What factors lead to enophthalmos

Research in the field of age-related changes in the human face made it possible to establish a relationship between the amount and localization of excess subcutaneous adipose tissue around the eye, the state of the ligamentous apparatus of the eye and the position of the eyeball. It was found that the displacement of adipose tissue and skin anteriorly leads to a displacement of the eyeball downward and backward, leading to the occurrence of enophthalmos – deep placement of the eyeball in the orbit. The main causes of enophthalmos include:

• genetic or age-related displacement of the eyeball;
• shift of adipose tissue around the eye to the anterior parts of the orbit;
• age-related resorption of adipose tissue inside the orbit;
• excessive removal of fatty hernias;
• too voluminous coagulation of adipose tissue inside the orbit.

Technique for performing blepharoplasty on the upper eyelid

The technique for performing blepharoplasty in upper and lower eyelid surgery is significantly different. After the skin incision of the upper eyelid, the previously marked excess skin is removed. In the center of the open zone, the circular muscle of the eye is separated for 4 mm. along the muscle fibers, then dissect the underlying orbital fascial septum and penetrate into the central pocket. By pressing a finger through the lower eyelid on the eyeball, the intraorbital adipose tissue is displaced anteriorly, and its excess is cut off and coagulated. Then an intradermal suture is applied to the wound. After removing excess skin from the incision to the lower edge of the eyebrow, a distance of at least 15 mm should remain, and from the ciliary edge of the eyelid to the incision – not less than 8 mm.

Blepharoplasty procedure on the lower eyelid

The incision line of the lower eyelid during blepharoplasty in this area is determined under the lacrimal opening. It continues at a distance of 1.5 mm. from the ciliary edge and 1-1.5 cm beyond the outer corner of the eye. The skin of the lower eyelid is dissected, its lower edge is separated from the muscle in accordance with the excess tissue in this area. Muscle fibers are separated by 3-4 mm. at the level of the central fat pocket, then the excess fatty tissue is gradually removed. After that, the wound is sutured with an intradermal suture. Blepharoplasty of the lower eyelid is a fairly simple plastic surgery, while its result significantly changes the appearance of the face. Such an operation can be performed not only with age-related changes, but also if a young patient wants to make his eyes more open and his facial expression more fresh.
