Poly-L-Lactic Acid for Tissue Volume Loss
Temporary fillers, permanent fillers, implants or fat grafting help us with the problem of volume loss. As for skin changes, there are many ways to get rid of this problem. Interestingly, tissue stimulants can help restore volume and firm the skin. Poly-L-lactic acid (Sculptra filler) has been used for many years and is also known as New-Fill. Other examples of fillers used in cosmetology are polycaprolactone Ellanse and Radiesse.
When using poly-L-lactic acid (or Sculptra), it may take 10-12 months to get final results, but the first changes can be seen after 8 months.
Important points when injecting Sculptra fillers etc.
With fillers, the main thing to remember is that if you overdo it with volume, the results will be unnatural. Therefore, care must be taken with the dosage – you can add a filler at any time.
All three parts of the face – upper, middle and lower – should be equal. It is also necessary to pay attention to the jaw area, which is difficult to work with due to the movement of fat. In addition, elastosis and loss of elasticity of soft tissues necessitate the use of surgical sutures. This area cannot be corrected with injections, however, by playing with shadows and contours, it can be corrected visually.
If the mental muscle looks like an island, surrounded on all sides by depressions, it seems to be separated from the entire face. To correct this phenomenon, you can introduce a filler and align the defect between these two zones.
Ellanse-type biostimulants are not injected into or near the red area of the lips. I always mark about half a centimeter from this area to avoid spreading the product.
What are the risks of vascular damage when using biostimulants?
There is always a risk of vascular damage with filler injections, regardless of the product used. However, Ellanse – the product is quite thick, so, as with Radiesse, it cannot be aspirated. Therefore, if you are not completely confident in your technique, it is better to use – they are safer in this regard.
Always pay attention to blanching of the skin at the injection site and pain of the patient – when hit in a vessel, they will be quite strong.
I always start with the safer injection and explain to the patient how he should feel during the injection. If his feelings are different, I stop the injection. Also, when introducing fillers, it is important not to rush – if you are injecting the drug in the wrong place, it is best to use the lowest possible dose.
How do you use the cannula?
It always frustrates me when someone travels entire centimeters of tissue to get to a place that could be reached with a simple injection through a point located near the target zone.
It is important to decide what exactly you need to do with the cannula. You either select the access point in the direction of travel, or if you want to penetrate several different zones through one point, it is better to do it perpendicularly, since it will be easier to manipulate the needle.
If you are using a very long cannula, you must be aware of the superficial temporal artery, which runs about a centimeter from the tragus. It can often be felt.
Into what layers of the skin can biostimulants be injected?
If I use Ellanase or a similar product, I never dermis it. Biostimulants should not be injected into the dermis layer at all. I run into a lot of problems resulting from incorrect product introduction. Biostimulants need to be injected deeper. In the case of shallow wrinkles, it is better to use hyaluronic acid, which is intended for this. I urge you to use your tools correctly, do not try to introduce heavy product into the surface plane – those. into the dermis. Such products must be injected deeper than the dermis layer.
