Collagen stimulating fillers: injection technique, contraindications, complications

In the course of preparing a person for injection procedures, it is necessary to use the asepsis method.

The depth of injection of the collagen stimulating filler is determined depending on the target area.

The author recommends supraperiosteal administration for:

  • temporal fossa filling;
  • malar area volumization;
  • cheek augmentations;
  • contouring of the lower jaw.

A more superficial injection is advisable if it is necessary to replenish the subdermal volume and provide the effect of biostimulation – increase in the amount of collagen and elastin in the dermis.

Doctor recommends using a 27 gauge needle or 25 gauge cannula for insertion.

Contraindications to correction collagen-stimulating fillers are autoimmune diseases such as inflammatory bowel disease or rheumatoid arthritis.

Side effects and complications

Swelling and bruising – typical complications that often occur after the introduction of any dermal fillers.

More serious consequences can be:

  • delayed nodule formation;
  • inflammatory reactions;
  • infections.

Case study: facial correction with Ellans fillersé

A 62-year-old female patient presented to the doctor with complaints of loss of volume in the malar zone, reduced definition of the contour of the lower jaw, and superficial wrinkles, especially in the perioral zone.

Preliminarily, the patient underwent:

  • SMAS-lift (15 years ago);
  • botulinum toxin injections (on a regular basis).

For contouring, the author recommended Ellansé collagen-stimulating fillers; S and Ellansé M.

Polycaprolactone microspheres in such preparations are smooth, do not exceed 50 mm in diameter and are stable in size and shape.

The results of the introduction of polycaprolactone collagen-stimulating fillers last much longer – for more than two years.

To make the procedure as comfortable as possible for the patient, the doctor mixed each 1 ml of filler with 0.2 ml of lidocaine (1% or 2%).

This amount of anesthetic is safe and does not affect the physical properties of the gel.

Volumization of hypodermal fat, increase in thickness and elasticity of the dermis is comparable to the effect after the use of boosters based on hyaluronic acid.

The doctor injected the filler supraperiostally, using both a needle and a cannula, along the mandible to provide support:

  • mental muscle;
  • premaxillary sulcus;
  • posterior part of the masticatory muscle at the angle of the mandible.

No more than 0.1-0.2 ml of filler per bolus or line. The procedure required 1.2 ml of the drug.

To volumize the deep malar fat pad and infraorbital fat, the doctor always uses a 23 or 25 gauge cannula and the product is injected supraperiosteally.

In this case, only 2 ml of – filler was needed to correct both cheeks. 1 ml on each side of the face.

0.4 ml of preparation was required for each temple, which was injected with a 27-gauge needle directly onto the bone.

Subdermal biostimulation technique involves passing a 23 or 25 gauge cannula into the superficial subdermal fat layer.

Cannula subcision is performed to release the superficial layers of the dermis and then inject a very small amount of filler – less than 0.05 ml per line.

Multi-vector lines (cross-hatch pattern) cover the entire target area.

The effect of biostimulation gradually increases over 6 months and lasts for about two years.

In the technique described above, the author treated the entire lower third of the patient's face, including the lips, with 2 ml of collagen stimulating filler.

The patient was completely satisfied with the results of the correction.
