Experts call rash vesicles on hands dyshidrotic eczema or pompholyx. This disease accounts for approximately 20% of all dermatological lesions of the hands and has a certain seasonality of manifestation – autumn-spring off-season, when the skin on the hands often dries up from sudden changes in temperature. One of the names of this disease is associated with a version that has not been proven today about a violation of the activity of the sweat glands, the other copies the Greek term, which means "bubble" in translation. The disease often develops against the background of nervous strain, chronic infections, weakening of the body's defenses. With timely adequate assistance, the blisters on the hands disappear quickly, but the disease tends to recur.

What are the reasons   bubbles on hands

The exact cause of pompholyx is not known. Most often, blisters on the hands as a manifestation of dyshydric eczema occur in those who have a history of atopic or nummular dermatitis. Other provoking factors for the development of the disease can be:

  • susceptibility to allergic reactions,
  • contact dermatitis (especially nickel allergy),
  • infectious diseases.

Often bubbles on the hands appear against the background of stress, strong emotional experiences, nervous and mental overstrain. But experts believe that such factors can only exacerbate the course of the disease, and not be the cause of its manifestation.

Features of blisters on the hands with pompholyx

Pompholyx, or dyshidrotic eczema, is characterized by the appearance of small blisters on the hands, a bit like croup. Sometimes the appearance of bubbles may be preceded by a slight fever, slight malaise, a tingling sensation in the skin. Erythema is very rare, the vesicles on the hands are localized on the palmar or back side, between the fingers. Quite rarely, pompholyx can develop on the soles of the feet – simultaneously with the defeat of the hands or even without it. Starting in one place on the palm, bubbles can spread from the palm to the back of the hand and even to other parts of the body.

Bubbles on the hands with dyshidrotic eczema are dense, can be single or grouped, they are filled with a clear liquid and cause severe itching. If the inflammatory process intensifies, then the bubbles become multi-chambered. Due to scratching of itchy plots, bubbles can increase and merge, in their place weeping erosions with a bright bottom and uneven edges are formed, which then become covered with crusts. When the crusts fall off, peeling remains in their place and cracks may appear on the skin.

Maceration of the skin can promote secondary bacterial infection. In addition, vesicles on the skin with dyshidrotic eczema can cause trophic changes in the nail plates.

How to treat vesicles on the hands with pompholyx

When diagnosing the cause of blisters on the hands, one should take into account the likelihood of developing not only pompholyx, but also contact dermatitis, and dermatophytosis, and atopic dermatitis. Laboratory and clinical data will help establish the diagnosis.

Typically, a pompholyx outbreak resolves after 2-3 weeks. The patient is recommended local emollients, antihistamines, if necessary, sedatives, especially at night. If large bubbles appear on the hands, their drainage is shown. The addition of a secondary infection becomes a reason for prescribing antibiotic therapy.

It is quite difficult to treat dyshidrotic eczema that has passed into the chronic stage. Local corticosteroid ointments and dressings, keratolytic agents are usually recommended. A good effect is given by hardware procedures – laser therapy, ultraviolet irradiation.

Patients with blisters on their hands should be advised that dyshidrotic eczema often recurs. You can prevent the recurrence of the disease by following a diet, a correct attitude towards working with household chemicals, hot water and other factors that irritate the skin.
