How to calculate the amount of adipose tissue that needs to be taken from donor areas for subsequent transplantation?

For the correction of the face and hands, fat tissue taken from the thighs is quite enough. I try to extract a layer of fat from the front and inner thighs about the thickness of a palm - about 5 mm. In this case, there is no subsequent sagging of the skin in the postoperative period. With lipofilling of the breast and other areas, more adipose tissue might be needed, but this issue must be assessed in each case individually.

How do you purify the resulting adipose tissue before transplantation?

Today, many surgeons are phasing out centrifugation of fat cells, and I agree with them. This method of cleaning destroys too many cells, so I prefer filtration. For this we take a dense fabric with cotton and nylon fibers, where the cell size is about 0.5 mm. Laboratory analysis shows that after pushing the adipose tissue extracted and cleaned with a special solution through a napkin, all cells remain completely intact, viable and suitable for transplantation.

Are there any peculiarities in the preparation of adipose tissue for lipofilling of various parts of the body?

Yes, I use nanofat for lipofilling of the face,because they are best suited for filling wrinkles, furrows and areas of the face that have lost volume. To get nanoparticles, I use a special device with a diameter of 1.5 mm, through which I pass fat microparticles about 30 times. For lipofilling of the hands, I use microfats, because they are better for adding volume.

What should I pay special attention to when doing facial lipofilling?

When distributing adipose tissue under the lower eyelid, the cannula must be inserted very superficially and very carefully, trying not to create excess fat. When injecting fat into the cheekbones, the syringe should be held at a 90-degree angle to avoid the formation of "sausages", and lightly massaged with your fingers to better distribute the tissue. Excessive injection of adipose tissue in this area should be avoided, because there are a lot of facial muscles here, and an excess of tissue threatens to disrupt facial expressions, the face will become inanimate.

What are the features of introducing fat into the hands?

I do not adhere to any precisely defined hand treatment pattern, I determine the technique of fat injection depending on the circumstances - the shape of the hand, the quality of the skin, the degree of volume loss. For a better distribution of adipose tissue, I always help with finger massage when inserting.
