors stop the patient from making a decision. They simply do not know that we can do this with a small number of injections, and the procedure will significantly improve their quality of life. Of course, we try to disseminate information about the possibilities of injectable rhinoplasty as widely as possible.

The reasons why the shape of the patient's nose may require correction are divided into two large groups. One of them – these are birth defects:  for example, a pronounced hump of the nose or a very pronounced retraction, or  the same hook-shaped nose. Here we can help the patient by adding volume in certain areas, smoothing out some irregularities with injectable preparations.  If, say, a patient has a hook-shaped nose – the introduction of the drug into the area of ​​the base of the bridge of the nose will add volume in the area of ​​the columella and bring the nasolabial angle to a level of more than 90 degrees, as a result, the nose can even be made snub-nosed.

The second group of defects requiring correction – these are acquired curvatures of the shape of the nose, which occur, for example, as a result of injuries or complex operations on the face. In this case, we correct or camouflage irregularities.

A special group of patients – teenagers who do not like their nose shape, which is why it is likely to develop severe psychological complexes. & nbsp; In this case (with the written consent of their parents) we can also offer correction of the shape of the nose  with the help of fillers.

–  Does it happen that the patient asks to correct the nose a little, and at the same time to correct other areas on the face? Is such complex modeling possible?

–  Usually the opposite happens. The patient plans to correct the cheekbones or nasolabial folds, and the surgeon during the procedure sees that for the symmetry of the face and the overall harmony of the appearance, it would be nice to introduce a little drug into the nose area. If the patient agrees  - we carry out a complex correction, it is absolutely justified  and possible, especially since a very small amount of the drug is usually required to improve the shape of the nose.

–  What technique of drug administration do you consider optimal? How to choose the right filler for the area to be corrected?

–  During the rhinoplasty procedure, both needle and cannula techniques are used. Depending on the zone of introduction and the task that sets itself & nbsp; doctor, both are used. But we understand that the cannula technique is less traumatic, the patient after the procedure is absolutely socially adaptable, can attend various public events, and no one will notice that he has recently had a nose shape correction.

If we correct a deep zone in the area of ​​the bridge of the nose, then only a needle is used, and the likelihood of bleeding, minor bruising increases. Social adaptability in this case is somewhat different, since & nbsp; bruises may remain for some time after the procedure.

The principle of choosing the drug depends on the area of ​​administration. If we inject the drug deeply, or if we need to form some additional form, for example, to sharpen the tip of the nose or form a thin strip of the line of the back of the nose - we use denser drugs that hold the shape, and for a long time and stably. In my opinion, the effectiveness of such drugs is much higher, and their use in rhinoplasty is more appropriate. But if we need to fill a large volume of space, and in this case we do not need to keep the shape, we can use, for example, in the area of ​​the cartilage of the nose, medium-density preparations. They are more consistent in density with soft tissues, they will not contour. By filling in the voids, the irregularities will be smoothed out, and this will give a good aesthetic effect.

–  In your opinion, should you discuss the specifics of the procedure with the patient?

–  This must be done without fail. The conversation with the patient takes even more time than the technical execution of the procedure. The fact is that there are two extremes: either the patient's expectations are too high, or, conversely, the patient's lack of information about what we can do about changing the shape of his nose.

During a detailed conversation with the patient, we collect an anamnesis and find out what the patient wants, for what purpose he wants to correct the shape of the nose and how he imagines the final  result.

–  Does the rhinoplasty procedure have any age or health restrictions?

–  Since the cause of aesthetic dissatisfaction with the nose can be not only acquired, but also & nbsp; congenital defects, the age category for injectable rhinoplasty is different – both youth and older people. There is only one condition – that for patients under 18 years of age, the procedure is allowed with the written consent of the parents. In the absence of medical contraindications, the drug is perceived by all patients absolutely adequately, without complications at any age.

–  How long will the effect of the correction last and what are the possible complications?

–  We hope that the correction procedure will be effective for at least a year. It all depends on the activity of the metabolic processes of a particular patient.  We know that mimic activity in the nose area is minimal, so the mechanical destruction of the drug & nbsp; not expected in this case.

If we use the needle technique, then there are risks of some complications in the form of  bleeding or ischemia in the injection area. If bleeding occurs – we carry out activities as in rehabilitation after a normal bruise. If these are ischemic complications, then the tactics of managing such patients is more close, more active, and such a patient must be observed every day in order for the dynamics of rehabilitation to be effective. The patient should know that if he is worried about some discomfort in the area of ​​​​filler injection already on the first day after or even in the first hours after the procedure, he should not do anything on his own, and he should urgently contact the doctor. Timely measures will prevent possible complications.

–  Are there any lifestyle recommendations for the patient that would help prolong the effect of the drug?

–  I would rephrase a little differently: there are some factors that can reduce the duration of the drug. These are universal contraindications for any contouring using preparations based on hyaluronic acid: the injection zone must not be massaged, it must not be heated, it must not be exposed to ultrasound. Everything that improves local blood circulation - accelerates the destruction of the drug. Baths and saunas are not recommended, but visiting the pool is not prohibited.

After contouring, it is recommended to sleep on your back for 1-2 weeks, so that the drug integrates into the surrounding tissues, fixes itself and then does not move. It is recommended to come to the doctor for a follow-up examination a few days after the procedure and see if everything is in order. Wearing any special dressings or plasters, in my opinion, is inappropriate.
