Sometimes it seems that over time, plastic surgery becomes easier and takes less and less time. However, in reality it is not. The more experts delve into anatomy, the more diligently they look for ways to minimize side effects — the longer and more complex the operations become.
Rhinoplasty — this is just one of those procedures that in recent years has become much more serious, scrupulous and indeed more complex, requiring special dedication.
In order to successfully perform rhinoplasty, surgeons study for many years and use only the most modern methods of preparation for surgery.
The role of imaging programs in the planning of aesthetic procedures was told to readers of estet-portal.com by the Italian plastic surgeon, a specialist in rhinoplasty - Enrico Robotti.
What are the main trends in modern rhinoplasty?
ER: When it comes to rhinoplasty, a lot has changed in regards to this procedure, however, achieving the best result — still remains the main goal of the plastic surgeon. It is impossible to perform a rhinoplasty in half an hour and claim that speed equals quality.
The demands of our patients are growing, but we are not far behind - and we can offer them many new innovative solutions.
The main problem of modern surgery is marketing, which does not always play into the hands of a specialist. Advertising can cause misconceptions and unrealistic expectations regarding the effectiveness of a particular technique, product, etc. And in this case, not something innovative, but simply advertised will be introduced into practice.

I also want to note that for a surgeon who wants to do rhinoplasty, training — this is the first need. It takes a lot of time, and it takes years before a specialist becomes a master of his craft.
How often do you have revision rhinoplasty?
ER: I see revision rhinoplasty quite often. At this stage, I only do rhinoplasty, and perform revision rhinoplasty in about 50% of cases. This type of surgery is much more difficult and requires more thorough preparation from the surgeon.
Three-point non-surgical rhinoplasty: technique and results
The main reason for the need for revision rhinoplasty – this is a poorly executed primary operation that was carried out without sufficient planning and preparation.
Detailed analysis and meticulous planning is the key to an excellent result in rhinoplasty.
What is your opinion on computer modeling?
ER: I'm a true fan of computer imaging, and without it, I don't even think about doing surgery. I always show the results of the simulation to the patient, and I do this for a better understanding, but not for marketing purposes. 3D face modeling: prevention of possible complications.
3D modeling allows the surgeon and patient to better understand each other.
In my daily practice, I meet people from different parts of the country and the world, and we find a common language, because we have a common understanding of the aesthetics of the nose and the face in general.

And in this case, 3D modeling gives us the opportunity to make a final decision, and get an excellent result. Therefore, I want to once again emphasize the importance of computer imaging in the practice of a plastic surgeon.

Enrico Robotti: The secret of successful rhinoplasty - planning and preparation
Sometimes it seems that over time, plastic surgery is becoming easier, and takes less time. However, in reality, it is not so. The more specialists go deep into the anatomy, the harder they look for ways to minimize side effects - the longer and more complex the operations become. Rhinoplasty is one of those procedures, the approach to which in recent years has become much more serious and scrupulous and actually more difficult, requiring a lot of dedication. This is essentially since the patient’s expectations are higher and results are better. Rhinoplasty is learned over many years and new methods of preparation for surgery, with much more finesse in analysis and use of specific tools such as imaging programs, appear. About the main secrets of the modern rhinoplasty to the readers of estet-portal.
What are the main trends of modern rhinoplasty?
Over the last years there have been a lot of true and fascinating changes, which are continuously evolving, in the field of the rhinoplasty. To achieve the best natural and functional result, it is impossible to do a rhinoplasty in half an hour, as it sometimes happened before, and it is misleading to say that speed equals the quality. The requests of our patients are more and more sophisticated, and nowadays such requests are technically feasible with experience and finesse surgery, often quite complex. This is even more so in the field of secondary rhinoplasty, for instance when using rib grafts.
The main problem of modern surgery is marketing. Advertising can cause confusion and unrealistic expectations about the effectiveness of a particular technique, product, etc. And in this case, something not really innovative, but simply heavily advertised and “branded” will be introduced into practice. This is a dangerous ground for the patient, since this marketing hype will be misleading and oftentimes dishonest
Also, I want to note that for a surgeon who wants to engage in rhinoplasty, adequate training is the first necessity. It takes a lot of time, and years pass before a specialist becomes a real master.
How often do you perform secondary rhinoplasty?
My current practice is rhinoplasty only and about 50% of my cases are revision rhinoplasty meaning secondary or tertiary etc.Revision rhinoplasty is even more difficult than primary cases and oftentimes techniques become even more complex including rib and fascia grafts. Once again, the original problem has often been of the first procedure done too quickly without paying attention to detail and to adequate planning. Proper analysis and good planning are the keys to a proper result in rhinoplasty.
What is your opinion about imaging and 3D modeling?
I'm a true fan of computer modeling, and without it, I do not even think of doing an operation. I always show the results of the computer imaging to the patient, and I do it for proper communication from patient to surgeon and from surgeon to patient, not for marketing purposes. 3D modeling has obviously further advantages, so that a person can better understand tridimensionale what can be done and If this will be in line with his or her expectations. In my daily practice, I come across people from different parts of the country and the world, and we find a common language because we have a common understanding the aesthetics of the nose and the face as a whole. And in this case, imaging and especially 3D modeling help significantly. Computer imaging, when honestly done according to the true capacity of the surgeon and shared with the patient, is for sure an essential part of modern rhinoplasty.
