Today, aesthetic gynecology is a very popular and sought-after area in medicine. Modern women want to be perfect in everything, and women's health greatly affects the quality of their lives. With the help of innovative methods, it is possible not only to eliminate pathological changes in the intimate zone, but also to return lost functions and to a large extent improve all areas of the patient's life. What are the most effective methods in aesthetic gynecology, and what opportunities does their use open up for doctors – Specially for estet-portal.com, Doctor of Medical Sciences, Professor, Head of the Department of Aesthetic Gynecology and Rehabilitation of the National Medical Research Center for Obstetrics, Gynecology and Perinatology named after M. IN AND. Kulakova Inna Apolikhina.
C what problems do patients most often come to you with?
It should be emphasized that aesthetic gynecology – this is a fairly new sub-specialty, and by the way it develops not only in Russia, but throughout the world, we can say that these problems are quite diverse. Most often, women come to us with various pelvic floor dysfunctions, and these are prolapse and prolapse of the pelvic organs, various types of urinary incontinence, atrophic processes that, unfortunately, accompany a woman with age, chronic pelvic pain, and sexual dysfunction. Often these problems are solved with the help of surgical methods of treatment, but following international standards, we always start with conservative methods. Indeed, in our arsenal there are minimally invasive technologies aimed at training the muscles of the pelvic floor, as well as increasing the elasticity and strength of muscle contractions,
What are the most common mistakes made by doctors who are not gynecologists but perform aesthetic gynecology procedures? It should be emphasized that many technologies used in aesthetic gynecology are taken from dermatocosmetology (this is primarily lasers: carbon dioxide and erbium, injection methods, etc.). But, unfortunately, doctors of other specialties often underestimate the functional aspects, so I always say that aesthetic gynecology should, above all, be functional. We not only need to change the form, satisfy the woman psychologically, but most importantly, we must preserve or even increase the function lost by the woman. And therefore, doctors make mistakes in terms of the technical use of certain methods, they do not choose all the right parameters or technologies and their combination.
Every woman – this is individuality, and we select certain methods that are shown specifically to her.
It is advisable not to blindly use everything new that appears, but only methods that have been optimally tested by time and research. Recently, women come to us after improper use of photodynamic therapy methods on the vulva, vagina and cervix with complications in the form of burns. To date, there are no clear clinical protocols for the use of light doses and photosensitizers, and we have to restore and rehabilitate them with tremendous efforts.
Before choosing a technique, a gynecologist must first master it, remember the anatomy and biophysical processes.
In aesthetic gynecology today, correction with mesothreads is used. How do you feel about this method?
All methods are good, including modern, biodegradable threads, which are now widely used. But it all depends on what this thread is made of and how competently it is offered to a woman, because if the threads are used for the purpose of perineal lifting in women who have a second degree of prolapse according to the POP-Q classification (Pelvic Organ Prolapse Quantification), then no results are to be expected from these threads. Therefore, the niche of possibilities for mesothreads – these are the initial stages of prolapse of the pelvic organs, sexual dysfunction, when there are still no functional disorders. With their help, we improve a woman's sexual well-being – this is a very important component.
Do you use PRP therapy in your practice?
Possibilities of platelet-rich plasma – this is a unique, amazing method, but, again, there must be clear criteria for selecting women for this procedure and a well-coordinated technology for its preparation. In this regard, I want to say that gynecologists only define these niches. In our center "National Medical Research Center for Obstetrics, Gynecology and Perinatology named after V.I. Kulakov", which is the leading institution in the country, for the first time in Russia we have introduced PRP therapy for various gynecological diseases, and have received very good results in the treatment of stress urinary incontinence in women, as well as a very complex disease – lichen sclerosus. But with vulvovaginal atrophy or, as it is now commonly called genitourinary syndrome, this method did not show us a sufficiently high degree of effectiveness, so now we are looking for what methods we can supplement PRP therapy in order to achieve the maximum effect in its correction. Without a doubt, the best method that complements platelet-rich plasma is laser technologies, namely the solid-state erbium laser and the carbon dioxide CO2 laser.
In what cases can aesthetic gynecological methods be contraindicated for a patient?
First of all, it is the presence of oncological diseases, as well as an active inflammatory process in the body. Determining the right niche for a toga or other method is very important, and in this regard, at this congress we have demonstrated the top 10 methods of aesthetic gynecology.
The top 10 aesthetic gynecology methods are laser technology, platelet rich plasma, hyaluronic acid bulking agents, lipofilling, various energies such as RF technology, high frequency technology, ultrasonic carbon dioxide injection, thread lifting and more .I am deeply convinced that a reasonable tandem of a gynecologist and dermatocosmetologist will help women maintain youth and beauty.
