Биостимуляция и биореструктуризация: препараты для комплексной коррекции возрастных изменений

Prolonging youth and slowing down the aging process remains one of the main tasks of aesthetic medicine. But in order to get the maximum result, it is necessary to understand what processes occur in the skin during its aging and which biological functions of the skin need to be stimulated in order not only to get a visible rejuvenation effect, but also to really restore and improve the skin. Tatyana Kolomoets – Candidate of Medical Sciences, dermatologist of the highest category, CEO of EuroEstetGroup.

What dermal processes cause the manifestation of age-related changes

In the dermis there is a continuous process of synthesis, renewal and biodegradation of biological components. Complex compounds such as collagen, elastin and glycosaminoglycans, hyaluronic acid are constantly being formed and broken down. Restoration of worn tissues is carried out due to the protein synthesis of fibroblasts. With age, under the influence of destructive factors, the rate of renewal of the epidermis slows down, and the barrier functions of the skin are increasingly weakened. Fibroblasts are damaged and, as a result, the synthesis of extracellular matrix components slows down. However, the skin has a fairly good ability to heal itself. And our task – help her in this, stimulate the cells of the dermis in order to renew it and, accordingly, rejuvenate.

Synthesis of the structural components of the dermis is accelerated by endomodulation. With regard to the dermis, this is the biological activation of its functions, which helps to improve its condition and stimulate the synthetic activity of fibroblasts.

Fibroblast is known to – this is the main cell of the middle connective layer of the dermis, its only cell synthesizing the components of the extracellular matrix.

The main task of progressive rejuvenation techniques – stimulation of fibroblast production activity.

It is possible to implement this in various ways, the most ancient of them – mechanical stimulation of the skin, massage. But the most efficient way – the introduction of drugs containing certain substances that help restore the biological activity of the skin. The most famous and popular of these substances is hyaluronic acid. She is the one – the source of youth and beauty of our skin due to its unique ability to regulate hydrobalance, influence the processes of cell regeneration and metabolism, and have a significant antioxidant effect.

How does the type of hyaluronic acid affect the success of age-related changes correction

One of the most popular methods of skin rejuvenation with the use of HA – biorevitalization, the introduction of drugs containing high molecular weight hyaluronic acid. This procedure undoubtedly gives a pronounced rejuvenating effect, however, VMHC, which is part of biorevitalization preparations, does not have a stimulating effect on fibroblast.

In scientific studies, it was found that fragmented HA, consisting of 20-38 disaccharides, has the maximum affinity for CD44 receptors in fibroblasts and is able to provide their active stimulation. Procedures with the use of NMGK are called biostimulation.

Biochemically fragmented HA has been proven to be:
•    promotes the restoration of the structure of the dermal matrix,
•    stimulates collagen synthesis,
•    prevents the accumulation of lipofuscin,
•    blocks the development of oxidative stress,
•    promotes the production of its own hyaluronic acid.

As a result of this, we get bright results in restoring skin tone and turgor, eliminating sagging, hyperpigmentation, manifestations of rosacea, reducing the depth of wrinkles.

The most famous and effective products based on LMHC are the FillerVit group of injectable preparations from the Italian pharmaceutical concern Italfarmacia, which is represented on our market by EuroEstetGroup. The basis of the composition of the preparations is NMGK (20-38 monomers), amino acids and other precursors, vitamins and buffer compounds –  everything that the dermis needs for self-healing. This combination of active ingredients contributes to achieving maximum results. HA monomers activate fibroblast cell membranes, and precursor amino acids provide biosufficiency to cells in their development, optimize the formation of the dermal matrix, and slow down the activity of metalloproteinases.

Why age-related changes depend on the production of collagen and how to correct them

Fibroblast produces immature collagen – tropocollagen, from which the "assembly" collagen fibers. There are mainly two types of collagen produced in our dermis. Reticular (collagen type III) consists of very thin water-soluble fibers and is characteristic of young tissues. It is he who provides the skin with elasticity, maintains its turgor and elasticity. With age, the production of this type of collagen decreases and type I collagen becomes dominant.

Fibrous collagen creates thick fibers and bundles in the deep dermis, providing strength and support to the entire skin. This type of collagen is characteristic of mature tissues and is produced due to intracellular components released during tissue damage and inflammatory mediators (a consequence of tissue damage). Those. The production of type I collagen occurs, among other things, due to traumatic factors.
So, by acting on the fibroblast with hyaluronic acid monomers, we force it to produce structural units of collagen, which subsequently form different types of collagen fibers.

