Нелли Конников о методах эффективной профилактики узелковых уплотнений после инъекций

Complications after the introduction of fillers – this is a nightmare for every injectable cosmetologist.

After the procedure, even the most talented and experienced doctor worries about the condition of his patient. Human skin is unpredictable, and if adverse reactions do not appear immediately, this does not mean that they will not occur later.

Nodular seals after the introduction of fillers – this is a fairly common complication, which at first glance may seem harmless, however, sometimes it can be a serious danger.

About why these granulomas occur and how to prevent their occurrence for estet-portal.com was told by professor of dermatology at the Boston University School of Medicine, USA, chief dermatologist and head of teledermatology at the Boston Veterans Hospital, member of the American Society of Dermatologists (AAD) and Society of Dermatological Surgery (ASDS), dermatologist Nellie Konnikov (Professor Nellie Konnikov).

Why do nodules most often occur after injections?

N.K.: “Lumps and bumps”, nodular seals or granulomas after filler injections is an important and urgent problem. Moreover, it concerns not only novice doctors, but also specialists with many years of experience. These nodules can form for many reasons.

To understand which one triggered the occurrence of this granuloma, one should understand the classification of seals. They are early and late, inflammatory and non-inflammatory in nature, and granulomas may also differ with the introduction of hyaluronic acid or calcium hydroxyapatite.

All these nuances need to be clarified with the patient, which is why it is so important to carefully collect an anamnesis during the consultation.

A doctor cannot simply treat nodules after injections - he must first treat the patient.

To prevent the occurrence of nodular seals after the introduction of fillers, we need to find out whether the patient has previously had an injection correction with fillers, what drugs were administered and in what volume, how long ago the procedure was performed.

It is also important to clarify the presence of infectious and systemic diseases of the oral mucosa in history. Thus, for us, those factors that later may or could lead to undesirable or unexpected complications after the introduction of fillers matter.

How to prevent the occurrence of post-injection nodules?

N.K.: To prevent complications, the doctor must know exactly what he is working with, he must trust the drug. Fillers must be legal, for example, in the US they must be approved by the FDA.

Don't try to save money and buy something on the black market. This is the first condition - even the law - that the practice must be legal.  Secondly, whatever you do - you must develop, improving your skills, and accumulating knowledge. Young beginners should learn from the experts.

The next thing that is also very important is to read literature, including English-language literature, where plastic surgeons, dermatologists, the best injectors in the world share their experience. It's always better to learn from someone else's mistakes than your own.

And the last – practice little by little, very carefully, mindful of your limits. And, remember, never take the time to talk to the patient, and do not begin work until you have obtained informed consent.

What is the most valuable piece of advice you would give to your patient?

N.K.: My most valuable advice to patients, dermatologists, and cosmetologists – Always be aware of your limits. Because sometimes patients rely on themselves, Google, Youtube, their phone, and the internet. They receive false information, after which they dictate their wishes to the doctor.

The patient must trust his doctor, because a good relationship between them – this is already a guarantee of successful treatment.

The doctor must also understand the limits of his abilities. For example, a specialist who does not have laser equipment and understands that with the help of local procedures he has already done everything possible for this patient – should refer the person to another dermatologist or plastic surgeon or any other colleague who has a laser office.

It is only by working as a team that we can succeed and deal with any unwanted complications and side effects.
