This period of premenopause is characterized by a number of unpleasant symptoms that greatly reduce the quality of a woman's life.
The first clinical manifestations of premenopause, which most persistently attract the attention of a woman and make her think about visiting a doctor, are vegetative-vascular disorders (the so-called hot flashes, excessive sweating, palpitations) and psycho-emotional instability (increased irritability, nervousness, sleep disturbances) .
Monthly cycles become irregular and – more scarce. But the saddest thing for a woman during this period – this is a distinct fading of the skin, which progresses just before our eyes.
Estrogens and premenopausal skin
Deficiency of estrogen and somatotropic hormone against the background of the relative predominance of testosterone in the premenopausal period gradually leads to the development of unpleasant somatic and metabolic changes in the body and skin, which form the main clinical signs of aging.
Experts call the skin one of the most important target organs affected by sex hormones. The fact is that most of the components of the skin – cells of the vascular endothelium and hair follicles, fibroblasts and sebocytes, keratinocytes and melanocytes, Langerhans cells – possess receptors sensitive to sex steroids.
That is why it is so important that the sex hormones, which significantly affect the structural integrity and functionality of the skin, are produced by the body in adequate amounts. If the production of hormones is disturbed, the thickness and elasticity of the skin immediately changes, its hygroscopic properties decrease.
It is precisely because of the violation of estrogen production during the onset of premenopause that women notice increased dryness of the skin, the manifestation of its flabbiness, a decrease in elasticity and a loss of a clear contour of the face oval.
If we consider age-related skin problems at the pathophysiological level, then it can be noted that under the influence of impaired estrogen production, neurovegetative and microcirculatory dystonia develops in it, which causes the activation of free radicals, the development of hypoxia, and increased glycation processes in the skin.
These unpleasant processes lead to damage to collagen and elastin fibers, disruption of the production and absorption of hyaluronic acid, depletion of the lipid mantle of the skin – and therefore, to damage to skin cells, its flabbiness and exhaustion.
External aging of the facial skin due to lack of hormonal balance is due to the stratification of fat packs, stretching of the connective septa between them, gravitational ptosis of the soft tissues of the face, due to which the oval of the face "floats".
Depending on the severity and nature of the severity of involutional changes in the skin of women after 40 years, the main signs of aging are formed. - balance the regimen and nature of nutrition, increase the amount of polyunsaturated fatty acids and products containing phytoestrogens in food;
- drink clean water in the amount of at least 30 ml/kg of body weight per day;

Phytoestrogens for the prevention of premenopausal skin aging
Naturopathic methods to improve skin quality in premenopausal women may be used. Of particular interest are phytoestrogens – a class of plant bioflavonoids exhibiting hormone-like activity. It is believed that phytoestrogens normalize the functions of skin cells without interfering with hormonal metabolism and without reacting to estrogen receptors.
Phytoestrogens can also be found in food, but they are contained there in amounts negligible for the prevention of aging, so they can be recommended in the form of pharmacy phytoextracts, which have the status of dietary supplements and do not require a prescription.
In my clinical practice, in the complex aesthetic correction of early age-related skin changes, I use a naturopathic phytoextract (Klimapin, Ukraine) with specific phytoestrogenic activity. It contains the most famous and active flavonoid glycosides and phytoestrins: hawthorn fruits, hop inflorescences, nettle leaves.
They are complemented by herbs containing vitamins, organic acids, trace elements, triterpene alkaloids and saponins. All these substances improve microcirculation, reduce the stagnation of fluids in the skin, have membrane-stabilizing, immunostimulating, anti-inflammatory effects, and help balance the psycho-emotional state of a woman.
After 3 weeks of using the phytoextract (1/2 teaspoon 3 times a day), there are noticeable improvements in skin color and quality, a decrease in pastosity and the disappearance of edema in the eye area, the quality of sleep and general mood improved in patients. This course must be repeated 2 times a year, along with taking Omega-3 preparations, vitamins of group B, C, folic acid.
Since naturopathic phytoestrogens extracts slow down and soften the signs of aging in the skin, in my opinion – they certainly should take their rightful place in the practice of dermatocosmetology in the implementation of a comprehensive strategy for skin rejuvenation.
Thread techniques of rejuvenation in the correction of age-related ptosis of the skin and soft tissues of the face
Aesthetic correction of gravitational ptosis in women with "tired" and "gravitational" aging morphotypes is most effectively performed using mesotherapy, contouring and hardware cosmetology.
Unfortunately, each of these methods of correction individually has rather limited capabilities in terms of tightening sagging tissues and cannot reliably fix premenopausal ptotic skin.
To eliminate involutional ptosis of the skin and soft tissues of the face in patients over 40 years old, I use thread rejuvenation methods with zonal implantation of self-absorbable threads based on polylactic and caprolactonic acids with microprotrusions along the entire length.

I supervised 47 patients with involutive age-related facial skin changes in premenopause, and I had the opportunity to compare the results of correction of soft tissue ptosis using thread lifting in combination with systemic methods for improving skin quality and the results of monomethods of facial correction. Based on these observations, a method of an integrated approach to facial correction in women in the premenopausal period was developed.
Methodology of an integrated approach to facial correction in premenopausal women
The method includes three steps:
Step #1.
- Normalization of neurovegetative processes in the body, elimination of estrogen-associated microcirculatory disorders, increased antioxidant protection of the skin, membrane protection and creation of favorable conditions for the activation of fibroblasts and the synthesis of structural proteins:
- phytoestrogens and bioflavonoids (Klimapin, Ukraine) ½ teaspoon 2 times a day for 2 weeks,
- Omega-3.6 1 capsule once a day for 2 weeks,
- vitamin C 500mg per day for 10 days (3 days before threadlift cancel),
- biotin preparations (Wolvit) 1 tab per day for 2 weeks.
Stage №2. Thread lift, and reinforcement of the skin and soft tissues of the face.
Stage #3. Rehabilitation period. Elimination of pain, aseptic inflammation, edema and normalization of hemodynamics, microcirculation, creation of optimal conditions around the threads for antioxidant protection and fibroblast proliferation, collagen synthesis:
- phytoestrogen to continue for 2 more weeks,
- Omega 3.6 continue for 2 more weeks,
- Nimesil 1 sachet 2 times a day for 5 days,
- biotin preparations to continue for another 2 weeks,
- phytoextract with a pronounced anti-edematous, angioprotective, anti-inflammatory and immunostimulating effect (Venoton, Ukraine) 1 teaspoon 3 times a day for 3 weeks.
Due to its complexity, this approach to the treatment of age-related changes in facial skin improves the effectiveness of thread lifting and shortens the rehabilitation period.
