Незримые нити: утонченная техника косметического шва

The healing of any wound goes through four stages: hemostasis, inflammation, proliferation and maturation. As a result of the healing of especially deep wounds, a scar is most often formed. Its severity is influenced by the genetic characteristics of the patient, external influences and the nature of the wound. But on open areas of the body, and especially on the face, the appearance of scars is unacceptable. In this regard, surgeons, when performing manipulations in these areas, use cosmetic sutures. The peculiarity of this type of suture is the use of absorbable suture material, as well as a specific suturing technique. Technical features of the cosmetic suture

The main task of a cosmetic suture is to achieve the maximum aesthetic effect. In this regard, the thinnest threads made of biodegradable materials, as well as atraumatic needles, are used for this type of suture.

The thread when applying a cosmetic suture should pass intradermally for better adaptation of the wound edge and for less disturbance of microcirculation.

The cosmetic suture is applied according to the lines of natural skin tension, making the end result look like a wrinkle or crease on the skin.

The main types of cosmetic sutures in surgery

In modern surgery, several types of cosmetic sutures are used, among which the most commonly used are:
•    continuous intradermal suture, which helps to preserve microcirculation as much as possible;
•    vertical mattress seam — considered cosmetic, as the punctures are made as close to the edge of the wound as possible, ensuring minimal trauma to the skin.

Extensive operations with a wide wound require reliable fixation, which a cosmetic suture cannot provide. In this case, preference is given to a more durable surgical suture, and subsequently scar plastic surgery is performed.

Suture used for cosmetic suture

The application of a cosmetic suture requires the use of a special suture material. Atraumatic needles and absorbable sutures are preferred.

Biodegradable suture materials are of organic origin (catgut), and synthetic (MedPHA - threads made on the basis of polyhydroxyacetic acid).

Synthetic threads are usually coated with absorbable polymer film to reduce skin trauma. The main advantage of synthetic suture material— this is the high reliability of the surgical knot and the strength of the suture.

Cosmetic suture care in the postoperative period

The care of a cosmetic suture in the postoperative period is no different from that of a conventional surgical suture. An important condition for successful healing is compliance with the rules of asepsis and antisepsis, as well as respect for the wound. The biodegradable suture used in this type of suture is self-absorbable, reducing the risk of infection when the suture is removed. Also, anti-inflammatory ointments may be recommended in the early stages of healing, and in the later period, agents that reduce scarring.
