Nuri Selik – a plastic surgeon from Turkey, Vice President of the Turkish Association of Aesthetic and Plastic Surgeons, professor of the International Association of Aesthetic Plastic Surgeons ISAPS, specializing in surgical facial rejuvenation, rhinoplasty and blepharoplasty, attended the ISAPS congress, which was held in Kyiv on May 19 and 20. Professor Selik shared with estet-portal.com valuable information about the most common complications of blepharoplasty of the upper and lower eyelids, as well as spoke about the most effective methods for preventing them.

- What are the most common complications after blepharoplasty?

The most common complication of upper eyelid blepharoplasty is excessive resection of adipose tissue and skin. As a result, in the early postoperative period, the patient cannot close his eyes. With regard to blepharoplasty of the lower eyelid, excessive excision of adipose and muscle tissue also occurs quite often, but the most common complication of this operation is the presence of open areas of the sclera in the early postoperative period. This complication is due to the weakening of the lateral ligament of the eyelid. Therefore, the surgeon, when performing lower blepharoplasty, must take into account the structural features of the patient's face and take appropriate measures in order to prevent this problem.

- What causes the occurrence of complications of blepharoplasty in the first place?

Unfortunately, the main problem leading to complications of blepharoplasty is textbooks on plastic surgery and methods of teaching surgeons to perform upper and lower blepharoplasty. If we analyze the postoperative results and emerging complications, we can see that the main cause of problems after blepharoplasty is precisely the excessive tissue excision. The second problem is the neglect of procedures to strengthen the weakened lateral ligament of the eyelid. Therefore, it is necessary to teach surgeons the correct technique for performing upper and lower blepharoplasty, starting with the need for maximum tissue preservation, especially fat and muscle. It is also essential that every surgeon understands

 - What methods of blepharoplasty do you prefer?

I don't have a particular surgical technique that I use most often in practice. However, I believe that the most important points are the preoperative assessment of the patient and the development of a clear plan of action during the operation. I try to remove as little fat as possible. In addition, I completely preserve the integrity of the orbicular muscle of the eye. During lower blepharoplasty, I fix the lateral eyelid ligament or perform a procedure to strengthen it. This ensures the best results and minimizes the risk of complications after lower blepharoplasty. In my opinion, these tasks are the most important.

- Why did you choose blepharoplasty as the topic of your report?

Dr. Denischuk asked me to talk about the complications of lower and upper eyelid blepharoplasty, as he was already present at my talk on this topic. I very often understand the issue of dealing with complications, especially in the early postoperative period, because due to such problems the patient enters a difficult period of social isolation, and my main goal is to return these patients to a normal life in the shortest possible time. Therefore, I operate on them much earlier than most plastic surgeons think is necessary.

- What is the average age of your patients for upper and lower blepharoplasty?

Most of my patients are between 40 and 45 years old. It is at this time that people, especially those living in the Mediterranean region, begin to show visible age-related changes in the area around the eyes. And for a facelift, patients come to me at the age of about 50.

- What is the role of cosmetology in postoperative skin care?

It is not uncommon for patients to have problems with oily skin. And in this case, cosmetology helps to solve the problem, as it provides proper therapy for oily skin before surgery. In addition, cosmetologists teach patients how to properly care for the skin in the postoperative period. And this is very important, since oily skin is more vulnerable to infections and prone to cyst formation after surgery. Therefore, the patient must be aware of their skin problems before surgery. The same applies to very dry skin, the owners of which also need to be taught proper skin care after surgery. In addition, patients may have problems with skin pigmentation, so it is necessary to correctly select drugs that can eliminate or prevent pigmentation disorders in such patients.
