Eczema is a common chronic skin condition. Probiotics have been suggested as an effective treatment for eczema; their use is expanding as numerous clinical trials are conducted.

The aim of the Cochrane Analysis was to evaluate the impact of probiotics on the treatment of patients of all ages with eczema.

Analysis of studies on the effectiveness of the treatment of eczema

Authors included 39 randomized controlled trials involving 2599 people. Participants of either gender, from the first year of life to 55 years of age (only 6 studies evaluated adults) who had mild or severe eczema were analyzed.

The trials were conducted in primary and secondary care settings, mainly in Europe and Asia. The duration of treatment varied from 4 weeks to 6 months.

The researchers used various doses and concentrations of probiotics. The probiotics used were Lactobacillus and Bifidobacteria species taken alone or in combination with other probiotics, given with or without prebiotics.

Probiotics appear to have little or no effect on eczema symptoms in participants (13 trials; 754 participants): symptom severity on a scale of 0 to 20 was 0.44 points lower after probiotic treatment (moderate-quality evidence). Further research with similar probiotic strains may be futile and will not demonstrate an alternative outcome.

The authors did not find any evidence that probiotics affect the quality of life of patients with eczema (six studies; 552 participants; low-quality evidence).

Probiotics may slightly reduce investigators' estimates of eczema severity (24 studies; 1596 participants). On the Atopic Dermatitis Overall Severity Rating Scale (SCORAD), patient or parent scores combining eczema severity based on eczema symptoms (itching and sleep loss) were 3.91 points lower after probiotic treatment than after no probiotic treatment (evidence Low quality). The minimum clinically significant difference for SCORAD was 8.7 points.

The authors found no evidence that probiotics affect the risk of side effects during active treatment (7 studies; 402 participants; low-quality evidence).

A modern approach to the use of probiotics for the treatment of eczema

Evidence suggests that, compared to the absence of probiotics, currently available probiotic strains are likely to have little or no effect on improving patients' symptoms of eczema.

Probiotics may have little or no effect on the quality of life of people with eczema or on the investigator's assessment of eczema severity (combined with the participant's assessment of eczema pruritus and sleep loss symptoms).

Thus, the use of probiotics for the treatment of eczema is not currently proven.
