Radiesse is already known to cosmetologists as an effective and safe filler that allows successful & nbsp; make up for the lack of volume and fill deep folds on the face. However, scientific research on the action of the drug is ongoing, and today we can talk about its new properties to improve the quality of the skin and obtain a pronounced, safe anti-aging result.
Anastasia Saybel — PhD, international expert Merz Aesthetics.
Tell me, please, why do you think it is important to recall the features of the drug? After all, he is already known to our doctors.
– Yes, Radiesse has been known for a long time. Calcium hydroxyapatite in this drug was registered in Europe in 2004 and in 2006 the drug was approved by the FDA. But many of my colleagues still doubt the safety and efficacy of this drug, and today we are already talking about the use of Radiesse not only for indications: facial augmentation, filling in volume deficits and filling deep folds, but also talking about the vector technique. lifting and about the third effect of Radiesse – improving the quality of the skin, not only the face, but also the body. Therefore, today, in order for us to use this triple effect of Radiesse as efficiently as possible, of course, we need to remember the features of the drug. Firstly, its excellent biocompatibility, because calcium – it is a natural component of our body. It is part of not only teeth and bones, it is also found in muscles and skin. It is important that we once again realize the safety and in order for us to get the maximum effect. It should be noted that the period of biodegradation of the Radiesse preparation is about one year, this rate depends, firstly, on the volume of the administered drug, on the depth of administration and on the characteristics of the patient's body – from age, from the speed of metabolic processes. Therefore, in older patients, we sometimes see a little more biodegradation. In young, active patients, in athletes, degradation can be even less than 9 months, so they will need correction a little earlier. In general, on average, the drug gives an effect for a period of one year due to the volume and maintains the duration of the result up to one and a half, two years due to the fact that
Why is it important to know dilution techniques?– In order for us to receive only stimulation of the synthesis of new, normal collagen of the first and third type, we must really dilute the preparation with physical. solution to remove its volumetric properties, otherwise we will get the so-called overcorrection effect. Therefore, it is important for us to reduce the viscosity and elasticity of the drug, for this we add physical. solution.
What are the features of the methods of working with the drug?
– If we are talking about the reinforcement of the skin of the face and body, the so-called meso-Radiesse, then it is very important to understand that we are transferring the gel form of the Radiesse preparation into the so-called suspension, where there is no longer a carrier gel, but the liquid phase of the drug due to physical. solution containing microspheres of calcium hydroxyapatite. This suspension must be homogeneous during administration, so we always use large volume syringes in order to evenly mix the drug itself with saline. solution. During the procedure, we constantly continue to mix and control the clarity of the distribution of microspheres, the uniformity of the suspension during the injection.
Due to what mechanisms does the drug affect collagenesis? How does his breeding depend on this?
– Dilution depends not so much on the mechanisms of action of the drug, but on the thickness of the patient's skin, on the correction zone. And as for the mechanisms of stimulation of collagenogenesis, this was first proven for the standard form of the Radiesse drug, namely Radiesse, and not its generics, in studies that proved both biodegradation and the features of a prolonged effect. These are quite old studies, since 2004, but in 2013 Professor Yutskovskaya and Professor Kogan conducted studies, including those proving the stimulation of neocollagenesis in breeding. This was an additional study in volunteer patients using the ½ and ⅛, and it should be noted that in the ½ and ⅛, we saw an increase in the thickness of the dermis due to the formation of collagen of the first and third type in all patients, but the quality of elastin fibers changed better in the ½ dilution, rather than ⅛. There was also a qualitative improvement, but it is more pronounced in a more concentrated dilution. Accordingly, based on this study, we can subdivide the indications for meso-Radiesse. If we see only age-related skin atrophy in patients, then we can, let's say, save the amount of Radiesse and use a larger dilution of ⅙, ⅛. If we see signs of photoaging in the patient, and this is elastosis, then in this case the drug works better in a more concentrated dilution ½, ⅓. This is how we choose the amount of dilution.

– The fact is that in any case, regardless of age, we will still receive stimulation of neocollagenesis. And with age-related skin atrophy, we also work with other remodeling methods, including hardware cosmetology (laser procedures, RF-lifting, Ultera). All these methods through trauma stimulate the formation of new collagen and cause remodeling of the dermis. But the age patient and the patient especially in the postmenopausal period have one small problem. If the patient does not receive hormone replacement therapy and indeed the age is already, let's say, large, then in this case the fibroblast has already forgotten that it needs to produce something, including collagen. In this situation, in order for us to get the most pronounced restructuring of the skin structure, it is necessary to reinforce Radiesse, for example, 3-4 months before laser procedures, RF lifting, altera therapy. In some patients, after three months, we again reinforce Radiesse in dilution in order to further enhance the effect, and then we get an excellent result. Moreover, it should be noted that thread methods are still popular with aged patients.
But when the patient has flabby skin, age-related atrophy is pronounced, thread liftings do not give a beautiful, natural effect, and in this case, again, meso-Radiesse allows you to first rebuild the skin, and then fix it, using, for example, thread techniques .
What can cause side effects when administering the drug?
– As a matter of fact, as with the standard use of Radiesse, unwanted effects can occur when we misuse – primarily when we use it in non-recommended areas. Just like regular Radiess, meso-Radiesse cannot be used in the periorbital, perioral, forehead area. In the perioral and periorbital region, actively working muscles, and the introduction of the drug in this place can cause the appearance of nodules. Such nodules – it is an accumulation of calcium hydroxyapatite microspheres. There is simply no pronounced flabbiness in the forehead area, so this indication is not entirely relevant.
Then – incorrect depth of injection and the injection technique itself. In order for us to use the drug as efficiently and safely as possible, all trainings are carried out for this. In addition, sometimes a patient hides information about his state of health, about previous procedures – in this case, undesirable phenomena may also occur.
But in terms of various biological causes and adverse events, Radiesse is completely safe. It does not have immunostimulatory activity, since, let me remind you once again, it is a natural component of our body.
How can a first-time doctor deal with a drug avoid possible complications?
– Come to training! This is the best. Practice, practice and more practice. We all once studied, & nbsp; everyone once started, but it is better to do it under the guidance of an experienced coach who will tell you and help you. In the presence of the patient, we never make any incorrect remarks, we simply try to direct the doctor, and we pronounce all the nuances in the absence of the patient in order, again, to preserve the reputation of the doctor and the drug.
What mistakes would you like to warn doctors about while working with the drug?
– First, we need to clearly select patients, because we still have patients with high expectations – this primarily concerns age-related patients with severe gravitational manifestations, for example, according to the Merz Aesthetics aging scale, these are stages 3 and 4. In this case, you need a plastic surgeon. Radiesse, both in standard dilution and meso, will only be an addition to this aesthetic correction.
Therefore, in my opinion, the selection of patients is important first of all, it is important to choose the right injection time, choose the area, and, accordingly, inject the drug technically correctly.
What results can a doctor achieve with the correct use of the drug? – Magnificent! Simply wonderful, because this unique triple effect is then seen as a qualitatively new skin, and meso-Radiesse perfectly complements the volumetric correction. I really hope that in the near future you will also have the Belotero line. In patients with muscular type of aging, when there is no tendency to edema, we can correct deep volumes with a filler based on stabilized hyaluronic acid (Belotero Volume), and improve skin quality using meso-Radiesse. In this case, we, again, will get a wonderfully pronounced and safe result.