Depending on the aesthetic problem and the age of the patient, the activation of the production of collagens of different types should occur in different ways. In accordance with this, we distinguish two concepts – biostimulation and biorestructuring.

If we talk about FillerVit injectables, then FillerVit B has a biostimulating effect, and FillerVit R has a restructuring effect. What is the difference between these methods and what causes different effects on the skin? In this case, we can say that a different biological effect on the skin is due to a different composition.

How does FillerVit B affect the skin

FillerVit In – preparation for biostimulation of the dermis. Indicated for young skin with signs of dehydration, reduced tone and turgor, loss of elasticity and other early signs of aging.  It contains:
•    LMHA (20-38 disaccharides),
•    L-form precursor amino acid complex (L-isoleucine, L-leucine, L-lysine hydrochloride, L-proline, L-valine, glycine, L-serine, L-alanine, L-cysteine),
     bicarbonate buffer.

HA monomers have a stimulating effect on the CD 44 receptors of fibroblasts, due to which their own hyaluronic acid, elastin and collagen begin to be actively produced.

The complex of amino acids serves as a kind of "substrate" due to which collagen fibers are formed (in the collagen molecule, 1/3 of the amino acid residues are glycine, proline and lysine are included in the amino acid sequence in the process of collagen synthesis in fibroblasts).

The bicarbonate buffer preserves the physiological pH, thereby preventing the formation of fibrous collagen. Also, maintaining the pH at 7.4 allows the HA molecules to be in a state of maximum hydrophilicity and hygroscopicity.

This preparation provides a significant improvement in the condition of the skin, has a significant moisturizing effect and provides the skin with a long-term hydro-reserve.

FillerVit B is indicated for patients over the age of 25 as a prevention of aging processes. In this case, we get an instant and pronounced effect. Also, this drug can be used in patients of an older age group in complex rejuvenation programs.

The drug is usually administered by papular minimally invasive technique, the recommended course is 6 procedures (4-1p per week, 2-14 days later). The introduction of FillerVit B into biologically active points leads to an increase in the effect due to a combination of pharmacological and acupuncture effects. The biostimulation procedure with FillerVit B is practically painless, leaves no marks and does not require a rehabilitation period.

What is the peculiarity of the effect on age-related skin changes in FillerVit R

The main purpose of FillerVit R – restructuring of the dermis, rapid production of type I collagen and a powerful lifting effect.

FillerVit R also contains HA monomers and the amino acids L-proline, hydrochloride  L-lysine, glycine. Indicated to eliminate flabbiness and atony of aging skin, restore facial contours, treat skin with a finely wrinkled type of aging, as well as to eliminate flabbiness of the skin of the body.

The acidic environment of the drug (pH5.8) provides a traumatic effect, due to which biorestructuring proceeds with the formation of dense type I collagen.

The effect of rejuvenation after a course of biorestructuring is not inferior to the results of RF-lifting and median peels.

At the same time, FillerVit R not only gives a powerful lifting effect, but also provides skin hydration.
FillerVit R also serves as an excellent alternative to the popular threadlifting procedure, because does not have those undesirable consequences that can occur when setting threads.

One ampoule of the drug contains 10 ml of active solution, which is used to work with the skin of the face, neck and decollete in one procedure.

Complex application of FillerVit preparations in the correction of age-related changes

FillerVit B and FillerVit R can be used as monotherapy or combined in skin treatment courses.

If we talk about complex programs for the use of drugs from the FillerVit group, then they are absolutely universal and, depending on the aesthetic problem, can be used in combination with other anti-age methods. It is advisable to use FillerVit preparations before starting a course of any other procedures, this allows you to start the processes of developing your own components of the dermis.

FillerVit recommended:
•    in programs that combine the administration of LMHC and IMHC preparations,
•    before some hardware techniques,
•    before fillers,
•    in combination with the plasmolifting procedure (PRP).

The preparations are also indicated in preparation for surgical interventions and rehabilitation after plastic surgery, laser correction of facial and body tissues, deep chemical peels. You can learn more detailed information about preparations, methods and application protocols at specialized seminars of the Training Center EuroEstetGroup.
