Age-related changes primarily affect the condition of human skin, and also manifest themselves in changes in the volume of soft tissues of the face and body.

Wrinkles and folds appear on the face, the skin in the neck area sags, age spots appear in the decollete area, atrophic tissue changes progress, which leads to their thinning and contouring of deeper bone structures.

V.P.: One of the first to change is the face and décolleté area. Often, it is these areas that patients attach particular importance to, and completely forget about the hands – a zone that gives out the true age, and which is also amenable to the above changes.

Age-related volume loss in the hands, as well as on the face, is one of the main factors of age-related changes. But in the arsenal of aesthetic medicine there are effective ways to correct even such areas.

The hands are not always given enough attention, but perhaps this is one of those areas that can give out age. Therefore, experienced cosmetologists and plastic surgeons very often suggest that patients pay attention to this area.

Patient, most often, themselves are the reason that the area of ​​hands is deprived of attention.

They are good at seeing problems on the face or neck, and hands don't always bother them. The task of the specialist – tell the patient about it.

Which age category patients are most likely to seek this procedure

V.P.: Of course, these are women who are over 40 and over, but a lot also depends on the constitution. There are such thin women of an asthenic physique who, even at a young age, can show such "boney hands" when the tendons and interphalangeal spaces are very clearly defined.  Therefore, age is not always the main determining indication for this procedure.

V.P.: Of course, you need to pay attention to relief, because, most often, patients are worried about the effect of "bony".

The second thing that patients can complain about, and what specialists should pay attention to – this is the severity of the venous pattern on the hands, since the veins can be very contoured. In some cases, this is a direct result of thinning of the subcutaneous fat.

There are ways to remove veins in the arms, and there are options when lipofilling can serve as an alternative solution. An alternative solution is that, having restored the volume of subcutaneous adipose tissue, the venous pattern is not so clearly visible.

The third point worth paying attention to is the condition of the skin itself, that is, its moisture content, the presence or absence of pigmentation. Peelings, biorevitalization, and plasmolifting can be applied here. Only a comprehensive solution to the problem leads to the best result.

How is the sampling of adipose tissue for microlipografting 

V.P.: Adipose tissue is taken, most often, from areas where there is a slight excess of fatty tissue, the so-called fat traps. This can be the lower abdomen, outer and inner thighs.

From these parts, most often, by syringe liposuction, a certain portion of fatty tissue is taken through small punctures with a thin cannula, and then it is cleaned.

Purification means that the resulting fat substrate, which contains a local anesthetic, may contain plasma and blood impurities, is subjected to centrifugation, and sometimes washing with saline, depending on what kind of substrate we received.

V.P.: If, for example, there is an admixture of blood and other components, then it is necessary  washing, settling and centrifugation. If, in advantage, there are fat cells without blood impurities – then only centrifugation is necessary.

Anyway, the purpose of fat preparation is to cleanse the living whole fat cells of other impurities, destroyed fat cells, fat emulsion, blood, and anesthetic.

What are the features of the recovery period after hand lipofilling

V.P.: There may be slight swelling in the donor area where the fat is taken from. Sometimes, if the patient's blood clotting is not very good, there may be small bruises, and this area may be a little sensitive.

In the injection zone, only swelling and a moderate increase in sensitivity to palpation are observed. Puffiness persists for about 10-15 days.

Minimum recovery period after microlipografting of hands – week, maximum -2-3 weeks.

A lot depends on the local tissue reaction, because there are pasty people who are prone to edema, whose lymphatic system works worse and microcirculation takes longer to recover. Much depends on the volume of hand lipofilling.

What complications can occur after this procedure

V.P.: A lot depends on the skills of the doctor who performs the procedure. In the donor area, complications occur very rarely, but if the specialist does not have good enough skills, and  there are some errors in the quality of the procedure, there may be irregularities if the fat was taken unevenly.  Puffiness, bruises and hematomas, if they are pronounced – this is also a complication.

In the recipient area where we inject fat – sometimes there may be the formation of such fibrous cords, encapsulated fat fragments, which can also create tuberosity, irregularities, as well as inflammatory complications, such as infiltrates, suppuration.

If too much fat was injected during arm lipofilling, this may lead to the need for a second surgical intervention in order to drain this space and eliminate dead fat cells, as this can lead to an inflammatory process.

How to avoid such complications

The key to avoiding these complications is a moderate amount of grafted fat, and a uniform introduction of it.

Today, the microlipografting technique is considered more acceptable, when fractional portions of fat are injected into different layers of soft tissues, rather than large conglomerates.

The thinner the layer of fat that is injected into the tissues, the more evenly it is distributed, the more likely it is to take root, and the less likely it is to inflammatory processes and irregularities, infiltrates, compaction, fibrosis, and so on.

The engrafted fat cells remain there for life - this is a permanent effect. Another question is that atrophy processes may occur, because we cannot stop the aging process.

So maybe  after a few years, a second procedure will be necessary, because atrophic changes may progress.

The disadvantage of the type of lipofilling – microlipografting of the hands is also that, within a short period of time (a month and a half), part of the fat cells are absorbed, and those fat cells that have not taken root and have not received sufficient nutrition from the capillaries die.

Phagocytosis occurs, the immune factors dissolve them, and about 30-40% of the volume is lost. But 60-70% of the volume that have survived and remained alive are integrated into a new tissue area, have their own blood circulation, they live a normal life of a fat cell and then create volume in a new area.
