Broca's pseudopelade or cicatricial atrophic alopecia is an acquired form of alopecia, which is manifested by focal hair loss, accompanied by scarring and skin atrophy.

Epidemiology of Broca's pseudopelade

Broca's pseudopalade occurs predominantly at the age of 35-40 years. The disease is typical for both men and women.

Clinic

The disease begins with the appearance of small cicatricial-atrophic asymmetrically located foci of alopecia, which increase in size, acquire irregular jagged outlines, merge with each other. At the beginning of the disease, moderate perifollicular hyperemia is observed in the lesions, then skin atrophy without signs of inflammation and peeling, the follicular apparatus is destroyed. Sometimes, in the center of the lesion, there are lonely visually unchanged long hairs. Patients with Broca's pseudopelade have no subjective sensations.

Current and forecast

The process is slowly but steadily progressing. Over time, hair loss stops, complete afforestation does not happen. However, with regard to hair regrowth in lesions, the prognosis is poor.

Pathology

Histological examination in patients with Broca's pseudopelade reveals a sharp atrophy of the epidermis, in the dermis - the sebaceous-hair follicle is destroyed, perivascular and perifollicular lymphohistiocytic infiltrates, located more superficially than in patients with alopecia areata, sometimes - follicular hyperkeratosis.

Diagnostics

Based on a characteristic clinical presentation and can be verified by histological examination.

Treatment of scarring alopecia

Scarring alopecia is irreversible and in most cases treatment is not effective. Patients are prescribed sedatives, lidase, nicotinic acid preparations that improve microcirculation, biogenic stimulants, vitamins, zinc, copper, and iron preparations. Ruthenic acid preparations are used in the treatment of patients with Broca's pseudopelade. It is also advisable to use physiotherapeutic methods - indirect diameter or inductothermy of the cervical sympathetic nodes and the transverse zone, cryomassage with liquid nitrogen.
