Wolfgang Redka-Svoboda (Germany), surgeon, associate professor at the Paracelsus Private Medical University in Salzburg, head of the Expert Day Anatomy anatomical course, member of the German Society of Surgeons, medical director of TEOXANE Germany, told estet-portal.com about the features of creating « sexual vector» of the face, the role of nasolabial folds in non-verbal communication and the features of their correction, and also shared interesting information about the author’s technique for correcting the middle part of the face through one point of injection of the drug (“beauty point”).
ED. Please tell me what a "sex vector" is. faces. How is he educated? How to create a "sexy vector" correctly on the face?
В.Р-С. Esthetists annually come up with new interesting terms, one of which is the sexual vector of the face. In fact, this vector – nothing more than a projection of the zygomatic arch, which gives the face a youthful appearance, and in older patients also provides the effect of lifting the lower part of the face.
I recommend working with the sexual vector at the level of the deep layers of the tissues, since there are retaining ligaments in the zygomatic region that prevent the injected material from descending. Therefore, if, when creating a sexual vector, we inject the drug too superficially, it will soon diffuse into soft tissues or the surface layer of fatty tissue – and the effect disappears.
To prevent this from happening, you need to inject the drug very deeply, into the area located above this ligament, which helps to keep the product at the injection site and get a lasting result. Thanks to this, we give the face a beautiful, youthful look that lasts for a longer time.
ED. What is the role of the nasolabial fold in nonverbal communication? How to work with this area?
В.Р-С. Previously, when working with the nasolabial fold, we performed a procedure aimed at smoothing wrinkles, that is, we corrected the nasolabial folds by filling them with fillers. Today we know that wrinkles and lines – this is of course a problem, however, the tired look of the face associated with the aging process is not the result of the presence of the nasolabial fold, but the loss of facial tissue volumes.
The more volume we lose, the deeper some wrinkles become, especially the nasolabial fold or marionette lines (Merkel lines), as we call the deep lines running from the corners of the mouth to the chin). By restoring the volume of the face, we are able to correct them without resorting to the injection of the drug directly into the area of the wrinkles and folds themselves.
Now we are introducing a little material to achieve the so-called photoshop effect – that is, fill the hall in the skin, if any. The nasolabial fold is very, very important for non-verbal communication: when we see a person at a distance of 50 meters, the fact that he smiles, laughs or cries can be seen from the nasolabial fold.
If we "erase" nasolabial fold from the face, by introducing too much filler into it (when the drug can practically be seen or felt), the face will simply lose its expressiveness – and it's ugly.
This is why I am very careful when working with nasolabial folds – I only add a small amount of product to achieve the photoshop effect, but I try to correct the rest, replenishing the volume in the middle part of the face.
ED. What are the benefits of your unique "one approach to beauty" method that you introduced today?
VR I attend various congresses around the world not only as a speaker, but also as a listener. Accordingly, I often watch my colleagues inject. Many of them work with 20-30 insertion points on the face, which made me think about how traumatic these insertion techniques are, and also to find an easier way to make this procedure more gentle for the patient.
Understanding the different fat compartments and using blunt-tipped cannulas that allow long-distance injection allowed me to create a “one beauty approach” that can treat approximately 80% of the midface with just one jugal puncture .
With the cannula, I can treat almost all the areas I need through this point. Of course, sometimes to work with the nasolabial fold, if a hall has formed there, or the mandibular line, you need to use a needle or create another access point. However, in any case, I use the minimum number of insertion points.
And thanks to the use of blunt-tipped cannulas, it is possible to avoid the appearance of a large number of hematomas and edema, since the procedure is performed atraumatically.
Such cannulas easily glide over soft tissues, so it is very easy to work with them and guide them. For me, the cannula is comparable to the bow of a violin, which allows you to play light romantic music.
Thus, due to the soft and smooth operation of the cannula, the procedure is atraumatic and excellent results are achieved – patients look younger and don't worry about bruising.
How to find the right "beauty point" to work with?
ED. V.R.S. I approach this issue very creatively. For me, injectable facial rejuvenation – it is a whole art. The point of beauty with which I work is located in the zygomatic region, however, I cannot say with accuracy at the intersection of which and how many lines it is located – I don't like the standardized approach to finding the injection site.
Every face is different, so the location of the insertion point varies from case to case.
Thus, I find the point of beauty, so to speak, intuitively. Of course, there is always one in the same area: sometimes 1 cm more medially, lateral, above or below – depending on the characteristics of the patient's face. However, there is no fixed point that can be precisely determined by calculation through which I work.
