The papilloma virus in women can have many manifestations, one of them – sexually transmitted bowenoid papulosis. Its connection with the papillomavirus was established not so long ago, and now, if warts and genital warts in the perineal area are detected during examination by a gynecologist, the doctor has reason to prescribe an additional examination for papulosis. The manifestations of this disease in the form of spots, papules and plaques are considered a precancerous condition of the skin, therefore, as estet-portal.com notes, they require urgent treatment.
Manifestations of bovenoid papulosis as a form of papillomavirus in women
Bovenoid papulosis practically does not cause unpleasant subjective sensations, therefore it is often detected only during a routine examination by a gynecologist. This disease is associated with the activation of human papillomavirus types 16, 18, 31 and 33 during their sexual transmission.
On examination, the doctor should pay attention to red-brown, yellowish or pink elements a few millimeters in size, which are localized on the skin of the perineum.
Inflammatory elements of bowenoid papulosis can be either single or multiple, reaching 3 cm in size. They come in different forms:
• pigmented papules – the color is red-brown, pink, purple or yellowish; doughy consistency; the surface is often smooth, sometimes warty;
• lichenoid papules – local thickening of the skin with a rough, enhanced skin pattern; do not differ in color from the skin, have a rough surface, sometimes covered with serous crusts;
• leukoplakia-like plaques – formed by the merger of individual papules; the edges are clearly demarcated from healthy skin; have a grayish or milky white color.
Typical arrangement of elements for women – on the labia and clitoris, in the vulva, around the anus, on the skin of the inner thighs. Pigmented papules are possible on the mucous membrane of the mouth and pharynx.
Bovenoid papulosis in women is often combined with warts and genital warts, with cervical dysplasia.
Sometimes a woman complains of itching in the area of manifestation of bowenoid papulosis elements. When they become inflamed, skin soreness can develop when touched. Papulosis is characterized by a long course without exacerbations and progression, without vivid symptoms, with periods of spontaneous remissions. However, there is a high probability of malignant degeneration of elements of bowenoid papulosis and the development of squamous cell skin cancer or cancer of the vulva.
Diagnosis and treatment of papillomavirus in women, manifested by papulosis
Assumptions about the presence of bowenous papulosis most often arise during a gynecological examination, after which a woman is assigned a consultation with a venereologist. The patient is assigned cytological and histological studies, PCR diagnostics.
• Cytology of a smear from the genital organs shows epithelial cells characteristic of bowenous papulosis with enlightenment around the nucleus or with doubled nuclei.
• Histological analysis shows atypical mitosis and proliferation of keratinocytes characteristic of malignant neoplasms.
• PCR research is aimed at identifying and typing the papillomavirus in women.
Bovenoid papulosis is differentiated from syphilis, lichen planus,
Combination therapy involves a combination of general antiviral treatment, local use of cytostatics on the affected skin areas and removal of papulosis elements – by cryodestruction, electrocoagulation, laser or surgical excision. The method is chosen depending on the localization of the elements. Despite the effectiveness of the therapy, the disease tends to recur. Therefore, patients with bowenoid papulosis should remain under dynamic medical supervision.
