Skin diseases are among those problems with which patients often turn to specialists, since it is quite difficult not to notice changes in the skin. In addition to the visual picture, dermatological diseases are often accompanied by symptoms that are very disturbing to the patient, such as itching, soreness, a feeling of skin tightness, and others. Some of the most common skin diseases today are atopic dermatitis and vitiligo. The President of the Ukrainian Academy of Dermatovenereology, Doctor of Medical Sciences, Professor, Head of the Department of Dermatovenereology of the National Medical Academy of Postgraduate Education named after I.I. P. L. Shupika, chief freelance specialist in the specialty "Dermatovenereology" Department of Health of the KSCA, director of the territorial medical association "DERMATOVENEROLOGY" in Kyiv, President of the All-Ukrainian public organization "Ukrainian Association of Psoriasis" Litus Alexander Ivanovich.
For the treatment of such a common dermatological disease as atopic dermatitis, various medications, immunotherapy, PUVA therapy and other effective methods are used. Are they used in Ukraine?
All these novelties are available in Ukraine today, and specifically one or another method of treatment or, say, patient care should be selected jointly by the doctor with the patient or the parents of the latter. But there are two main approaches. This is prevention, in this case, if we are talking about atopic dermatitis, it should be proper care, regular proper bathing, washing with the right products, it is imperative to use emollients – These are products that compensate for the lack of sebum on the skin. Well, systemic therapy, which is used in severe cases, is also present in Ukraine
What are the most relevant innovations in the treatment of this pathology?
In fact, there is nothing new in the treatment of atopic dermatitis as such. New drugs just appear and, first of all, these are emollients that are optimally suited for the treatment of patients with such a dermatological disease.
The so-called personalized therapy is now developing all over the world, when each patient is selected, depending on his skin type and the severity of his illness, one or another emollient or other drug that can compensate for all this.
In fact, the most important thing, at the moment, in the management of patients with atopic dermatitis is the attention and motivation of the patient to take care of himself, and the doctor only helps the patient in this by selecting the right care methods, and, if this is a difficult case, then treatment methods.
What is new today in the treatment of such a dermatological disease as vitiligo?
Unfortunately, psoriasis and vitiligo are from the category of systemic diseases caused by some kind of genetic disorders. And there is no cardinal treatment that could remove these problems today. Perhaps it will appear in the near future, because a huge amount of research is being carried out in this direction in the world. With regards specifically to vitiligo – in the early stages, the combination of calcineurin antagonists with phototherapy has shown itself very well to date. There are also a huge number of cosmetics, even aesthetic ones, that level this problem. These are all kinds of paints, tattoos and just make-up.
How relevant are tattoos to mask vitiligo. Could such a skin injury provoke further development of the pathology?
The fact is that some kind of penetration or damage does not always lead to the process escalating.
Particularly stigmatizing areas, such as the face, can be such a big irritant and problem for the patient that he goes to get a tattoo, and thus hide his defect.
In this case, of course, only a doctor can determine, through some preliminary manipulations, whether this patient will have a complication after the same tattoo, or not. That is, each patient should have a separate approach.
