The possibilities of aesthetic medicine are expanding every year, and the requests of patients for the correction of appearance are growing. On the one hand – this is due to the emergence of new equipment, the development of advanced technologies, the improvement of techniques in plastic surgery, and on the other hand – of course, it depends on the growing qualifications of doctors who strive to fully realize themselves in their favorite profession.
About trends in plastic surgery, about the requests of patients and about the importance of self-realization for a specialist – read in a special interview for estet-portal.com Vasily Vasilyevich Khrapach, MD, professor, specialist in plastic, general and vascular surgery.
Why has the number of plastic surgeries increased so much lately?
V.Kh. I think that, firstly, people have the opportunity to perform such operations. I mean not the financial, but the technological side - that is, today we have the necessary equipment, specialists, methods, and we can provide people with the opportunity to do this.
Regarding rejuvenating operations – there are fewer interventions on the face, if you count them as a percentage of the complex of plastic surgeries. Why? Today, very effective cosmetic methods have appeared - injection, with the use of fillers, threads. At the same time, there is no competition between surgery and cosmetology in any way, because surgery – basic: that is, we pulled up the block – and the improvement is already being made at the level of cosmetology.
In general, now in society there is a desire among people to look good. This is especially true for women, and their desire to look better is sometimes explained by social reasons. For example, there are studies that clearly trace the trend when, after any cataclysms in the country, including wars, women's expenses increase, let's say, to improve their appearance.
Why? Because a hard life in conditions of cataclysms accelerates aging, and young girls are growing up nearby, competition is intensifying.
Remember the era of the 50s of the last century – short skirts, fashion for cleavage, a sharp rise in interest in anti-aging treatments.
For other reasons – let's say, improving appearance for the sake of success in business - this, in my opinion, does not work for us. In our country, there is no rigid dependence yet, when the appearance of the applicant strongly influences the decision of the employer. If a woman – leader, has her own business, then she is engaged in her appearance for herself, and not for the evaluation of others. Yes, and men too.
There are often situations when problems start in the family, and the patient complains that her husband threatens to leave her if she does not take care of herself. We are trying to explain that even if she eliminates some flaws in her appearance, this will not save the family, it will not affect the relationship.
Which plastic surgeries are in the greatest demand now
V.Kh. Mammary gland prosthetics are of the greatest interest - these operations are certainly in the lead. In second place is rhinoplasty, in third place are purely anti-aging operations (facelift, blepharoplasty).
It should be noted that in different countries the preferences for the correction of appearance are different. In Italy, for example, less facial plastics are performed than nose correction operations; in America, more facial corrections; in Brazil – pay more attention to the contours of the body (chest, buttocks).
Now there are a lot of operations with the use of lipofilling, because liposuction and lipofilling in the eyes of the layman – this is something not very operational, so the demand for them has become more.
Most of the patients of the clinic of aesthetic medicine – they are usually women. Do men go to plastic surgery? And with what problems?
V.Kh. Of course, men also apply. Most often they are concerned about aging – they come with the traditional phrase that it has become disgusting to look at themselves in the mirror in the morning, and they would like to have a rejuvenated face. This request is made mainly by men over 45.
Among other problems, the more serious – this is rhinoplasty, when nose correction is needed after injuries to restore nasal breathing; younger men are treated with this – 30 year olds on average. Sometimes we correct cryptorchidism – put the missing testicle in place.
Young men are treated with gynecomastia – this happens if you overdo it with pumping muscles according to incorrect, simplified methods, and you have to carry out surgical correction.
How popular are simultaneous operations, when several zones can be corrected at the same time
W.H. I once had a personal record – I did nine operations in one night for one person. True, I must admit that in that case my opinion was not asked, it was simply necessary. So, in principle, it is possible. I believe that if it is possible to combine the interventions of – it's worth doing, it's better than doing several separate operations. Much depends on how voluminous the operations are. If the wound surface is not very large and we see that it is not life threatening, then it can be carried out.
It is considered that if the duration of the operation does not exceed two hours – this is one criterion for the risk of complications, more than two hours – other.
With the current level of anesthesia, according to our observations, we have reduced this limit to three hours. If there is more in the total duration, I try not to combine.
Do do you have any favorite operations that you, as a professional, are most interested in?
V.Kh. Of course, mammoplasty. You understand, the face – this is caution, nose – this is a mathematical calculation, belly – it is a semi-general surgery. Lipofilling – there is nothing at all, a cannula and that's it. And while operating on the chest, you are constantly sculpting, creating. Especially if you come across a difficult case – these operations are technically interesting.
What are the most interesting discoveries in plastic surgery that you would note over the past year?
V.Kh. Perhaps there was nothing so revolutionary. Mostly at major international forums, we are talking about technologies for improving – ranging from tools and equipment to technologies, say, forecasting.
Do you think that our domestic specialists work at a sufficiently high level?
V.H. I myself – just such a domestic specialist (laughs, ed.), and I know for sure that we work at the same level as surgeons from other countries, there are no questions here. Plastic surgery – it is a specific industry. First – we have the opportunity to buy what we want – equipment, tools, materials, which is difficult for doctors of other specialties to do.
Thus, we have practically the same equipment as any Western clinic, we have the same technologies, our level of knowledge is no worse.
Do you think that you have managed to realize yourself?
V.Kh. Yes, I consider myself a happy person. I received many invitations to work in Western clinics, but I decided that I would better realize myself here. My profession, my vocation, my way of obtaining a livelihood coincided with me. This rarely happens to anyone. I'm lucky. And besides, I love my job.
What else would I like to get out of life? To be able to work, to do my favorite thing for as long as possible.
