Hyaluronic acid fillers are most often used for injectable facial correction. But in patients with deformative-pastotic type of aging, who are prone to edema, the result of HA administration is often unsatisfactory. Moreover, after repeated injections of hyaluronic acid, the face of patients does not change for the better – then the cosmetologist understands that it is necessary to look for an alternative. This also applies to patients in whom, due to the increased activity of the hyluronidase enzyme in the body (HA fillers quickly degrade); as a result, they complain about the short-term effect of the correction.
In such cases, as well as in the presence of lipoatrophy, good and long-term volumization effect without swelling give preparations based on calcium hydroxyapatite. These include the South Korean novelty – Facetem S filler from the official distributor of TOTISPHARMA GROUP, which estet-portal.com will talk about in this article.

Facetem S calcium hydroxyapatite filler: general characteristics
The first and most important thing to consider when choosing a filler, – presence of registration. Facetem S based on CaHA exists and has been successfully used since 2012. In Ukraine, Facetem S filler (Daewoong, South Korea) was registered by TOTISPHARMA GROUP in June 2020.
The composition of the filler includes:
• calcium hydroxyapatite microspheres;
• gel carrier (carboxymethyl cellulose sodium salt, glycerin, water).
SaNA – a substance that does not cause side effects (inflammatory, allergic reactions, the formation of granulomas, does not have a mutagenic effect, does not affect the course of pregnancy and fetal development, if the procedure was performed technically correctly.
Benefits of Facetem S filler:
• stimulates neocollagenesis;
• provides maximum volume and modeling;
• easy to enter;
• controlled biodegradation due to lattice-porous structure;
• has a pronounced viscoelasticity (the mass of the gel carrier is over 700,000 Daltons);
• biocompatible, non-toxic, non-antigenic and non-irritant, does not require preliminary sensitivity tests.
Unique technologies and features of the Facetem S filler
An important feature of Facetem S – the size of calcium hydroxyapatite microspheres. For the production of the filler, microspheres of the safest size – 25–45 microns that do not penetrate the circulatory and lymphatic systems and do not cause pathological fibrosis.
Facetem S's key technology – lattice-porous structure that degrades gradually from the surface to the center, which provides more controlled biodegradation than the structure of other CaHA preparations.
The homogeneity of the Facetem S filler provides extreme ease of administration of the drug. CaHA particles of the same size do not bunch up, resulting in a soft and smooth injection – another advantage when working with this filler.
CaHa, which does not have a lattice-porous structure, melts like a sugar cube, while FAcetem S degrades gradually from the surface to the center and provides a more controlled degradation.
The high viscosity of Facetem S makes it ideal for 3D volumetric facial correction. The filler excellently retains its shape and volume after injection and resists the pressure of surrounding tissues.

One of the main advantages of Facetem S – no swelling after the procedure. Calcium hydroxyapatite is not hydrophilic, therefore it is the drug of choice when working with the deformation-pastose type of aging. Preparations based on hydrophilic hyaluronic acid are completely unacceptable for such a morphotype, especially if we work in the infraorbital region, in the middle third of the face.
How to use Facetem S
Calcium hydroxyapatite preparations were originally administered to patients with lipoatrophy in order to obtain a predictable and long-term result. Fillers based on CaHA – an excellent solution for athletes and patients with increased hyaluronidase activity, in whom the effect after the introduction of HA does not last long. And, of course, Facetem S is ideal for volumizing the face with a deformative-pasty aging morphotype, since it does not cause puffiness.
Indications for use of the Facetem S filler:
• replenishment of lost volumes of soft tissues (cheekbones, cheeks, temples);
• restoration of the oval of the face and chin;
• filling of nasolabial folds and marionette lines;
• collagen stimulation for loose and atonic skin on any part of the body;
• aesthetic correction of the dorsum of the hands (skin tightening).

Recommended injection volume of Facetem S:
• face – 1.5 ml per side;
• neck – 1.5 ml;
• neckline – 1.5 ml;
• inner surface of the forearm – 3.0 ml per arm;
• area above the elbows – 0.8 ml each;
• area above the knees – 0.8 ml each;
• inner thigh – 3.0 ml per side;
• the back surface of the brush – 1.5 ml each (for very thin skin – 0.8 ml).
In order to volumize the face, is injected supraperiostally with a needle or cannula.
Preparation of the preparation: 0.3 ml of anesthetic is added to the initial volume (0.8 or 1.5 ml).

Facetem S do not use:
• in the area between the eyebrows (to avoid complications);
• on the lips, where the preparation will show through thin skin;
• in the periorbital region (do not go beyond the bony edge of the orbit);
• on the bridge of the nose;
• with a burdened cosmetic history (if the patient does not know what drugs he was previously injected into the target area).
Undesirable effects that may occur after the introduction of the Facetem S filler are standard, as for all injection procedures. If the patient is correctly selected and the procedure is carried out, they do not occur.
It is important to remember that there is no antidote for calcium hydroxyapatite fillers!
Possible adverse events include:
1. Immediate: erythema, edema, ecchymosis, pain (usually resolves in 5-7 days).
2. Delayed: nodules, irregularities, contouring
Possible complications:
1. Tissue compression, ischemia, infection, necrosis.
To avoid complications of injection correction, it is important to avoid too superficial injection of the drug, injection of an excessive amount of filler, carefully collect the patient's history, follow the rules of asepsis and antisepsis, know anatomy well in order to prevent injection of the drug into the vessel.
Using the Facetem S filler from the official distributor of TOTISPHARMA GROUP correctly and as intended, you get a predictable and long-lasting result (12-15 months), no swelling , active and long-term stimulation of collagen and elastin synthesis (skin tightening by 40%) and reliable protection from environmental influences. This drug will perfectly complement the arsenal of every modern cosmetologist.
