The presence of a pathological process in any organ is often accompanied by a reaction of the whole organism. Many chronic processes are characterized by the development of arthropathy. Then patients think they have another disease that needs to be treated. In fact, these are reactive changes in the joint, which are manifested by arthritis and arthralgia. These changes in the joints disappear with the treatment of the underlying disease. Find out what chronic pathologies can be accompanied by arthropathy? How are the joints affected in Reiter's syndrome?
Reiter's syndrome is accompanied by the development of a triad of symptoms: damage to the organs of vision, genitourinary system and joints. The causes of the syndrome are chlamydia, shigella, salmonella, etc. Usually, symptoms of an acute genitourinary infection (urethritis, cystitis), or enterocolitis, first appear. Then the eyes are affected with the development of keratitis, retinitis, iridocyclitis, conjunctivitis or uveitis. Arthropathy develops in a month – one and a half after infection.
Often the development of arthropathy is the first of the symptoms with which the patient turns to the doctor, not noticing the other manifestations of the disease or not associating them with pain in the joint. As a rule, the joints of the lower extremities are affected, the lesion is asymmetrical. The joints are involved in the process from the bottom up, all the joints of the upper limbs are gradually affected. At the same time, they are hyperemic, edematous, sometimes an effusion is found. Possible development of sacroiliitis or calcaneal bursitis.
Arthropathy due to infectious and parasitic diseases
The development of many infectious and parasitic diseases is accompanied by arthropathy. So, brucellosis, trichinosis and Lyme disease develop with volatile joint pain, sometimes in combination with myalgia. Mumps is accompanied by arthropathy, which is migratory in nature with possible inflammation of the pericardium, which resembles the clinic of rheumatoid arthritis. Chickenpox and infectious mononucleosis are also associated with mild arthropathy.
Against the background of meningococcal infection, arthropathy manifests itself in the form of monoarthritis of the knee joint, which develops a few days after the onset of the disease. Viral hepatitis develops with a symmetrical lesion of large joints, as well as small joints of the hand. Joint pain usually occurs before jaundice appears.
Features of arthropathy in the presence of pathologies of the immune, endocrine and vascular systems:
- Arthropathy in vasculitis. Takayasu's syndrome, periarteritis nodosa and Churg's granulomatosis – Strauss, as a rule, proceed in the form of arthralgia. Kawasaki disease is characterized by arthritis or arthralgia. Symmetrical damage to large joints is observed in Shenlein's disease – Henoch and Wegener's granulomatosis. In addition to the pain syndrome, there is also swelling of the tissues.
- Allergic arthropathy. The development of allergic reactions may be accompanied by pain in the joints. Moreover, arthropathy can occur both immediately after contact with the allergen, and after a while. Articular syndrome is manifested by pain during movement and at rest. Arthropathy disappears on the background of desensitizing therapy.
- Arthropathy is also observed in menopause, diabetes mellitus, hyperparathyroidism, hypo- and hyperthyroidism.
Manifestation of arthropathy in pathologies of internal organs
The most famous arthropathy in pathologies of internal organs is Marie's syndrome – Bamberger, which is accompanied by deformation of the nails and fingers. The nails become like watch glasses, and the fingers take on the appearance of drumsticks. Such a deformation develops against the background of ossifying periostosis of the distal parts of the tubular bones, which appears when the bone tissue reacts to violations of the bone-basic balance and oxygen deficiency. Such a syndrome develops with pathologies of the pulmonary system.
Crohn's disease and UC are accompanied by the development of acute migratory arthritis affecting the knee and ankle joints. After the course of treatment, acute manifestations of arthropathy disappear within two months.
