Medical technologies are developing rapidly, and in aesthetic medicine, new methods of correcting appearance and rejuvenating techniques are being intensively mastered. On the one hand, they are becoming more and more complex, which requires constant improvement from specialists, and on the other hand, they are becoming more diverse, so the best results for the patient can be achieved through the joint efforts of cosmetologists and plastic surgeons.

Alexander Ivanovich Nerobeev, one of the world's leading plastic surgeons, Doctor of Medical Sciences, Professor, General Director of ART-Clinic, answered a few questions about the trends in the development of aesthetic technologies.

What's new in aesthetic technologies lately?

There have been many changes. First of all, an endoscopic lifting technique has appeared, which involves micro-punctures instead of incisions. She quickly gained popularity, although she met with some resistance from doctors. The fact is that at one time there was a bias towards endoscopy, a huge number of patients wanted to do this particular type of facelift. But the technique turned out to be difficult and took a long time (sometimes the operation could last 8 hours), many surgeons encountered difficulties. It happened that after an endoscopic facelift, the appearance of patients changed greatly, which was especially noticeable in the example of some actresses. This is due to the fact that during endoscopic surgery, tissues exfoliate from the bones and move to a new place, which can cause a radical change in appearance. Hence, a number of surgeons have a negative attitude towards the technique itself. There was a division of opinions: someone believed that endoscopy is an excellent modern technique, others pointed out its individual shortcomings.

But progress didn't stop there?

Definitely not. It can be assumed that the complexity of the endoscopic technique and the cautious attitude towards it prompted the search for alternative low-traumatic methods.

As a result, the second wave of improvements began, the now popular thread lifting. In the West, it is called "office" surgery, because the scale of intervention here is minimal, the procedure can be performed under local anesthesia. Roughly speaking, in the morning I had an operation - in the evening I went to a banquet.

What results can a thread lift achieve? What threads are used for this?

Threads can be used to move the skin in a given direction and fix its new position. For example, a thread lift makes it possible to straighten the nasolabial folds, raise the "flews", and make the chin line clearer. The result is stored up to 2-3 years. Moreover, the threads are suitable not only for rejuvenation and lifting, they can be used to correct the shape of the nose without any operation, raise its tip, and even improve nasal breathing. Already 15 minutes after the threads are placed, the patient not only admires his reflection in the mirror, but also begins to breathe freely.

Following the facelift, threads began to be actively used to lift the mammary glands, abdomen, buttocks.
Today, there is a large selection of different threads - absorbable and non-absorbable, with cones, with notches, with loops, some of the threads can even be re-tightened.

The latest novelty is meson threads, which are inserted into the thickness of the skin and affect its quality and structure. As you know, there are two main types of aging - deformational and finely wrinkled. Deeper threads help mainly people with deformation changes in the face, who have relatively few wrinkles, and the problem is sagging skin. Now we can work in two layers at once - directly in the skin and in subcutaneous tissues, that is, to rejuvenate patients with a fine wrinkled type of aging.

In other words, thread lifting has become a real breakthrough in aesthetic medicine?

Undoubtedly. Thanks to the threads, the facelift has become affordable and popular. A huge plus is that the procedure does not require general anesthesia, which many fear. People argue like this: I’d rather do a simple thread lift now, which does not require a long rehabilitation and is quite affordable for me, and in 2-3 years I will repeat it, than I will lie down under the surgeon’s scalpel and then wait a month until all bruises and swelling go away. Patients now have a comfortable alternative!

But it should be noted that with all its advantages, thread lifting is not indicated for everyone, but only for patients with initial signs of aging without pronounced ptosis (omission) of tissues. That is, we are talking about the interval from 40 to 54 years, when it is still possible to do without serious surgery.

Will minimally invasive techniques eventually replace the traditional facelift?

Unfortunately, no. After 60-65 years, it will not be possible to radically change the appearance with threads alone, with all the desire. But if a person from 30-35 years old regularly visits a cosmetologist, skillfully combines hardware and injection procedures, supplementing them with a thread lift, then youth can be extended for many years. The question here is how early you realized and began to make efforts to preserve what is.

Judging by our clinic: four years ago, the main demand was for surgical techniques, and now patients are more willing to go for minimally invasive procedures (including hardware ones) and cosmetology.

What can we expect in the near future?
First of all, the further development of minimally invasive techniques. For example, already now there are threads that, 1-1.5 months after the introduction, turn into a gel that fills the tissues. Thus, the effect is doubled: first we get a thread lift, and a month later - contouring.

What about conventional surgery?

High-quality surgical facelift (endoscopic or SMAS) will not go anywhere and will remain the "gold" aesthetic standard. Another question is that now there are fewer and fewer specialists of this class who can carry it out.

By the way, in the West in medicine there is now an active introduction of computer technologies and robots that can perform various operations. Perhaps this trend will also reach aesthetic surgery.

Surgery or cosmetology?

Ideally, each patient should be seen by a beautician and a surgeon, and they should work together to develop the best plan of action. Because the cosmetologist is limited in his techniques, and the surgeon is limited in his, together they can achieve maximum results. This approach is the future.
